As associates who work at a Medical Center and as health care consumers we are keenly aware of these issues. As an employer, we have a responsibility to control costs while continuing to provide a competitive, comprehensive compensation and benefits program that meets the needs of our diverse workforce and provides the best value for the dollars you and Montefiore spend. In the face of our own rising health care costs, we are making a few adjustments to the Associate Benefits Program. We believe that these changes best meet the needs of the vast majority of our associates and allow Montefiore to meet its overall objectives. You ll find an overview of the changes that will take effect on January 1, 2010 in this Newsletter. The Annual Benefits Election Period begins November 16th and ends December 1st. 2 Montefiore Human Resources New, Easy-to-Use Enrollment Materials During Enrollment, Don t Forget Personal Information If you have had a change of name, address, or phone number during 2009, you should notify HR on a Change of Information Form. Health Care Make sure you record each family member you wish to enroll in health care coverage! All enrolled family members must have their Social Security Numbers on file to be eligible for coverage. Flexible Spending Accounts You must make a new Health Care and/or Dependent Care FSA election if you want either or both of these accounts in 2010, your current contributions will not roll over into next year. Life Insurance Beneficiary Make sure your beneficiary designations are up-to-date for Life, AD&D and Supplemental Life Insurance. You will notice that this year s annual benefits election materials have a new look and feel. We have redesigned these communications to support the go-green initiative using less paper and presenting your benefits information in a more cost-effective, user-friendly format. Your 2010 Annual Benefits Election Kit includes: A new Benefits CD. This innovative approach makes it easier to access the information you need, provides links to MonteBenefits and other online resources including most of our benefit vendors. If you need a paper version of the contents of the Benefits CD or a replacement CD, please Montefiore s HR-Benefits Office at montebenefits@montefiore.org. A Medical Overview, which provides a side-by-side summary of the key features of each medical option including HMOs. This comparison should make it easier for you to review your medical options and choose the option that will work best for you and your family. We Want To Hear From You! 3 Health Care Premiums Increase in MonteCare PPO Premiums As the cost of medical coverage increases, our salary-based premium contribution structure assures that MonteCare PPO coverage remains relatively affordable for associates at all income levels. Over the past years, Montefiore has been able to maintain and improve its health care programs and generally hold the line on premium increases. However, as a result of increased utilization and claims costs, the bi-weekly premium contributions for full-time and part-time Associates will increase for 2010 to the levels shown below Bi-weekly Premium Contributions Salary Band Full-time Associates Part-time Associates You Only You + Your Family You Only You + Your Family Under $60,000 $49.50 $17.50 $ $ $60,000 $99,999 $59.50 $ $ $05.11 $100,000 $149,999 $64.50 $ $11.64 $15.01 $150,000 and over $69.50 $19.00 $117.1 $25.27 HMO Premiums Bi-weekly HMO premium contributions will also increase for 2010 as shown below. Contributions are the same for each of the available HMOs based on whether you elect single or family coverage. You Only You Pay $15.50 You + Your Family You Pay $57.40 Reduced Dental Premiums 2010 Bi-weekly HMO Premium Contributions (for full-time Associates) Your cost for dental coverage is based on the option and coverage level you elect. You pay the full cost of dental coverage during your first year at Montefiore. After one year, Montefiore begins to subsidize the premium for the Preventive and Diagnostic Care and the Indemnity/ PPO Dental Plans. Bi-weekly Premium Contribution You Only You + Your Family Did You Know? Montefiore has only increased the associate premium contribution for the PPO twice in the last five years including Even with the increase in 2010 contributions, associate contributions for MonteCare will still be at or lower than the median contributions being paid at other surveyed Greater New York Hospital Association (GNYHA) hospitals for generally comparable plans. 4 Montefiore Human Resources Medical Care Increased MonteCare Cost-Sharing There is no change in copayments if you use Montefiore facilities or receive professional services from a physician in the Montefiore Integrated Provider Association (MIPA) or Montefiore Behavioral Care Integrated Provider Association (MBCIPA). However, you may pay somewhat more out of your own pocket if you obtain out-ofnetwork care or use a non-mipa physician. Here s what s changing: Higher Out-of-network Deductible the annual deductible for out-of-network care will increase to $500 for an individual; $1,250 for a family. The deductible is the amount you must pay before benefits for certain out-of-network covered expenses are paid. The deductible applies to each covered individual once each calendar year. Higher Office Visit Copayment if you see an Empire BlueCard PPO Network primary care physician (non-mipa), the office visit copayment will increase to $25. New Emergency Room Copayment If you are enrolled in the MonteCare PPO and use the emergency room, you will pay a $50 copayment (whether the hospital is in-network [Montefiore included] or out-of-network). If you are admitted to the hospital from the emergency room, the copayment will be waived. Updated Mental Health/ Substance Abuse Benefits To comply with