The City of Hazelwood, Missouri is accepting sealed bids for the purchase of fitness equipment along with delivery and assembly in its entirety at Fire Station 1 and Fire Station 2. Specifications may be obtained at the Hazelwood Fire Department Station 2, 6800 Howdershell Road, Hazelwood, MO Monday through Friday, during normal business hours, 8:00 a.m. to 4:30 p.m. or at If you retrieve your bid information from the website, please submit your information to Lisa Schwan, Admin Secretary, Hazelwood Fire Department, 6800 Howdershell Road, MO or lmschwan@hazelwoodmo.org All bids must be received by 10:00 a.m., Friday, October 24, 2014 by the Fire Chief, at which time they will be opened publicly.

3 CITY OF HAZELWOOD, MISSOURI CONDITIONS OF BIDDING 1. The City of Hazelwood will receive sealed bids at the office of the Hazelwood Fire Department Station 2, 6800 Howdershell Road, Hazelwood, MO until 10:00 a.m., Friday, October, 24, 2014 for the purchasing of fitness equipment to include deliver and assemble of the list in it entirety at Fire Station 1 and Fire Station 2. Bids will be opened at that time. 2. The bidder shall follow specification sheets as provided for items bid, or list exceptions. Bids will include delivery and assembly to Hazelwood Fire Department, Station 1, 6100 N. Lindbergh Blvd., Hazelwood, MO and Hazelwood Fire Department Station 2, 6800 Howdershell Road, Hazelwood, MO 63042, all bids shall be net delivered price (F.O.B. Hazelwood, MO) and exempt from excise and sales tax. Dealers are to furnish exemption certificates. 3. Each proposal shall be made on the attached bid form and must include the signature of the owner or authorized officer/s of organization submitting said bid and the complete mailing address of said organization. Any exceptions to these specifications must be noted and any supplementary or supportive materials may be attached. Equal as well as substitute bids will be considered. 4. Bids are to be submitted in a sealed opaque envelope, clearly marked Fire Department FITNESS EQUIPMENT. 5. Bid must include warranty on all parts and labor. 6. Non-collusion affidavit must be completed and notarized. 7. The City reserves the right to reject any or all bids and to waive informalities in bidding. 8. The specification must be returned with the bid proposal. Any all exceptions to the specifications outlined herein must be noted on this bid specification and thoroughly explained in the bid proposal. It will be the responsibility of the Fire Department to determine if a substitution to the specification is considered equal to. The bidder must provide with any exceptions, detailed specification so that the Fire Department can make the necessary comparisons to make a proper decision. Failure to do so may result in rejection of your bid for noncompliance. Exceptions must be noted on the spec sheet.
