In the event that any of these specifications conflict with previous specifications, the specifications in this addendum shall control. Prepare your bids accordingly: 1. The Deadline for bids has been extended to Tuesday, July 12, 2016 at 3 p.m. 2. Pre-bid attendance sheet is attached. 3. Replace Section 12 of the IFB (Bid Form) with the attached revised form. 4. Additions to Section 2, Background: Currently the County has a Waterworks Operator on staff that maintains all wells owned by the County. This person will retire as of June 30 and there is no one else on staff certified to do the work. The County has opted to enter into a contract with a local vendor to provide these services instead of continuing to provide them in-house. The County also has entered into an agreement with a vendor to provide temporary service until an annual contract can awarded. 5. Revisions/Additions to Section 3, Scope of Work: Contractor shall provide all materials, equipment and labor to perform preventive maintenance, inspections, sampling, testing and reporting. County will supply all sampling bottles and chemicals. Contractor shall provide repeat samples at a fixed fee listed on the revised Bid Form. All tanks shall be flushed once a year during the summer months (July-August). Additional Services are defined as non-emergency repairs, repeat sampling, or other services not part of the normal preventive maintenance or required sampling. These IFB Addendum #1 Page 1 of 8

2 services will be paid on time and materials basis at the hourly or fixed fee rates as listed on the bid form. During regular maintenance and inspection, should the contractor see that repairs are needed, they shall contact the County for approval prior to completing repairs. When called upon by the County, the Contractor shall respond to all service requests within 2 hours and have staff on-site within 24 hours or a reasonable time frame as discussed upon contact. Emergency Services are defined as work required to correct any problems that disrupt the County s normal operations. County will note at time of contact whether a service request is an emergency or not. The Contractor shall respond to all emergency service requests within 1 hour and have staff on-site within 2 hours of initial contact. Emergency Services will be paid on a time and materials basis at the hourly rates listed on the bid form. Materials used by the contractor for repairs outside of the normal preventive maintenance and sampling shall be billed to the County at actual cost to the Contractor. Actual cost shall be defined as the exact dollar amount paid to a supplier by the contractor for materials to be used or that have been used on behalf of the County. No additional markup of any kind shall be allowed on materials. The County s regular business hours are Monday Friday from 8:30 a.m. 5:00 p.m., excluding state holidays. 6. Section 6.1, Contractor shall have a minimum Class IV rank Waterworks Operator through DPOR. 7. Below is a list of average usage: High School and Middle School approximately 3,000 gallons per day Elementary Schools approximately 2,500 gallons per day Historic Southside HS recent usage has varied based on occupancy Courthouse approximately 300 gallons per day Government Complex approximately 1,000 gallons per day 8.