the Mental Health Parity & Addiction Equity Act of 2008, the MonteCare PPO will provide the same benefits for inpatient and outpatient Mental Health/Substance Abuse benefits as they do for any other condition. There will no longer be limits on frequency, number of visits or days of coverage. New Health Care ID Cards MonteCare PPO and HMO participants will be receiving new ID Cards for When you log on to the MonteBenefits website to enroll, be sure to check your address so your new ID Cards are mailed to the correct address. If you have had a change of name, address, or phone number during 2009, you should notify HR on a Change of Information Form (a copy is located under the Resource section on the Benefits CD). Forward the completed form to the HR-Benefits Office by montebenefits@montefiore.org or Fax (914) Health Care Continuation for Students on Medical Leave of Absence Recent legislation called Michelle s Law provides you with the opportunity to continue medical coverage for an eligible dependent student who would otherwise lose student status under the plan while on a medically necessary leave of absence. Coverage can generally continue for up to one year after the absence starts. Expanded Dependent Coverage for HMO Participants To comply with recent New York State legislation (New York State Coverage Expansion through Age 29 [Young Adult Option]), an unmarried child, who is no longer eligible for dependent coverage under your HMO because of age may be eligible to purchase individual coverage under the HMO group plans offered at Montefiore as long as your HMO coverage continues. To participate, your child must be unmarried, age 29 or younger, not insured or eligible for both medical and hospital benefits through his or her own employer and no longer eligible for dependent coverage. Your child does not have to live with a parent, be financially dependent on a parent or be a student to qualify. If your HMO coverage stops for any reason, your child loses the right to purchase individual coverage. To obtain additional information, you should call the HMO. When you call, be sure to use the full name of the HMO. HMOs offer many different plans and you want to be sure you obtain information about the specific plan offered through Montefiore. 4
5 2010 Prescription Drug Benefits New Features Added to Prescription Drug Benefits Prescription drug costs continue to climb at a faster rate than many other areas of expense. Every year, we look for ways to better curb these costs without compromising benefit levels. After considering many alternatives, we are adding several new utilization management features that will, among other things, help control prescription drug costs for you and Montefiore and, at the same time, continue to provide access to the most effective medications. Here is an overview of these new features. Enhanced Concurrent Drug Utilization Review This new feature is designed to help control costs by better regulating the timing between prescription refills. You can continue to refill your prescriptions but you must use at least 75% of the prescription before the refill will be approved. If you need a prescription refill sooner for example, if you will be traveling you can call Medco for a waiver. Preferred Drug Step Therapy (PDST) This new feature requires you to try a generic alternative or preferred brand name medication before a higher cost non-preferred drug is used. Generic alternatives are generic drugs that are approved by the FDA for the exact same use, or for similar uses, when treating a particular medical condition, i.e., where they are clinically interchangeable. Generic alternatives are not the same as generic equivalents (which have the same active ingredients) but they treat the same conditions. If your doctor prescribes a non-preferred drug, Medco will work with your doctor to see if a generic alternative or preferred brand name medication would be equally effective. (In some cases, special circumstances may require you to use a non-preferred drug.) This new feature generally applies to the following drug categories: gastrointestinal, anti-depressants, osteoporosis, sleep medications, intranasal steroids, and migraine medications. Medco will notify you if you are currently taking non-preferred drugs that have lower cost alternatives available. Note: If your prescription history shows that you have already tried preferred drugs, your prescription will be filled without a review. Drug Specific Prior Authorization This new feature insures that drugs are being used for their designed purpose. If you are taking any of the following classifications of drugs, Medco will review your prescriptions and contact your doctor to determine if your prescription qualifies for drug coverage based on nationally accepted clinical guidelines and standards: dermatologicals, erythroid stimulants, human growth hormone, interferons, immunomodulatory agents, Multiple Sclerosis therapy, myeloid stimulants, select androgens and anabolic steroids. If you are taking drugs that require prior authorization, Medco will notify you and provide you with instructions for beginning a coverage review. Revised Prescription Drug Copayments To encourage you to use generic drugs whenever they are available, the copayments for generic retail and mail order prescriptions will decrease. All other prescription drug copayments will increase. The 2010 copayment amounts are shown in the following table. MonteCare PPO/HMO Participants Copayments Medco Participating Retail Pharmacies Medco By Mail Program Generic $10.00 $20.00 Brand Name $5.00 $50.00 Non Formulary (Brand Name or Generic) $45.00 $60.00 Important Note: You can continue to fill your prescriptions (up to a 90-day supply) at Moses FCC and Weiler/Einstein outpatient pharmacies at no cost to you and your covered family members. Employee Assistance Program (EAP) The Employee Assistance Program (EAP) is a confidential resource that provides assistance with a range of personal issues. Examples include marital, parent-child, legal or financial difficulties, stress, depression, anxiety, grief reactions, substance abuse, or any other emotional or behavioral problem. The Program, which was introduced early in 2009, is available to you and your family. The EAP is provided at no cost to you. Contact Longview Associates at (800) or online at 5
