Healthcare Plan The UnitedHealthcare (UHC) plans described here are available to employees and their eligible family members (i.e., spouse, children). 2 Dental Plan Dental benefits are offered through Delta Dental, which utilizes both the Delta Dental PPO and the Delta Dental Premier networks. 3 Vision Plan Vision benefits are available through VSP, and the plan is entirely employee paid. The table below summarizes the key benefits: VSP Vision Plan Feature Coverage Frequency Well Vision Exam $10 copay Prescription Glasses Lenses Lens Enhancements Contacts (instead of glasses) $20 copay $130 allowance for a wide selection of frames $150 allowance for featured frame brands 20% savings on the amount over your allowance Included in Prescription Glasses Single vision, lined bifocal and lined trifocal lenses Polycarbonate lenses for dependent children Tints/Photochromic adaptive lenses Included Scratch resistant coating Included Standard progressive lenses $50 copay Premium progressive lenses $80 $90 copay Custom progressive lenses $120 $160 copay Contact lens exam (fitting & evaluation) Up to $60 copay $130 allowance for contacts; copay does not apply Employee Cost and Contributions You will be able to review the cost of your benefit options on the CSC Benefits Center enrollment site when you are ready to enroll. Your contributions for your elected healthcare coverage will be taken from your gross salary through payroll deductions. These deductions will be taken before federal income taxes are calculated and before state income taxes are calculated (in states where legally permitted). 3
4 Tax-Favored Accounts CeleritiFinTech offers several types of tax favored accounts to benefits eligible employees. These accounts allow you to set aside your money on a pre tax basis to pay for qualified healthcare, dependent care or commuting expenses. All tax favored accounts are administered by Aon Hewitt s Your Spending Account (YSA) platform. Rules governing these accounts are set by the IRS. Health Savings Account (HSA) An HSA allows you to set aside your money on a pre tax basis to pay for qualified healthcare expenses for yourself or your eligible dependents (see IRS Publication 969). If you have funds remaining at the end of the year, the balance rolls over, and the account is yours to keep. You can bring your account with you if you leave CFT or retire. You can elect how much you want to contribute via payroll deduction at enrollment, but you can also change your contributions at any time during the year. You may only open an HSA if you enroll in the UHC HDHP, and meet eligibility requirements set by the IRS. The IRS sets HSA annual contribution limits each year. These limits include any company contributions you might earn through the wellness program. The IRS also allows catch up contributions for HSA participants age 55 or older. The 2017 catch up contribution is $1, Total HSA Contribution Limit Single $3,400 With Dependents $6,750 Flexible Spending Accounts (FSA) There are two types of FSAs (See IRS Publication 969): A Health FSA lets you set aside your money on a pre tax basis to reimburse yourself for qualified medical expenses that are not reimbursed through any healthcare plan (e.g., medical, prescription drug, dental, vision). The 2017 annual contribution limit for a Health Care FSA is $2,550. A Dependent Care FSA lets you set aside your money on a pre tax basis to reimburse yourself for qualified dependent care expenses that are necessary to allow you and your spouse (if applicable) to work or look for work. The 2017 annual contribution limit for the Dependent Care FSA is $2,500 if you file taxes separately, $5,000 if you are a single parent or file taxes jointly. Once you enroll in an FSA you cannot change your elections unless you have a qualified change in status (such as a birth or marriage) during the plan year. Commuter Reimbursement Account A Commuter Reimbursement Account allows you to set aside your money on a pre tax basis to pay for qualified commuting expenses (see IRS Publication 15 B). The 2017 contribution limits are $255 per month for public transit (e.g., trains, buses, ferries) and $255 per month for parking near work. Other Benefits Employee Life: CeleritiFinTech provides employees Basic Life Insurance equal to Base Annual Earnings (BAE). Employees may purchase Supplemental Life Insurance of 1x 5x BAE. Employee Accidental Death and Dismemberment (AD&D): CeleritiFinTech provides employees Basic AD&D Insurance equal to BAE. Employees may purchase Supplemental AD&D Insurance of 1x 5x BAE. Dependent Life: This optional, post tax employee paid plan offers up to $100,000 life insurance for your spouse and up to $25,000 for your dependent children. Disability Income Protection: CeleritiFinTech offers optional Short and Long Term Disability plans that provide income protection in the event of your total disability. Business Travel Accident: All employees are covered for accidental death or dismemberment while on authorized business travel. The benefit for each employee is 3x BAE, up to plan maximums. Matched Asset Plan (MAP): You are eligible to join the CSC Matched Asset Plan, CSC s 401(k) retirement savings plan, if you are at least 21 years of age. From 1% to 50% of your eligible compensation (base pay and commissions, subject to IRS limits) may be invested on a pre tax payroll deduction basis in various investment options. CFT will match 50% of your employee contributions to the plan, up to 6% of your eligible compensation. CFT s matching contribution is made on an annual basis. You are 100% vested in the company match after 1 year of service. If you are a represented employee, your contract agreement s terms and conditions apply. Vacations: CeleritiFinTech offers a competitive vacation package. Holidays: You will receive 9 paid holidays per year. Sick Leave: You may accrue a maximum of 10 days of paid sick leave per year, but this could vary depending on your work site and/or contract. Any unused sick leave will be carried forward to the next year without any limit. LifeManagement Program: This company paid service enables CeleritiFinTech employees and their eligible dependents to receive help from qualified professionals to deal more effectively with work life problems that may impact job performance. Work life specialists can assist with a broad range of issues such as personal counseling, adult/elder care, child care, tobacco cessation, legal, financial, crisis management and more. Educational Assistance: You may receive financial assistance for tuition, registration and laboratory fees for approved courses upon their successful completion. 4
5 Military Reserve Training Allowance: CeleritiFinTech pays the difference between your military and company salary for a maximum of 10 working days per year. Credit Union: Membership in the CSC Employees Federal Credit Union is available to employees and their families.
