3 What to know. Open Enrollment is October 16 to October 30. Fine-tuning and enhancements. Not much is changing this year, so your current benefit elections will automatically carry forward into You only need to enroll if you wish to make changes or put pre-tax dollars into a Flexible Spending Account. A few highlights: Prescription drug coverage will move to CVS/Caremark providing an enhanced Crewmember experience through its convenient retail network. Plus, prescription expenses will now count toward your medical plan s out-of-pocket maximum. Dental coverage has been enhanced to include dental implants and posterior composites (white fillings). More leading innovations and support are coming throughout 2015 LiveHealth Online to video chat with a doctor continues in 2015; expansion of HealthyMoms HappyBabies beginning January 1; BlueCenters of Surgical Excellence starting April 1; Applied Behavioral Analysis (ABA) therapies coming July 1 just to mention a few! 4 Medical/Rx. JetBlue offers medical options with UnitedHealthcare and Anthem BlueCross BlueShield. Both options have plan accounts (with FREE money from JetBlue) and include prescription drug coverage. Medical remains the same for expenses count toward the Green Plan (PPO with HRA) Health Reimbursement Account (HRA) Funded only by JetBlue. Debit card for easy access to funds. Account balance rolls over year after year. Can also fund up to $2,500 (pre-tax) into a Healthcare FSA. Earn an additional $250 ($500 family) by working with a BlueCarpet nurse for an ongoing health condition through our Care Management program this will reduce your deductible even further. 6 What to know. Dental. The Delta Dental PPO plus Premier plan has two network choices: Delta Dental Premier and Delta Dental PPO SM. The plan covers typical preventive, basic and major dental services, including orthodontia for dependents under age 19. Vision. The vision plan, administered by EyeMed Vision Care, covers routine exams, lenses, frames and contacts. And, you can see any eye doctor you d like, but you ll save the most money if you go to an EyeMed Vision Care Access Network provider. NEW: Coverage for dental implants and posterior white fillings. For more updates, see the dental section on lifeisbetterinblue.com. 4 Flexible Spending Accounts. Contribute a portion of your earnings pre-tax to one or both Flexible Spending Accounts (FSAs) the Healthcare FSA or Dependent Care FSA. Use the Healthcare FSA to help pay healthcare expenses for you and your family. Use the Dependent Care FSA to help pay qualified day care expenses. These accounts let you have tax-free dollars set aside to pay qualified expenses while reducing your taxable income. Remember, you cannot fund a Healthcare FSA if you elect the Blue Plan with a Health Savings Account (HSA). $ Use up FSA dollars! Spend your pre-tax FSA dollars before December 31, 2014, because you can only carry forward $500 from a Healthcare FSA into the new year. Dependent Care FSA money does not carry forward. You have until March 31, 2015 to submit all FSA claims for reimbursement.

8 What to know. More benefits, please. Life Insurance and AD&D You get Basic coverage for yourself (automatically) plus Supplemental coverage that you can purchase. You can also buy Dependent coverage for your family at group rates. Disability Disability coverages replace a portion of your pay if you get sick or injured. Group Legal You get access to a nationwide network of professionals provided through Hyatt Legal. 6

Choose the coverage you want which plans to elect and which dependents to cover for Be sure to update all dependent information. Enroll on BCSS go to bcss.jetblue.com and follow the prompts to update your dependents and make your 2015 elections on or before October 30. Most of the coverage you elect during Open Enrollment begins on January 1, 2015 and stays in effect for the entire year. Changes are only allowed if you have a Qualified Life Event, such as a birth, marriage, divorce, change in work status, etc. Visit lifeisbetterinblue.com for information on when each benefit begins such as Disability and Life Insurance. Enroll in the CSPP. Crewmember Stock Purchase Plan (CSPP) enrollment is also October 16 to October 30.
