For weekly rates, take the biweekly rates and divide by 2. 2 In some instances your paycheck may not be enough to cover the entire amount of your benefits premiums. In those cases, the amount of the premium above your paycheck is still owed and will be collected from your future paychecks.
6 What s New in 2013 YOUR MEDICAL COVERAGE MEDICAL COVERAGE FOR FULL-TIME HOURLY AND SALARIED ASSOCIATES (AND THEIR DEPENDENTS) AND PART-TIME HOURLY ASSOCIATES The Home Depot is happy to announce that beginning January 1, you have an exciting addition to your full-time medical coverage: the new Kaiser Permanente Point-of-Service (POS) plan. Watch for on-site Kaiser Permanente representatives at your store soon. What s New NEW Kaiser Permanente Point-of-Service (POS) Plan. The new Kaiser Permanente POS plan gives you the flexibility to choose from three types of provider groups to get the care you need. Learn more on page Medical Plan Options NEW Kaiser Permanente Point-of-Service (POS) Plan Kaiser Permanente HMO HMSA Preferred Provider Plan (PPO) Note: available in 2013 only if you are currently enrolled in this plan HMSA Health Plan Hawaii Plus (HMO) Note: available in 2013 only if you are currently enrolled in this plan Other Important Information If you are not enrolled in medical coverage in 2012, you will automatically be enrolled in associate-only coverage under the Kaiser Permanente Point-of-Service Plan (POS) for 2013 unless you change this election during your enrollment session. If you do not want medical coverage, Hawaii state law requires that you complete a HC-5 form. This form verifies that you are exempt from enrolling in the medical plan and will be mailed to you once you elect no coverage during your enrollment session. Medical Coverage for Dependents of Part-time Hourly Associates For information on medical coverage for dependents of part-time hourly associates, visit the electronic enrollment guide at livetheorangelife.com. Your healthcare should be provided in consultation with your doctor. You are solely responsible for choosing a healthcare network and providers that best fit your needs. Neither The Home Depot nor the Medical Plan makes any warranties or makes any representations regarding the quality of such care. 3
7 What s New in 2013 YOUR MEDICAL COVERAGE (CONTINUED) The NEW Kaiser Permanente POS Plan The new Kaiser Permanente Point-of-Service (POS) plan lets you choose where to receive health care each time you need it. You have the flexibility to choose Tier 1, Tier 2 or Tier 3 providers: Tier 1 Kaiser Permanente HMO providers: You ll receive the highest benefits when you get care from Kaiser Permanente s HMO full-service care delivery system of 18 Kaiser Permanente facilities on Oahu, Maui and the Big Island and Kaiser Permanente contracted physicians on Kauai, Lanai and Molokai. Tier 2 contracted providers: You can see more than 2,150 Kaiser Permanente contracted providers and 175 contracted hospitals and care facilities in Hawaii. For a list of these providers, visit livetheorangelife.com. Tier 3 non-contracted providers: You can see any other licensed provider in Hawaii or on the mainland. A Quick Look at the New Kaiser Permanente POS Option Tier 1 Kaiser Permanente HMO You Pay 4 You will pay no deductible and receive the highest benefits when you get care from Tier 1 Kaiser Permanente HMO providers. If you receive care from a Tier 3 provider, you will pay 20% coinsurance plus any amount over the maximum allowable charge (MAC). The MAC is the maximum charge that Kaiser Permanente will consider for a covered service. For emergency room and urgent care services, you can use any provider. These benefits will be paid at the same level the plan will pay for a Kaiser Permanente provider, provided the services are medically necessary. If you change your coverage from HMSA to the Kaiser Permanente POS or HMO and have questions about the transition of care between HMSA and Kaiser Permanente, visit and look for Transition of Care within the In The Know section on the home page. 8 NEW for 2013! THE HOSPITAL INDEMNITY PLAN The Home Depot is offering a new benefit for 2013 to full-time hourly and salaried associates and their dependents: the Hospital Indemnity Plan. This plan pays a cash benefit if you have an inpatient hospital stay. You can enroll yourself and eligible family members in Hospital Indemnity Plan coverage as part of your 2013 benefits package. Part-time hourly associates currently enrolled in this plan will default into coverage for If you or someone you re close to has ever been an inpatient at a hospital, you know the out-of-pocket expenses for even the shortest stay add up fast. And these expenses often come at a time when you re experiencing a loss of income due to the illness or injury, making it difficult to pay your mortgage, rent, car payment and other expenses. The Hospital Indemnity Plan can help by providing cash if you have an inpatient hospital stay. Cash When You Need It Most The plan is very simple. If you have an inpatient hospital stay, the Hospital Indemnity Plan pays the following benefits: Inpatient Benefit (once per calendar year) $1,000 $100 Per-Day Benefit (up to 100 days per calendar year) The benefit is paid in cash and how you use it is up to you groceries, rent, bills or whatever you need. Here s an example of how the plan would pay benefits for two hospital stays within 2013: 2013 s #1: 6 days #2: 12 days Benefit (once per calendar year) Per-Day Benefit ($100 per day up to 100 days per calendar year) $1,000 $100 x 6 = $600 $1,600 $0 $100 x 12 = $1,200 Enrolling in the Hospital Indemnity Plan Hospital Indemnity Plan Benefit $1,200 Full-time hourly and salaried associates, you must actively enroll yourself and any eligible dependents you wish to cover. You can enroll in this plan regardless of whether you have medical coverage through The Home Depot. Insurance plan is underwritten by Aetna Life Insurance Company (referred to as "Aetna") and administered by Aetna or Strategic Resource Company (SRC, an Aetna company). Important Disclosure: This plan does not offer comprehensive benefits. It provides limited coverage and is not intended to replace other health insurance coverage. This plan has specific limits and other restrictions on the dollar amounts covered under the plan. Once these limits have been reached, the plan will not pay any more towards this coverage. The SRC Benefits Summary explains these limits. Please read it carefully so that you understand the limits to what the plan will pay before you enroll. 5
