2 Purpose of the Tracker Help make utilities safer The esafety Tracker will help utilities document and analyze safety-related events o Meetings, job briefings, injuries, near-misses, investigation reports, etc Utilities will be able to run reports on their incident history and perform MUCH better benchmarking o Ex: Causes report, OSHA 300 reports, Monthly report o Better data allows for a better understanding of root causes of incidents 2

15 Walk Through Pilot version of the Safety Tracker is in progress Mock-up images will be presented to show the various features Basic information: o All safety events are either meetings or incidents o Injuries, Near-Misses, Fatalities, Investigations, and Property Damage fall under Incidents o Safety Meetings and Job Briefings are managed separately o Utilities can customize forms to suit the information they need to record o Utility profile and user information is also managed separately o Reports aggregate the records entered by the utility 15
16 Home Page Access all features of the Tracker: recording new safety meetings or job briefings, record and view incidents, Safety Manual and manage utility/user profiles These images assume an admin view of the Tracker 16
17 Meeting/Briefing Clicking the Meeting/Briefing tab, brings you to a page where you can record a new meeting Sub-navigation bar allows users to toggle between recording a new meeting or briefing 17
18 Meetings Basic information to be captured: meeting date/time, speakers, participants, description/topic 18
19 Meetings Utility can also include any relevant additional information or custom field* *Custom fields would be managed through the utility s profile o Would appear on all meeting forms for the utility 19
20 Reportable Feature This is a way for a utility to denote whether a completed record can be used in running analyses in the Reports tab Default option would be Yes for all record types Certain records would be required to have Yes marked (ex: injuries with days missed or restricted from duty, or fatalities) o The Tracker would be able to detect this automatically 20
21 Briefings OSHA requires a briefing to be conducted before each job o (c)(2) Subjects to be covered. The briefing shall cover at least the following subjects: hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements Required section for all utilities: 21

24 Incidents Incidents serves as the umbrella category for the following items: oinjuries, Near-Misses, Fatalities, Investigations, and Property Damage Basic information about an incident should be the first thing that is recorded oex: time/date of the incident, weather, location oonce this is entered, the specific record type can be selected othat way all records for one event can be linked 24
25 Incidents View of the incidents page: o Existing records can be reviewed, and new ones can be created 25
26 Incidents When creating a new incident, you follow a form wizard to walk you through the recording process 26
27 Incident Record Type: Investigation Select an incident record type. Many utilities may choose to start with an investigation, but any can be selected Next steps in the wizard will collect the relevant information for each record type 27
28 Investigation Form Allows the utility to collect information on how an incident occurred, if proper instructions were provided, etc. In the Basic Investigation Details section, information on when and who is collected 28

30 Investigation Form - Causes Allows the utility to indicate what was the primary/underlying cause of the incident Example cause options: Unsafe behavior: Proper safety procedures not followed, Personal Protective Equipment not worn, Improper Physical or Mental Condition Safety Procedures Not Followed: Job briefing inadequate or not held, Inadequate supervision, Inadequate utility safety policies 30

32 Incident Record Type: Injury Select an incident record type. Fatalities can be noted under Injury 32
33 Injuries Injuries can be categorized at 3 levels in the Basic Details section of the reporting form: Minor (could be considered not reportable) Severe (likely the most commonly used option) Fatal (if fatality occurs at the time of event) 33
34 Injuries Next section allows the user to indicate each section of the body that was affected, what the injury type was, and how it was treated This might reveal trends in injury types Selections include left/right, and back/front options Options change based on the area selected Ex: If ear is selected, options would include loss of hearing, while if eye is selected, loss of sight would be an option Treatment options are very broad (should not include detailed medical information) 34
35 Injuries Next section collects information for use in the OSHA 300 form Brief description of the incident and location Cause and context can also be provided 35
36 Injuries Additional information and files can be collected, and customized fields can be added Note: days missed/restricted from work would be collected through an Injury State Change form Allows for careful tracking of how the employees health might change and accurate accounting of days missed A state change form can only be entered if an initial injury record exists Examples of information collected: Date of health change If the employee misses days of work If employee goes from days away from work to days of restricted duty or on job transfer If an employee s health worsens to become a fatality 36

38 Near Misses For any incident where an affected person is not injured, but was close to being hurt Collects all the same information as an injury form, excluding injury information/affected body parts Like all other forms, custom fields can be added 38

40 Property Damage Allows the utility to record any damage that occurs to utility property or to non-utility property Basic information about the damage can be given, and more details can be provided in the Additional Information section 40
41 Safety Manual APPA Safety Manual can be viewed digitally through the Safety Tracker Every copy of a Manual purchased by a utility = 1 version that can viewed by a user Views/access to the Manual would be managed by the utility admin user o If a person leaves the utility, their Manual view could be allocated to a different employee (just as their hard copy version might be given to someone else) Licensed versions of the Manual could be accessed as well 41
