3 OVERVIEW: The Occupational Safety and Health Act of 1970 (OSHA) requires most employers to maintain accurate records of work-related injuries. CareMc tracks claim information needed for OSHA forms 300, 301, and 300A information. CORVEL S OSHA RESPONSIBILITIES: CorVel will complete the required fields in CareMc in order to produce the 300, 301 and 300A forms. The Program Member will have the responsibility to review all reports for accuracy. All claim specialists will be trained on OSHA reporting expectations and documentation guidelines. Claim specialists are not OSHA experts and CorVel will work with each member making edits where necessary. FORMS 300 and 301: Several fields automatically populate from system data, to include: Case No (Claim Number) (A) Employee s Name (B) Date of Injury or Onset of Illness (D) Case Classification (G-J) Number of Days the injured worker was away from work or on job transfer (K-L) Full Name (1) Address (2) Date of Birth (3) Sex (5) Case number from log (10) Date of Injury or Illness (11) Time of Event (13) 3 The claim specialist is required to complete the following OSHA data in CorVel s claim system, CareMC. Job Title (C) Where the Event Occurred (E) Description of Injury or Illness (F) Injury or Illness Type (M) Date Hired (4) Name of Physician or other Health Care Professional (6) Time Employee Began Work (12) What was the employee doing just before incident occurred? (14) What happened? (15) What was the injury or illness? (16) What object or substance directly harmed the employee (17) If the employee died, when did death occur? (18)

4 DAYS AWAY FROM WORK: The claim specialist must manually document restricted duty, the waiting period (3 days) or whether the employee has lost no time in CareMc s Lost Days Tracker, which is where the Days Away from Work data in OSHA s form 300 is pulled from. Indemnity payments, to include Temporary Total Disability, Temporary Partial Disability, and Salary Continuation via voucher, are automatically included in the Lost Days Tracker and subsequently, the Days Away from Work data as they are pulled from the claim s payment register. Please note that the Lost Days Tracker and the 300 form, will not include up to the minute information due to information transfer delays in obtaining medical records and disability slips. REPORTS: CorVel will provide the OSHA 300 and 300A to each Program Member quarterly at agreed upon delivery dates after the close of each quarter. The 301 will be provided upon request. If the Program Member wishes to make changes after the posting deadline has passed, CorVel will work with the Program Member to update the data upon notification by said Program Member. Please note that CorVel will be reviewing the report on a monthly basis for quality assurance purposes. MCSIP s OSHA RESPONSIBILITIES: Each Program Member must review all OSHA documents carefully and in their entirety upon receipt of same from CorVel. If any changes need to be made, the Member must notify the CorVel Account Manager regarding said changes. Once the changes are made, CorVel will reproduce the document and forward same to the Program Member. The Program Member will then complete the OSHA filings in accordance with the law 4 OSHA REPORTS PARAMETERS AND FILTERS: Parameters Location for Establishment Name click True to show the Location City and State instead of the Company Proper Name in the Establishment Name line. Location Level 1 must be selected on the Groups tab. Show Page Number by Group (OSHA 300 only) click True to include a page number count for each different group.

6 Filters Claim Number enter a specific claim number to generate report on a single claim WC Claim Type generate reports based on Indemnity, Medical Only or Report Only claim type. Both filters can be applied to a report. OSHA 300/301 information is available under Claim Summary Reference Links. 6 The OSHA flag on the Claim Summary Claim Info tab shows whether the claim is OSHA reportable.

7 OSHA 300A New Parameters New Parameters You can now manually input additional regulatory data into the OSHA 300A claim report. The new input parameters generate in the report when one run report location is selected. Average number of employees Total hours worked (by all employees) North American Industrial Classification (NAICS) code Standard Industrial Classification (SIC) code 7 PLEASE NOTE: If more than one run report location is selected, a DATA WILL NOT BE INOUT TO THE REPORT notification messages appears.

8 OSHA 300A New Excel Version You can now generate an Excel version of the OSHA 301 report. OSHA Report Descriptions & Examples OSHA Form 300 This is the OSHA Log of Work-Related Illness and Injuries. Federal law requires employers to record all reportable injuries and illness that occur in the workplace on this form. 8

9 OSHA Report Descriptions & Examples, continued OSHA Form 300A This is the OSHA Summary of Work-Related Injuries and Illnesses. Federal law requires employers to post this form in the workplace annually. 9 OSHA Form 301 This is the OSHA Injury and Illness Incident Report which is required by the Unites States Department of Labor s Occupational Safety and Health Administration (OSHA) for employer provided workplace incident records.
