1 Your 2011 Benefits Orientation Guide For Permanent Hawaii Salaried, Full-time and Part-time Hourly Associates

2 Welcome to The Home Depot! We re glad you decided to put on the orange apron and bring your skills and know-how to our Company. When your apron comes off at the end of the work day, your Home Depot benefit plans, programs and services keep working for you to help you live your best possible life what we like to call the Orange Life. Now it s time for you to start enjoying the benefits of your new Orange Life. 3 NEW ASSOCIATE ENROLLMENT MUST-KNOWS You MUST enroll on the Your Benefits Resources Web site or by calling the Benefits Choice Center by your enrollment deadline: For full-time and part-time hourly associates: before your 29th day of employment For salaried associates: before your 31st day of employment You MUST actively enroll in Medical 1, Dental, Vision, Critical Illness Protection Plan and/or the Spending Accounts 2 or you will not have coverage in these plans for the rest of You MUST designate your beneficiary(ies) for your life insurance coverage online during your enrollment session. Full-time and salaried associates also must designate a beneficiary for AD&D coverage. Remember, all full-time hourly and salaried associates have Basic AD&D coverage and must designate a beneficiary online for this coverage. 1 If you don t go online and choose an option, you will automatically be enrolled in associate-only coverage under the HMSA PPO. If you don t want medical coverage through Home Depot, an HC-5 form will be mailed to you once you elect no coverage during your enrollment. 3 Your Home Depot Benefits AVAILABLE TO ALL ASSOCIATES Health Care Benefits Remember: You Must Enroll By Your Enrollment Deadline to Have Medical Hospital Indemnity Plan (available to dependents of part-time hourly associates only) Critical Illness Protection Plan Dental Vision Other Benefits FutureBuilder 401(k) Savings Plan FutureBuilder can help you save money for your future through convenient payroll deductions. After one year of service 1,000 hours in a 12-month period) you can begin receiving a Company matching contribution. Employee Stock Purchase Plan (ESPP) The ESPP allows you to set aside up to 20% of your eligible pay (subject to certain tax limits) to purchase The Home De MetLaw Legal Services MetLaw provides coverage for legal services, such as wills and estate planning documents and real estate matters at a discounted price. IMPORTANT: To have coverage in 2011 you will need to enroll by your 61st day of employment. Learn about these benefits and more at Best Doctors Program Auto and/or Home Matching Gift Program 529 Savings Tool Tuition Reimbursement (not available to part-time associates) Adoption AVAILABLE TO FULL-TIME HOURLY AND SALARIED ASSOCIATES Health Care and Dependent Day Care Spending Accounts Disability To help provide financial protection if you become disabled and are unable to work because of illness or injury. Full-time hourly associates are automatically enrolled in long-term disability coverage* and will have payrol tions begin on your 91st day of employment unless you decline coverage. If you decline, Evidence of Insurability will b for future enrollments. Salaried associates are automatically provided Company-paid short-term disability (S long-term disability (LTD) coverage. Salaried associates are defaulted into the LTD Tax on Plan Cost option. Th that the cost of the LTD coverage that the Company pays for you will be charged to you as income and you will pay taxes on the LTD cost. By doing this, if you were approved for LTD, then your benefit would be a tax-free benefit. Yo change this option during your enrollment session. * Full-time associates in Hawaii have short-term disability coverage through the state disability plan. Company-Paid Basic Term Life Insurance The Company provides you with basic life insurance (if you participate in a Company medical plan) Associate and Dependent Voluntary Term Life Insurance You ll have an opportunity to purchase additional life insurance coverage, in multiples of pay, that best meets your need Company-Paid Basic Accidental Death and Dismemberment (AD&D) The Company provides you with basic AD&D insurance. Voluntary Accidental Death and Dismemberment (AD&D) You have an opportunity to purchase additional AD&D coverage, in multiples of pay, that best meets your needs. AVAILABLE TO PART-TIME HOURLY ASSOCIATES Term Life Insurance Associates are automatically enrolled into coverage. Part-time associates will have payroll deductions begin on 91st day of employment unless you decline coverage. Dependent Term Life Insurance You can purchase life insurance for your spouse and children.

6 Quick List of Your Health Care Options through The Home Depot Remember, to have coverage and to participate in the spending accounts* through The Home Depot for the rest of 2011, you must enroll by your enrollment deadline! For complete information on all of the valuable plans, programs and services available exclusively to you as a Home Depot associate, go to Action Needed: To choose your medical coverage and to elect other health care plans, you must enroll by your enrollment deadline. * Available to full-time hourly and salaried associates only. Medical Coverage Options Your medical options are: HMSA Preferred Provider Plan (PPO) HMSA Health Plan Hawaii Plus (HMO) Kaiser Permanente HMO If you don t go to the Your Benefits Resources web site and enroll in the medical option of your choice by your enrollment deadline, you will automatically be enrolled in associate-only coverage under the HMSA Preferred Provider Plan (PPO) for the rest of However, you must actively enroll your dependents in a medical option or they won t have coverage through Home Depot for the rest of this calendar year. If you don t want medical coverage for yourself and your dependents for the rest of 2011, Hawaii state law requires that you complete an HC-5 form. This form will be mailed to you once you elect no coverage during your enrollment. Medical and Hospital Indemnity Plan Coverage for the Dependents of Part-time Hourly Associates As a part-time associate you can choose one of these medical plan options for your dependents: Aetna/SRC $10,000 Max option Aetna/SRC $20,000 Max option Under each option, you ll pay less for medical care when your dependents use Aetna Open Choice PPO network providers. Please note that preexisting conditions apply and coverage through the Aetna/SRC options is subject to plan limits. You also can choose Hospital Indemnity Plan coverage for your dependents. This plan provides benefits that can help you pay the cost of a covered dependent s inpatient hospital stay. Please note that preexisting conditions apply and coverage through the Hospital Indemnity Plan is subject to plan limits. Critical Illness Protection Plan The Critical Illness Protection Plan can be purchased in four benefit amount options: $5,000 benefit $10,000 benefit $20,000 benefit $30,000 benefit The plan pays a lump-sum benefit for specific conditions, such as heart attack, stroke, cancer, transplant, Alzheimer s disease and paralysis and benefits for eligible travel, lodging expenses and wellness services. Dental Coverage Each MetLife dental option provides free preventive care until you have reached your annual maximum benefit: MetLife $500 Annual Maximum option MetLife $1,000 Annual Maximum option The MetLife $2,000 Annual Maximum option

7 The $500 annual maximum option covers preventive and basic restorative care only. The $1,000 and $2,000 options cover most dental services, including orthodontia. Vision Coverage Each vision option provides free eye exams from EyeMed Select providers, which include independent optometrists and ophthalmologists and the vision care professionals at Sears, LensCrafters, Pearle Vision, Target Optical and JCPenney Optical. EyeMed Select $120 option EyeMed Select $150 option The Health Care and Dependent Day Care Spending Accounts for Full-time Hourly and Salaried Associates* You may contribute up to $5,000 in a Health Care account and $5,000 in a Dependent Day Care account in To participate in 2011, you must enroll by your enrollment deadline. Spending accounts allow you to pay for certain qualified expenses with before-tax payroll deductions, reducing your taxable income. In those cases, the amount of the premium above your paycheck is still owed and will be collected from your future paychecks.