6 Montefiore Human Resources Improved Dental Benefits We are changing the name of the $1,500 Annual Maximum Dental PPO Plan to the Indemnity/PPO Dental Plan and improving the benefits it provides. If you use a dentist in Montefiore s Department of Dentistry, you may receive an additional $500 in benefits each year. You are still free to use any dentist you choose in or out of the Empire Dental Premium Care PPO Network. Each covered person may receive up to $1,500 in annual benefits for preventive and diagnostic, basic and major services combined. The annual maximum increases to $2,000 if you use a Montefiore dentist. Introducing Non-tobacco User Savings for Supplemental Life Insurance Tobacco use translates into additional health risks and increased dollars being spent on health care and has an adverse effect on overall life expectancy. Beginning January 1, 2010, if you are a non-tobacco user, you enroll and answer No to the tobacco use question, you will receive a 10% discount on your Supplemental Life Insurance premiums. You are considered a non-tobacco user if you have not smoked cigarettes or cigars, used pipe tobacco or chewing tobacco during the 12 months immediately before October 1, If you are enrolled for Supplemental Life Insurance, you must make a new election during this annual election period to receive the non-tobacco user savings (a 10% discount on Supplemental Life Insurance premiums). If you do not enroll, your Supplemental Life Insurance premiums will continue unchanged even if you are not a tobacco user. Failure to accurately respond to the tobacco use question is a policy violation that may result in loss of benefits and future forfeiture of Supplemental Life Insurance proceeds. 6
7 2010 Montefiore Medical Center Voluntary Tax Deferred Annuity 40(b) Plan Maximum contributions are determined by the IRS and are adjusted annually. Plus, there are two catch-up opportunities: If you are age 50 or older and you contribute the maximum permitted amount, you are eligible to make an annual Age 50+ Catch-up contribution If you have worked for Montefiore for at least 15 years and your previous Voluntary Tax Deferred Annuity 40(b) contributions did not exceed an average of $5,000 for each year of service, you may be eligible to contribute up to a maximum of $15,000 of 15+ Years of Service Catch-Up contributions, limited to $,000 in any one year. If you are eligible for both types of catch-up contributions, the dollars that you contribute as catch-up contributions will first be treated as 15+ Years of Service Catch-Up contributions to the maximum extent permissible, then as Age 50+ catch-up contributions. Maximum Voluntary Tax Deferred Annuity 40(b) contribution amounts for 2010 are shown below. If you: Are under age 50 $16,500 Qualify for the full annual 15+ Years of Service Catch-up Contribution Qualify for the Age 50+ but not the 15+ Years of Service Catch-up Contribution Qualify for the Age 50+ and the 15+ Years of Service Catch-up Contributions The maximum you can contribute is: $19,500 (16,500 + $,000) $22,000 ($16,500 + $5,500) $25,000 ($16,500 + $,000 + $5,500) At any time during the year, to elect the Voluntary Tax Deferred Annuity 40(b) Plan, change your election and/or if you are eligible for and want to make a catch-up contribution, contact Principal at (800) or go online to 7
8 Montefiore Human Resources 2010 Annual Enrollment Checklist Use this Checklist to prepare for annual enrollment. Complete each section so you have the information you need when you enroll. Log on to Montefiore s Benefits Website or call the Benefits Enrollment Call Center (888) , Monday through Friday between 8am and 8pm EST. You ll speak to an enrollment specialist who will help you enroll. Review your current information (address, phone number, etc.) and notify HR of any corrections on a Change of Information Form (a copy is located under the Resource section on the Benefits CD). Answer the tobacco use question for Supplemental Life Insurance. Provide the name, date of birth and Social Security Number of each family member you wish to enroll in health care coverage. Write your dependents information here: Designate and/or update your beneficiary for Basic and Supplemental Life, AD&D and BTA Insurance. Write your beneficiary(ies) names and Social Security Number(s) here: Choose your coverage. Review your choices, determine your costs, and then make your selections. Your Username: Your Password: Answer Yes if you have smoked cigarettes or cigars, used pipe tobacco or chewing tobacco at any time during the 12 months prior to October 1, Answer No if you are a non-tobacco user. It describes benefit changes as well as additional Plan administrative information required by the Employee Retirement Income Security Act of 1974 (ERISA) and the Health Insurance Portability and Protection Act (HIPAA). You can find additional information about the Associate Benefits Program on the Benefits CD and on Montefiore s Benefits Website at The actual provisions of the plans that make up the Associate Benefits Program are governed by the legal documents for each. If there is a discrepancy between the information presented here, in the Summary Plan Description and the legal documents, the legal documents will govern.
