It shuld be signed persnally by the applicant (signature must be placed bth t field 37 and t the field at the bttm f the last page). Guardian signs a visa applicatin fr a minr applicant. The Murmansk ffice f the Cnsulate General in St.Petersburg accepts applicatin frms filled in Russian. Please fill the applicatin in capital letters if filling by hand. 2. Travel insurance plicy Each applicant (including children) must have an insurance plicy. An exceptin are spuses f EU citizens and their under 21-year-ld children, as well as hlders f diplmatic and fficial passprts n missin. They d nt need insurance. An applicant fr multiple-entry visa may prvide an insurance plicy, which is valid during the first trip nly. The applicant agrees t purchase an insurance plicy fr further trips when signing the visa applicatin. Minrs (belw 18 years) Birth certificate is required if the applicant is a minr; the cpy f the birth certificate shuld be prepared befrehand. If a minr applicant travels alne r with ne f the parents, then a ntarized cnsent f the secnd parent r the guardian is required fr traveling. In case it cannt be prvided, dcuments prviding the reasn fr lack f the permissin (single parent reference r ther dcuments) needs t be presented. Cnsent has t be valid in all Schengen cuntries and be nt lder than 12 mnths befre the dcument submissin.
