9 2 Know Your Options Prescription Drug Coverage Each medical plan provides prescription drug coverage How prescriptions are covered may vary by the insurance carrier If you have specific questions, call the carrier to see how your prescription drugs will be covered
10 Working Spouse Surcharge (Medical Plan) If you cover a working spouse who has group medical coverage available through his or her employer, you will pay an additional $25 per week. The surcharge will not apply if your spouse: Also works for Safelite, or Is Self-Employed You must certify this status during enrollment 10
11 Tobacco Surcharge (Medical Plan) If you or anyone you cover on the medical plan is a tobacco user, you will pay an additional $10 per week surcharge You must certify this status during enrollment 11

Other benefits Health Savings Accounts If eligible, you can contribute to an HSA and Use the money to help pay your deductibles, copays, and coinsurance; or Bank of America administers the HSA, but you will enroll and/or elect your contribution amount through the My Choice enrollment site. To be eligible, you must: Be enrolled in a HSA medical coverage level Not be enrolled in Medicare or a veteran s medical plan (TRICARE) Not be claimed as a dependent on someone else s tax return Not be covered by any other health insurance plan, such as a spouse s plan Not be enrolled in a Health Care FSA 19

Other benefits Health Savings Accounts, continued Contributing to an HSA For 2019, pre-tax payroll contributions, up to $3,500 for individual coverage and $7,000 for covering you and your dependents Catch-up contributions up to $1,000/year if age 55 or older (or will reach age 55 during the plan year) Using your HSA Use your debit card to pay your provider directly from your account or you can pay the provider up front and pay yourself back later The funds in your HSA are yours to keep 20

Other benefits Flexible Spending Accounts Opportunity to set aside money before-tax to pay for certain eligible expenses You must enroll and select an annual contribution amount each year, estimate expenses carefully you must use it or you ll lose it by year-end Health Care FSA: Contribute up to $2,650 per year to reimburse certain eligible out-of-pocket health care expenses You cannot contribute to both a Health Care FSA and a Health Savings Account (if eligible) in a plan year Dependent Day Care FSA: Contribute up to $5,000 per year to reimburse certain eligible expenses to care for a dependent child or adult Please Note: If you are enrolled in an FSA in 2019, you will be able to submit claims incurred through 3/15/2020 until 3/31/2020 with Your Savings Account (YSA). 21

Other benefits Life & Disability* Life Insurance Accidental Death and Dismemberment (AD&D) Short-Term Disability (STD) Long-Term Disability (LTD) Safelite automatically provides basic life insurance for you free of charge. And, if you decide your family needs more protection, you can purchase optional life insurance coverage. You can also purchase dependent life insurance coverage for your spouse and/or children. Safelite automatically provides basic AD&D coverage for you free of charge. And, if you decide your family needs more protection, you can purchase optional AD&D coverage. STD benefits replace a portion of your income if you re unable to work. Safelite automatically provides core STD coverage for you free of charge. You also can purchase buy-up STD for additional coverage. Safelite offers you the option to purchase LTD coverage. LTD benefits pick up where your STD benefits end providing you with a portion of your income. *You may be required to complete Evidence of Insurability before new coverage can go into effect
23 4 Enroll! Other benefits Critical Illness & Accident Critical Illness Accident Insurance Living with a critical illness can mean daily unplanned expenses. To help pay for them, Safelite provides you access to critical illness insurance as a voluntary benefit to purchase. Also voluntary to purchase, accident insurance pays cash benefits for expenses associated with an accidental injury that occurs outside of work. Accidental Death and Dismemberment (AD&D) You can enroll for these benefits on the My Choice enrollment website accessible from People System!

Other benefits Voluntary Benefits Group Legal Services Covers attorney fees for things like creating or updating a will, real estate matters, divorce or separation, traffic matters, small claims court issues, tax audits and more (Carrier ARAG) Identity Theft Monitors your personal and financial information around the clock; can help you catch fraud in its early stages (Vendor: Lifelock) Accidental Death and Dismemberment (AD&D) You can enroll for these benefits on the My Choice enrollment website accessible from People System!

26 4 Enroll! k Retirement Savings Plan Highlights and Contacts Safelite offers a 401(k) program that matches up to 6% of your pay. The percentage is based on your length of service. You will be automatically enrolled (at 3%) into the Safelite 401(k) plan unless you stop or increase/decrease this percentage by calling or by visiting vanguard.com/retirementplans.
