2 INVITATION TO BID PROJECT: OWNER: Boiler System Replacement Van Buren Intermediate School District 490 S. Paw Paw Street Lawrence, MI The VAN BUREN INTERMEDIATE SCHOOL DISTRICT is soliciting competitive bids for the removal and replacement of the boiler system located in the Technology Center building at 250 South Street, Lawrence, MI Awarded bidder must also provide sole source services including mechanical, plumbing, controls, energy, and training for Building Supervisors. Sealed Bids are due Tuesday, May 30, 2017 at 2:00 p.m. at the Van Buren Intermediate School District Business Office, 490 South Paw Paw Street, Lawrence, Michigan 49064, at which time bids will be publicly opened and read aloud. Bids received after that time will be returned unopened. Envelopes shall be sealed and marked Boiler System Replacement. All proposals shall be made in accordance with Bidding Documents. Work must be completed on or before September 1 st, This project does not require prevailing wage. A single lump sum bid is being requested for the complete project as outlined in the attached specifications. Bid quotes are guaranteed for 90 calendar days. The Van Buren Intermediate School District reserves the right to accept or reject any or all bids, and to waive any irregularities which it deems to be in the best interest of Van Buren Intermediate School District. A mandatory pre-bid inspection must be held with Randy Olds, Maintenance Supervisor. To set up an appointment between Monday through Friday between 8:00 a.m. until 3:30 p.m., you may reach Randy Olds at (269) or (269) Direct all questions regarding said proposal to Randy Olds, Maintenance Supervisor, (269) or(269) AFFIDAVITS In compliance with MCL , all bids shall be accompanied by a sworn statement disclosing any familial relationship that exists between the owner(s) or any employee of the bidder and any member of the Board of Education of the School District or the Superintendent of the School District. Also, a completed certification form as attached for compliance to the Iran Economic Sanctions Act is required with the bid proposal. The Board of Education shall not accept a bid that does not include the sworn and notarized disclosure statements. BID SECURITY All bids must include a Bid Security for five percent (5 %) of the bid amount payable to Van Buren Intermediate School District, in either the form of a certified bank check or bid bond.

3 The Van Buren Intermediate School District will have the right to retain the Bid Security of bidders to whom an award is being considered until either: a) The Contract has been executed. b) The specified time has elapsed so that bids may be withdrawn. c) All bids have been rejected. CERTIFICATE OF INSURANCE Before commencement of any work, a Certificate of Insurance must be provided by the award bidder s insurance agent or carrier showing required insurance coverage as follows: Commercial Liability Insurance written on an occurrence policy form with limits of liability not less than $1,000,000 each occurrence, $1,000,000 aggregate, combined single limit, that includes coverage for your operations, personal injury, XCU (explosion, collapse and underground), independent contractors, contractual, and productscompleted operations. Workers compensation, including Employer s Liability Coverage, of $100,000 each accident, $500,000 annual aggregate, in accordance with all applicable Michigan law. Failure to provide the certificate may be considered material breach of the agreement and may be grounds for terminating the agreement. 4 Scope of Work Van Buren Intermediate School District is requesting proposals from companies for the removal and replacement of the boiler system located in the Technology Center building at 250 South Street, Lawrence, MI The awarded bidder must provide sole source services including mechanical, plumbing, controls, energy, and training for Building Supervisors. The company is responsible for all measurements and other details of this proposal. A response to this request for proposal is an indication that the vendor has checked all aspects of this request. Van Buren Intermediate School District reserves the right to reject any or all bids, for any reason. The awarded bidder must comply with the applicable Federal, State, and local safety requirements. Prior to initiation of services, the awarded bidder shall provide a copy of their written safety program, which shall include health, safety, and environmental quality for review. The awarded bidder shall give all necessary notices, obtain all permits, and pay all government sales taxes, fees, and other costs. The awarded bidder will file all necessary plans, prepare all documents, and obtain all necessary approvals of all governmental departments having jurisdiction, obtain all required certificates of inspection of work and deliver same to the Owner before acceptance and final payment of work. Design and engineering including stamped drawing Provide two (2) copies of all manuals, drawing(s), and other documents. Provide start-up and training for Owner s personnel. Demolition of existing hot water heating system. Demolition of existing steel vent stack in boiler room. Supply and install three (3) new boilers, packages including: o Three (3) new Fulton Endura condensing high efficient boilers o Duplex stainless steel heat exchangers o Digital controls for efficient operation New carbon steel piping where needed Valves and fittings New primary header New condensate neutralizer kits New backflow preventer New gas header, connections, and valves New stainless steel vent stacks CSD-1 controls Low voltage control wiring Two (2) new building system pumps Two (2) new variable frequency drives for pumps New CSD-1 compliant emergency shut off for mechanical room Pipe insulation Welding Core drilling Wall removal for boiler demolition

5 Masonry work for wall replacement Concrete sawing Equipment rental Pipe support steel Roof work Electrical Programming Mechanical permit Disposal of old equipment per all safety Federal, State, and local requirements. Installation will include any additional accessories required to meet code requirements and provide a fully operating system. Warranty Boiler system must carry a Thermal Shock and Pressure Vessel warranty of at least 10 years and a labor and parts warranty of a minimum of one year from date of final installation.

8 AFFIDAVIT OF COMPLIANCE IRAN ECONOMIC SANCTIONS ACT Michigan Public Act No. 517 of 2012 The undersigned, the owner or authorized officer of the below-named bidder (the Bidder ), pursuant to the compliance certification requirement provided in the Van Buren Intermediate School District (the School District ) Request For Bids For (the RFB ), hereby certifies, represents and warrants that the Bidder (including its officers, directors and employees) is not an Iran linked business within the meaning of the Iran Economic Sanctions Act, Michigan Public Act No. 517 of 2012 (the Act ), and that in the event the Bidder is awarded a contract as a result of the aforementioned RFB, the Bidder will not become an Iran linked business at any time during the course of performing any services under the contract.
