2 HEALTH CARE QUALIFYING HOURS ENROLLMENT You are receiving this enrollment guide as our records indicate that you have met the required hours of service to be eligible for health care benefits through The Home Depot for the 207 plan year. Under the plan, new part-time associates who work on average 30 hours or more per week during a -month initial look-back measurement period are eligible for health care coverage. This guide will give you high-level information about these benefits. For complete benefit details and eligibility rules, see the 207 Benefits Summary for Health Care Qualifying Hours Associates at livetheorangelife.com. These health care benefits are offered in addition to the standard benefit offering as a part-time associate. For complete details of the benefits offered to you as a part-time associate, see the 207 Benefits Summary for U.S. Part-Time Hourly Associates at livetheorangelife.com. 6 HEALTH CARE SPENDING & SAVINGS ACCOUNTS SAVE ON ELIGIBLE HEALTH CARE EXPENSES. A Quick Look at the Spending Accounts Health Care Spending Account IN 207 YOU CAN CONTRIBUTE: $260 minimum up to $2,600 maximum FOR ELIGIBLE EXPENSES YOU INCUR BETWEEN: January, 207 March 5, 208 HOW YOU SAVE MONEY USING THE SPENDING ACCOUNT IF YOU CONTRIBUTE $,000/YEAR ($38 BI-WEEKLY) TO AN ACCOUNT, YOU LL SAVE AT LEAST: $266 a year When you pay for eligible health care expenses such as deductibles, coinsurance, copayments and prescription drugs through this account, you are using before-tax dollars, which are put into your account before taxes are taken out of your paycheck. The amount you save depends on your tax bracket and the tax rate in your state. So, if you re in the 5% tax bracket and you also pay the 7.65% Social Security/Medicare tax, you could save as much as 22.65% on expenses you pay for through the accounts. If you pay a state income tax or are in a higher tax bracket, you ll save even more. 4 The Health Savings Account (HSA) Option THE HEALTH SAVINGS ACCOUNT (HSA) IS AVAILABLE TO YOU IF YOU ARE: Enrolled in the Silver Plus High Deductible Health Plan or the Bronze High Deductible Health Plan; and Not covered under another medical plan that is not an HDHP such as your spouse s medical plan. Home Depot Provides Matching Contributions to Your HSA. Instant Access to Your HSA Using the YSA Debit Card. No claim forms, no paying out-of-pocket when you use your YSA card, your eligible expenses are deducted automatically from your health care account. Your HSA Earns Interest. To find the Tobacco-User payroll deductions (which are on average 5% higher), visit livetheorangelife.com. Associates covering three or more children will pay an additional $25 per biweekly pay period. 2 Kaiser Permanente (KP) HMO plan is only available in certain areas, based on your ZIP code. The KP HDHP is only available in certain California ZIP codes. 3 You can contribute an additional $,000 if you are age 55 or older. For weekly payroll deductions, take the biweekly payroll deductions and divide by 2. In some instances your paycheck may not be enough to cover the entire amount of your benefits premiums. In those cases, the amount of the premium above your paycheck is still owed and will be collected from your future paychecks. TOBACCO-FREE STATUS Don t forget to make your Tobacco-Free Certification. The Tobacco- Free definition that we use aligns with the FDA s Center for Tobacco Products definition. The Medical Plan recognizes the use of tobacco products as any of the following: Cigarettes Cigars Dissolvable tobacco products Electronic Delivery Systems (such as e-cigarettes/vape pens) Pipes Smokeless tobacco Roll-your-own tobacco 5 If you do not certify that you are Tobacco-Free, you will automatically receive Tobacco-User Medical payroll deductions. PLEASE NOTE: After the 207 Health Care Qualifying Hours Enrollment Period you will not be able to make a change unless you experience a Qualifying Life Event. This is a requirement by the Federal Government. To qualify for the Tobacco-Free Medical payroll deductions, you must actively certify during your enrollment that you are at least 4 months Tobacco-Free (or have never used tobacco) and will continue to be Tobacco-Free in 207. If you think that you are unable to meet the Tobacco-Free standards, contact the Benefits Choice Center at and we will work with you (and, if you wish, your doctor) to find a reasonable alternative to allow you to earn the lower payroll deductions.
