2 The Koch benefits program is designed to help you meet your financial needs both now and in the future. These benefits are an important part of your total compensation package. In an effort to encourage a healthy lifestyle and contain costs, the Koch medical, dental, life insurance and disability insurance coverages have Tobacco-Free and Tobacco-User premiums. Unless otherwise noted, most employees regularly scheduled to work 30 or more hours per week are eligible to participate in the Koch benefit plans. 1 Medical Koch offers three medical plan options to choose from. Claims / Network Administrator Gold Silver Bronze Deductible Out-of-Pocket Maximum Employee Only Family Employee Only Family Employee Only Family $1,400 $2,800 $2,400 $4,800 $6,000 $12,000 $3,000 $6,000 $4,000 $8,000 1 $6,000 $12,000 1 Medical Services Coinsurance Koch pays 90% after deductible Koch pays 80% after deductible pay 10% after deductible pay 20% after deductible Koch pays 100% after deductible Preventive Care 2 Covered Services Provider Network Tobacco-Free Medical Premium Savings Health Savings Account (HSA) Health Flexible Spending Account (FSA) Generic Prescriptions Koch pays 100% without you having to meet your deductible All three plan options cover the same medical services All three plan options use the same Anthem Blue Cross Blue Shield network Save $75 each month for all three plan options Eligible Eligible Koch pays 80% after deductible Brand on the PDL 3, 4 Brand not on the PDL 3, 4 Preventive Medications 5 (90-day Supply) Generic Brand with no generic available Koch pays 75% after deductible Koch pays 50% after deductible Koch pays 100% after copay pay $5 copay pay $40 copay Koch pays 100% after deductible Brand with generic available 4 pay $60 copay or $5 copay + cost difference between the brand drug and generic drug alternative Note: All coverage amounts above show in-network providers. 1 Due to a requirement under ACA, the Out-of-Pocket Maximum amount for any individual enrolled in a plan with family coverage is capped at $7,350 for Subject to certain limits based on clinical guidelines used by the claims administrator. 3 PDL = Performance Drug List, which is available on 4 The pay the difference provision applies, meaning if you choose to fill a brand name drug that has a generic equivalent, you will pay the cost of the generic drug as covered by the plan, plus the difference in cost between the two. In order for the pay the difference provision to not apply, you must obtain pre approval of your physician s request for an exception with CVS/Caremark. 5 The copay only applies if you purchase a 90-day supply of a preventive medication via mail order or at a CVS retail pharmacy (including Target). If not, you must purchase up to a 30-day supply and it will be subject to deductible and coinsurance. 2 The 2018 Koch Benefits Program

Employees who select tobacco-user status will pay an additional $75 per month. Koch offers two dental plan options to choose from. Claims / Network Administrator Dental Option Base Enhanced Annual Deductible (Per Person/Per Family) $50/$150 Annual Maximum $1,500 $2,500 Lifetime Orthodontic Maximum $1,500 $2,000 Preventive 100% Basic 80% after deductible Major 50% after deductible 75% after deductible Orthodontic 50% after deductible Note: All premiums above reflect tobacco-free employees/families. Employees who select tobacco-user status will pay an additional $5 per month. 4 3 Vision Koch offers a vision plan with access to a nationwide network of physicians, optometrists and opticians. The Vision Plan offers out-of network coverage, but you pay more when you use out ofnetwork providers. Go to koch.vspforme.com for more information, or call VSP at Claims / Network Administrator In-network Exams (every calendar year) pay $0 Frame (every other calendar year) Lenses (every calendar year) Includes single vision, lined bifocal and lined trifocal lenses Contacts (instead of glasses, every calendar year) Plan pays up to a $150 allowance 1 20% savings over the allowance pay $0 Plan pays up to a $130 allowance Fitting and evaluation pay $55 1 $80 allowance at Wal-mart, Sam s Club, and Costco Monthly Premiums Employee Employee and Spouse Employee and Children Family $5.13 $10.26 $10.98 $ Flexible Spending Accounts Health Care Flexible Spending Account (FSA) may contribute up to $2,600 of your annual pay to this account on a pre-tax basis to be used for eligible expenses. Expenses may be incurred by you or an eligible family member. Dependent Care Flexible Spending Account (FSA) This account may be used for qualifying child care for dependents 12 years of age or younger or care for dependent adults. Each year you can contribute a maximum $5,000 (less for highly compensated employees) of your annual pay to this account on a pre-tax basis. IRS limits apply based on your tax filing status. Health Savings Account (HSA) If you enroll in the Koch medical plan, you may be eligible to open a Health Savings Account (HSA) with Bank of America to be used for eligible expenses. An HSA is not an employersponsored plan through Koch, but does provide a pre-tax payroll deduction option. For 2018, you can contribute the following amounts to an HSA: Employee Only Coverage: $3,450 Family Coverage (Employee + one or more dependent): $6,900 If you are age 55 and older, you can contribute an additional $1, The 2018 Koch Benefits Program
