2 OPEN ENROLLMENT The Human Resources Benefits Office is hosting its annual Open Enrollment period. This is the time of year when you can change coverage or add or terminate dependents and domestic partners to your Medical, Prescription, and Dental plans. You can also get more information on other benefit programs. The Open Enrollment Event (August 25) provides you with an opportunity to speak with plan representatives, gather information and select the plan(s) best suited for you and your family. You may not change plans or add dependents mid-year unless there is a life event such as marriage, divorce, birth of a child, adoption, spousal loss of a job, or dependent s loss of eligibility for coverage. You must inform the Benefits Office within 30 days of a life event in order to make a change in benefit coverage. However, recent law changes have added two (2) new enrollment events relating to eligibility for coverage. Benefit changes may be made within 60 days of a life event of coverage under Medicaid or a State children s health insurance program (CHIP). Eligibility Guidelines for Health Benefits for Dependents To cover a spouse, the employee must provide a marriage certificate. For dependent coverage, the employee must provide a birth certificate/court order showing that the employee is the parent or legal guardian of that child, including children from other relationships. For domestic partnerships, employee must complete affidavit and declaration forms. Dependents on Medical & Dental As part of the Affordable Care Act legislation, the College implemented the Age 26 Dependent coverage for medical insurance effective September 1, Employees who have a dependent child under the age of 26 will be able to enroll or keep the dependent child under the parent s College medical benefits plan. The following summarizes the Age 26 Dependent coverage: The dependent child may enroll for medical and prescription coverage only (no dental coverage). The dependent child will be covered until the end of the month he/she turns age 26 (child will then be eligible for 36 months of COBRA). There is no student, employment, residency or marital requirement. Employees (parents) must provide a copy of the dependent s birth certificate or adoption papers. Forms can be found on the College HR website. Please complete both forms, attach a copy of the dependent child s birth certificate and return all three forms to the Human Resources office attention Beth Kauffman. Dependents can be added during the regular annual open enrollment period which runs from August 25, 2014 through September 26, Coverage will start on October 1, Dependents under the dental plans, Delta & United Concordia, will be termed from coverage in accordance with our current policy at age 19, unless the dependent is a full-time student (12 credits or more). Coverage continues until age 23 for full-time students. The Human Resources office now handles student verification for dependent coverage for the dental plans. To continue dependent coverage past age 19, the employee (parent) will need to complete a Student Verification form (found on the College HR website) and submit that completed form along with a copy of the student s (dependent s) roster to the Human Resources office each year during the annual open enrollment period. The deadline is September 26, *** NOTE: OPEN ENROLLMENT IS ONLINE AGAIN THIS YEAR. YOU WILL HAVE TO LOG INTO TO YOUR MYCCP ACCOUNT AND COMPLETE AN ELECTRONIC FORM. Once you log into your MyCCP account, go to the employee self-service area, and click on Employee. Benefit enrollment changes will be accepted from Monday, August 25 through Friday, September 26, Benefit coverage changes will be effective October 1 for medical, prescription and dental benefits.

3 MAIN CAMPUS OPEN ENROLLMENT/BENEFITS FAIR ACTIVITIES MONDAY, AUGUST 25-11:30 AM TO 4:00 PM Representatives from the following plans/vendors will be present: Medical Plans Independence Blue Cross; Prescription Plan Express Scripts (formerly Medco); Dental Plans Delta and United Concordia; Retirement Plan Investment Vendors TIAA-CREF and Fidelity; Employee Assistance Program Carebridge; PT Faculty Medical Benefits Linda Sotera from HealthNow and Lolita Lukes, CCP; Life Insurance - Symetra ; Liberty Mutual employee discounts on auto, homeowner s & tenant insurance. HEALTHY COOKING DEMONSTRATION CCP Instructor Trenay Parker from Culinary Arts & Hospitality Management will demonstrate healthy cooking and whip up three healthy dinner recipes for you to sample. The cooking demonstration will be held in the teaching kitchen on the second floor in the Pavilion from 12:30 PM to 1:30 PM. Recipes will be available at the event. Yummy!! FREE CHAIR MASSAGES BY HAND & STONE SPA Representatives from Hand & Stone Massage and Facial Spa at Logan Square (near Whole Foods) will be on hand to give free chair massages to employees. They have many locations, offer employee discounts and corporate memberships and are open 7 days a week. At the Benefits Fair they will have a raffle for a free spa gift certificate. Buena!! FREE NUTRITIONAL COUNSELING SESSIONS ON CAMPUS YEAR ROUND Registered Dietitians from Family Food Corporate Wellness Nutrition will conduct free, one-on-one employee nutritional counseling sessions on campus year round. As part of your Blue Cross medical plan benefit, members are eligible for six, free nutritional counseling sessions. You don t have to pay up front and wait for reimbursement. Family Food will complete all the paperwork on your behalf. Sessions will be held in the Human Resources annex starting September 15 with appointment times between 9 AM to 5 PM. Dates at the three Regional Centers will be announced in the near future. 4 THE FLEXIBLE SPENDING ACCOUNTS PROGRAMS (Healthcare & Dependent Care) run on a calendar year, starting January 1, We will hold a separate special open enrollment period for the FSA programs in early November The maximum annual dollar amount for the Healthcare Spending Account is $2,500, the minimum amount is $300. New for the 2014 plan year is a carryover of up to $500. Flexible Spending Accounts are for full-time employees working at least 32 hours a week and who are scheduled for twelve (12) months of continuous service with the College at the time the plan year begins. COMMUTER BENEFITS are handled by ADP online at There is no enrollment period; eligible employees (see eligibility above) can go on the website each month to purchase transit passes, checks, vouchers, MasterCard, and parking. The cost is deducted from the following pay period on a pre-tax basis. SUMMARY PLAN DESCRIPTIONS for Personal Choice and Keystone Point of Service medical plans, Delta and United Concordia dental plans, Medco/Express Scripts prescription plan, and Flexible Spending Account can be found on the College HR website. Go to Human Resources main page, scroll down the left side and click HR Benefit Booklets and Links. RETIREMENT PLAN: Employees can change their retirement plan investment vendor (TIAA- CREF or Fidelity) any time during the year. Please note that any employee, regardless of their employee classification or number of hours worked, may enroll into the College 403b Supplemental Contributions Plan (elective deferrals, no College contributions) at any time. We will have retirement plan information and enrollment forms at the Benefits Fair on August 25, Forms to change your retirement plan contribution amounts/percentages are found on the College HR website. You can change/increase your contribution to your retirement account (above the mandatory 5%) at any time. The annual 2014 maximum contribution limit is $17,500 for those under age 50, and $23,000 for age 50 and above.
