2 T Tax Dollars at Work Public Spending for Selected Health and Social Programs in Michigan By County, FY2004 his report includes spending and recipients for a number of programs administered by the Departments of Human Services, Community Health, Education, and Labor and Economic Growth. The current debate over spending reductions or revenue increases for public programs heightens the importance of understanding both the human impact of these programs and the dollar value of these programs as an economic stimulus in Michigan communities. These programs provide vital services to the poor, ranging from the young through the elderly, and from families to individuals. Program reductions, discussed in aggregate at the state level, may not seem drastic, but are often devastating to deal with at the individual or the community level. For example, proposals in the FY2006 budget to close individual prisons have generated much debate as the local economic impacts have been identified. So it is with the programs included in this report. Therefore, it is critical to understand potential local impacts on both the citizenry and the local economy when program reductions are discussed at the state level. Studies continue to confirm the benefits of public spending to maintain public services and to promote economic and job growth. Not only do many public programs garner federal funds that match or exceed the state funds invested, but these public program expenditures also create demand for other goods and services and expenditures within local communities. For example, a Family Independence Program payment to a recipient will generate income to a landlord or utility company who will, in turn, use the funds to purchase goods or services or pay employees. A Medicaid provider payment will generate income to the provider that will, in turn, be used to pay employee salaries and purchase other goods or services. In addition, strong public services and infrastructure provide the support that businesses seek when determining where to locate. Low taxes and business incentives are not the only factors that attract businesses. Recently, Toyota Motor Company left $200 million of tax breaks and incentives on the table in Alabama, and decided to build their plant in Ontario, Canada with an offer of half of the tax breaks, but a better educated and more skilled workforce. Public investment won out over tax breaks. Individual county profiles for Tax Dollars at Work are available that identify not only the amount of revenue flowing into the community for various programs, but also the number of people impacted by selected programs and services. 3 Michigan Highlights While FY2004 spending for the selected programs included in this report increased by about $745 million over FY2003 spending, over $500 million of the increase is federal funds that provided economic stimulus to local communities. Summary of safety net spending, by category, statewide: Total Public Spending for Selected Programs $11.5 billion Health Care $7.5 billion (65.0%) Income $1.6 billion (13.5%) $1.14 billion is Supplemental Security Income Nutrition Services $1.2 billion (10.5%) Work Support $603.0 million (5.2%) Children s Services $547.2 million (4.8%) Emergency Services $110.9 million (1.0%) Tax Dollars at Work: Public Spending for Selected Health and Social Programs FY2004 Medical 65.0% Work Support 5.2% Children's Services 4.8% Income 13.5% Nutrition Services 10.5% Emergency Services 1.0% Family Independence Program 3.4% State Disability 0.3% Supplemental Security Income 9.9% At the state level, medical services and supports represent nearly two thirds of the safety net spending at $7.5 billion, assisting not only low-income adults and children with needed medical care, but also providing significant economic opportunity and jobs within the community. The next largest individual program of safety net spending is the Supplemental Security Income program, providing $1.14 billion in federal and state funded cash assistance to aged, blind, and disabled recipients. Of the $1.14 billion, about $57 million are state funds, the balance is federal funds. Other cash assistance programs (Family Independence Program and State Disability ) total about $420 million. Spending for Work Support programs, which include the Child Day Care Program and Work First/Welfare to Work Programs, provides over $600 million of services to families and individuals, and infuses a like amount of revenue into communities. More than 85 percent of safety net spending goes directly to service providers in local communities, assisting not only the individuals and families, but also the economies of the local communities. While health care is the major component of safety net spending, the percentage of total public social and health expenditures that relate to health care spending varies considerably among counties from lows in Genesee (57.2%) and Wayne (58.8%), to a high in Iron County of 81.9%. Total state spending to support Day Care Services exceeds Family Independence Program cash assistance spending by nearly $100 million. Nearly every program included in this analysis brings federal dollars to the State of Michigan and provides a return to Michigan of the tax dollars paid to the federal government. For DHS programs, Emmet and Charlevoix Counties are combined and all data are reported under Emmet County. For the Supplemental Security Income Program, the statewide estimate is based on actual expenditures, as reported by Social Security Administration, for calendar year 2003, updated by a 2.1% inflation factor for 9 months to estimate fiscal year 2004 expenditures. County distributions are based on each county s share of SSI eligibles reported in DHS Publication 170, Table 57. The Medical estimates include statewide spending for regular Medicaid payments, disproportionate share hospital payments, Title XIX funded mental health, the Title XIX component of Children s Special Health Care Services, Home Help and Personal Care Services Payments, and the Adult Benefits Waiver (which replaced the State Medical Program). The distribution by county is based on FY2003 distributions. The estimated recipients are the unduplicated numbers of Medicaid enrollees for the fiscal year, by county. FY2004 Work First and Welfare To Work program allocations have been distributed to individual counties, from Michigan Works! Agency totals, by calculating each county s percentage of Work First participants for each multi-county Michigan Works! Agency, and then applying the county s participant percentage to the Agency dollar allocation. A listing of Michigan Works! Agencies and the counties covered by each agency is available on the Michigan Works! Association website which can be found at T he Michigan League for Human Services seeks to ensure that lowincome individuals and families across the state achieve economic security through a range of public and private supports including public assistance, work and family supports, job training, and education. Through research, analysis, public education and advocacy efforts, the League brings credible and accessible information to the public policy discussion of the critical issues that affect the lives of low-income people.