5 5 Life Insurance Koch offers life insurance coverage options for you and your family. Plan Description of Coverage Who Pays Basic Life Insurance $50,000 Koch Voluntary Life Insurance* Spouse Life* Elected in multiples of $10,000 up to 10 times your annual base pay OR $2.5 million (whichever is less) Elected in multiples of $25,000, up to $250,000 Child Life $5,000, $10,000 or $20,000 * may be required to satisfy evidence of insurability. Plan Description of Coverage Who Pays AD&D Employee Only AD&D Employee & Spouse AD&D Employee & Child(ren) Elected in multiples of $10,000 up to 10 times your annual base pay OR $2.0 million (whichever is less) The benefit for the Spouse is 55% of the Employee s coverage amount. Maximum benefit amount for the Spouse is $250,000 The benefit for each Child is 20% of the Employee s coverage amount. Maximum benefit amount for each Child is $40,000 AD&D Employee & Family The benefit for the Spouse is 50% of the Employee s coverage amount and 15% for each Child. Maximum benefit amount is $250,000 for the Spouse and $40,000 for each Child The 2018 Koch Benefits Program 5
6 7 Disability Insurance Koch offers disability insurance coverage for both short-term and long-term disabilities. Plan Description of Coverage Who Pays Short-Term Disability Plan Long-Term Disability Plan** Provides 60% of weekly pre-disability earnings for an approved disability up to 180 days Provides 60% of your monthly pre-disability earnings, up to a maximum of $25,000/month after completion of a 180 day elimination period Koch Koch and ** may be required to submit evidence of insurability if electing after the period you were first eligible. Monthly Long-Term Disability Cost Per $100 of monthly base salary Tobacco-Free Cost $0.364 Tobacco-User Cost $ Retirement Employees Savings Plan are immediately eligible to participate in the Koch Industries Employees Savings Plan ( Savings Plan ). Employee Contributions. If you elect to participate in the Savings Plan, you may contribute from 1% to 75% of your pay each pay period on a before-tax, Roth 401(k), and after-tax basis, subject to IRS annual limits ($18,500 for 2018). If you are age 50 or older before the end of the year, you can contribute an additional amount over the annual limit ($6,500 limit for 2018; known as a catch-up contribution). Company Match. If you contribute to the Savings Plan, the company matches your before-tax, Roth 401(k), and after-tax contributions $1 for $1 up to the first 6% of your eligible pay for each pay period. Savings are eligible to earn matching contributions after one year of service. Catch-up contributions are not matched by the company. Fixed Company Contribution. If you are eligible, the company will make a contribution, immediately on your first paycheck, to the Savings Plan based on your years of service (as shown), even if you have not elected to participate in the Savings Plan. Employer Contributions are limited, based on terms of the Savings Plan and certain IRS annual limits. Vesting. are 100% vested in your contributions. Company Match and Fixed Company Contributions vest 20% after each year of service until you reach 100% after 5 years of service. Rollovers. can roll over eligible before-tax, Roth 401(k), and after-tax contributions from other qualified retirement plans into your Savings Plan account. 7 9 Employee Assistance Program (EAP) This is a free, confidential and professional resource for employees and family members who may be dealing with a wide range of issues. The program includes web-based resources, face-to-face and telephone counseling. 10 Business Travel Accident Insurance r Koch company provides Business Travel Accident Insurance coverage which covers all full-time, part-time and temporary employees under contract with the company for covered injuries, including death, sustained while traveling on business. 11 Educational Assistance The Company encourages employees to continue to develop their skills and abilities to add greater value to the Company and consequently to their own intrinsic and financial rewards. will be reimbursed, up to $5,250, for costs associated with an educational course that is approved for reimbursement by your Supervisor.
