2 REQUEST FOR QUOTES I STATEMENT OF WORK 2017 Asphalt Paving Repair Work 1. REQUEST FOR QUOTES. The City of Leavenworth, Washington is requesting quotes from qualified contractors for asphalt repair and replacement services on specified areas of pavement throughout the City Description of Work. The work performed under this contract consists of cutting, removal, and disposal of existing asphalt and undesirable subgrade; subgrade prep, compaction, asphalt replacement, and joint seal Instructions to Bidders. All proposal packages should be labeled 2017 Asphalt Bid Repair and sent to: City of Leavenworth Public Works 2017 Asphalt Paving Repair Bid Attn: Chantell Steiner, City Clerk 700 US Highway 2 / P.O. Box 287 Leavenworth, WA Bidders are responsible for assuring delivery. No electronic bids will be accepted. For questions, contact Herb Amick at herba@cityofleavenworth.com or Ext All answers to questions may be shared with other vendors participating in the RFQ process One reproducible copy of the proposal may be mailed or delivered to the address above. Bidders are responsible for assuring delivery Proposals must be received by the City no later than Thursday June 29 th, :00 PM. All bids will be timed stamped by the receptionist at the Leavenworth City Hall reception area based on the official bid clock located in the reception area. 1.6 Bids must be sealed in an enveloped and properly addressed and endorsed with the bidder s name in the top left hand corner. 1.7 Rejected Bids - Any bids not received by the deadline or not submitted properly will be rejected and returned to the bidder unopened. 3 1.8 Bid Opening Thursday June 29th, :10 PM 1.9 Notification and Award - Proposals will be evaluated, successful bidder notified, followed with a recommendation to go forward to the full City Council for project award. The award process is triggered and contingent upon the mayor's signature of the Contract Agreement Small Works Roster. The City of Leavenworth uses Municipal Research and Services Center of Washington (MRSC) as the City's list of qualified businesses for small works projects. Proposers are required to be registered with Municipal Research and Service Center of Washington (MRSC) as a participating Small Works Business. For further information, view 2. STATEMENT OF WORK A. These specifications include requirements to provide asphalt replacement/repair specified as two types. Type 1 consisting of precut, minimal prep/compaction, asphalt replacement and joint sealing. Type 2 consisting of cutting, removal and disposal, subgrade replacement, compaction, asphalt replacement, and joint sealing. B. This work shall include the furnishing of all materials, equipment, tools, and labor as required for the specified work. C. Products and work shall conform to the City of Leavenworth specifications and/or Washington Department of Transportation s Standard Specifications for Road, Bridge, and Municipal Construction. 3. GENERAL REQUIREMENTS 3.1 General This is a lump sum contract under the City Administrator contracting authority for street repairs listed Insurance - Certificate of Insurance with Additional Insured Endorsement at City specified amounts as required. The Contractor shall maintain the following insurance limits: Page 3 of 31 a) Automobile Liability insurance with a minimum combined single limit for bodily injury and property damage of $1,000,000 per accident. Automobile Liability insurance covering all owned, non-owned, hired and leased vehicles. Coverage

4 shall be written on at least as broad as Insurance Services Office (ISO) form CA Automobile or a substitute form providing equivalent liability coverage. If necessary, the policy shall be endorsed to provide contractual liability coverage. b) Commercial General Liability insurance shall be written with limits no less than $3,000,000 each occurrence, $3,000,000 general aggregate and a $2,000,000 products-completed operations aggregate limit. Commercial General Liability insurance shall be written on at least as broad as ISO occurrence form CG and shall cover liability arising from premises, operations, stop gap liability, independent contractors, products-completed operations, personal injury and advertising injury, and liability assumed under an insured contract. The Commercial General Liability insurance shall be endorsed to provide the per project general aggregate limit using ISO form CG or an equivalent endorsement There shall be no e exclusion for liability arising from explosion, collapse or underground property damage. The City shall be named as an additional insured under the Contractor s Commercial General Liability insurance policy with respect to the work performed for the City using ISO Additional Insured endorsement CG and Additional Insured- Completed Operations endorsement CG or substitute endorsements providing at least as broad of coverage. c) Workers Compensation coverage as required by the Industrial Insurance laws of the State of Washington Performance & Payment Bonds - Required All work must be completed by August 31, Work will be administered through the Public Works Director and office. Work will be inspected by the Public Works Department or representative engineer The Contractor shall furnish all tools, materials, and equipment necessary to perform the work Prevailing Wage - All work performed under this contract is applicable to prevailing wage rates as set forth by the State of Washington Department of Labor and Industries Prevailing Wage Schedule, RCW It is the responsibility of the contractor to insure that the appropriate labor classification(s) are identified and applied Intents and Affidavits The contractor shall provide certified documentation of filing with the Department of Labor and Industry a Statement of Intent to Pay Prevailing Wages prior to Page 4 of 31

5 start of work. Certified documentation of an Affidavit of Wages Paid must be provided prior to final payment. Cost of these filings is the responsibility of the contractor and not the City. 3.2 Traffic Control Traffic control shall be responsibility of the contractor. All flagging or traffic control will be done by state certified flaggers. 3.3 Waste Disposal All waste generated from this project shall be disposed of by the contractor at an approved recycling site/center. 3.4 Safety Contractor shall perform work in a manner to protect the health and safety of all workmen and the public. All work shall be in accordance with standard industry safety practices and in compliance of current OSHA regulations. 3.5 Warranty Contractor shall warrant all work to be free of defects for a period of one (1) year from date of final acceptance. 4. EXECUTION OF WORK 4.1 General Preconstruction Meeting A preconstruction meeting with Public Works staff will be arranged and scheduled by the successful bidder at City Hall prior to the begin of all work Work shall not commence earlier than 7:00A.M., nor extend beyond 7:00 PM, without the express approval of the Public Works Director or designee. No work shall be performed on Fridays, Saturdays, Sundays or holidays unless authorized by the Public Works Director or designee All waste material generated by the Contractor's staff shall be removed daily from the work site and the area left clean and clearly marked with appropriate safety signage/barricades/etc. upon completion of work All road areas must be left in drivable/safely passable condition on Friday, Saturday, Sunday, and holidays. 6 4.1.5 Square footage amounts shown are estimated. Verification for bid purposes and lump sum price is the responsibility of the bidder. 4.2 Type 1 Repair Remove and dispose of any unwanted material and/or subgrade in the areas listed Prep and compact appropriately to accommodate 3 of new asphalt Pave with 3 of new asphalt. Roll smooth new asphalt to comply with existing grades and seal existing cut. 4.3 Type 2 Repair Cut, remove, and dispose of broken asphalt as shown at the listed areas Excavate, remove, and dispose of improper subgrade. Replace, grade, and compact new subgrade as specified Pave with 3 of new asphalt to meet existing grade. Roll smooth and seal new cuts. 4.4 Testing and Inspection Type 2 repair requires compaction testing and documentation of required specified compaction prior to new paving A city Public Works Employee or representative engineer will make ongoing and final inspections of all sites. 5. PROPOSAL TERMS AND CONDITIONS 5.1. The City reserves the right to reject any and all proposals, and to waive minor irregularities in any bid proposal The City reserves the right to request clarification of information submitted, and to request additional information from any bidder The City reserves the right to reject any proposed agreement or contract that does not conform to the specifications contained in the bid.
