4 Enrollment made easv. y DENALI DENTAL & VISION MAKES IT QUICK AND EASY TO ENROLL YOUR CLIENTS IN THE DENTAL PLAN OF THEIR CHOICE. When enrolling a client make sure you use your dedicated link. If you are using your dedicated link you should see your name and phone number at the top of the quote page (after you enter in the quote information). Below is a Step-By-Step process of an enrollment. 1 Enter your client s date of birth and ZIP code. Select the coverage needed (Applicant, Applicant + One, Applicant + Family, or Child Only). Select Effective Date. - Enroll by 5pm (Central) on the 25th for an effective date of the 1st. - Enroll by 5pm (Central) on the 4th for an effective date of the 15th. Then select NEXT. Your information as the agent will appear in this bar. This ensures that you will get credit for the enrollment. 2 View available plans and select an option. Plans vary by state and only available plans for the ZIP code entered will be displayed. Once a plan is chosen, click ENROLL NOW. After you click ENROLL NOW, a box will pop up and you will be given the choice to add the optional vision plan for your client.

5 3 Enter personal information for your client. If you added a spouse or dependent in Step 1, fill in their information in the Spouse or Domestic Partner box. If dependents, click ADD DEPENDENT and enter their personal information. If the member does not have an address, use your address, you will get the confirmation and can print for your client. Click SAVE AND ENTER PAYMENT INFO. 4 5 Choose billing option and enter your client s payment information. Read Authorization Agreement, Limits and Exclusions and Fraud Notices. 6 7 Click to agree to the Authorization Agreement and Important Fraud Notices. Click SUBMIT. CONGRATULATIONS! 10 Why Should You Choose a Renaissance Network Dental Plan? In addition to paying lower monthly premiums, the Renaissance network can help reduce your out-of-pocket costs. Network dentists have agreed to accept a set contracted amount for each service rendered as the basis for payment under the Denali Dental Plan. This amount is typically significantly less than the amount which could be charged by an out-of-network dentist. These network dentists are prohibited (by contract with the network) from charging you the difference between their typical fee and the amount negotiated with the network. Dentists not participating in the network are not subject to the negotiated amounts and are permitted to charge any fee for services they provide. This may lead to greater out-of-pocket costs for you and your family members. The sample comparison chart below will give you an idea of how you can save money by selecting one of Denali Dental s network plans and visiting an in-network dentist for services. It compares the charges between visiting in-network and out-of-network dentists.
