We encourage you to evaluate and elect the benefits offered that best fit your personal needs. This guide highlights the many options available to you. 3 3 Eligibility Eligible employees include full-time employees regularly scheduled to work 32 or more hours per week and half-time employees regularly scheduled to work 24 to 32 hours per week. Eligible Dependents Many of the benefit plans also offer coverage for eligible dependents. Eligible dependents include the following: Your legal spouse, common-law spouse (with completed affidavit), or same-sex domestic partner (with completed affidavit). Your children to age 26 (including stepchildren, foster children, legally-adopted children, and children placed with you for adoption), regardless of marital, student, or tax-dependent status (medical, dental, and health care FSA coverage only). This varies for other coverages please see Human Resources for more information. Your dependent children over age 26 who are physically or mentally unable to care for themselves. Qualifying Event You will not be able to enroll or make changes during the year unless you experience a qualifying event. A qualifying event is: Marriage, legal separation, or divorce Birth or adoption of a child Change in employment status for you or your spouse Change in a dependent s benefits eligibility status (e.g., a dependent child exceeding the maximum age for coverage) A significant change in the cost or coverage of your spouse s benefits Change in place of residence causing a loss of eligibility (i.e., moving outside of the service area) Loss of a dependent (death) To change your benefits, notify the Human Resources Department within 31 days of the qualifying event. You will need to provide proof of the change, such as a marriage certificate or record of birth. 3

4 4 Aspen Valley Hospital Health Plan Aspen Valley Hospital offers you and your eligible dependents two comprehensive medical plans from which you can choose. The plans are administered by Regional Care Inc. (RCI). The plans offer a higher level of benefits if services are provided by a participating network provider. You receive the maximum benefit under the plans, and pay a smaller amount out of your pocket, when you seek medical treatment from a network provider. Benefits are summarized below. Mandatory Generic Prescription Drug Program A penalty will apply if a member requests a brand medication when there is a generic equivalent available. The penalty will be the cost difference between the actual cost of the brand and generic medication. There will be no penalty if the doctor specifically recommends the brand choice when a generic equivalent is available. In these situations, the prescribing doctor will need to write dispense as written or DAW on the prescription. Nurse Advice Line Aspen Valley Hospital offers a nurse advice line to all employees at no additional cost through Cooperative Health Services, Inc. Members enjoy access 24 hours a day, seven days a week to multilingual registered nurses with no less than 10 years of clinical experience. Registered nurses advise members on the most appropriate method of care for their medical problems. Call the nurse line at (877) or access the CHS audio health information library at 4

5 5 Aspen Valley Hospital Dental Plan The Aspen Valley Hospital dental plan is administered by Regional Care Inc. (RCI). Under this plan, you have the flexibility to see any dentist of your choice as there is no network of dentists to choose from. You will be reimbursed 90% of the expense that RCI sets as reasonable and customary for the procedure. If the provider charges more than the amount RCI sets as reasonable and customary, you will be responsible for that amount. 6 6 A RETURN ON YOUR HEALTH WELLNESS PROGRAM OVERVIEW The new and restructured Aspen Valley Hospital A Return On Your Health wellness program began on October 22, To be eligible for AVH health plan preferred premiums, you must complete the following activities by the dates below: 1. Register online for A Return on Your Health at 2. Attend one of the AVH employee health screenings by November 14, Complete the online PWP questionnaire by December 21, Once you do these three activities, you can continue to track your quarterly points beginning on January 1, Employees who become eligible for benefits after January 1, 2013 should meet with the Employee Wellness Coordinator to discuss options for participation in the program for the remainder of the 2013 plan year. What is A Return On Your Health? AVH s wellness program, powered by SimplyWell, is a points-based program that provides participants with tools to enhance their own personal health. As a participant, you will log your information and participation, and SimplyWell will keep track of it. To ensure you have access to past health data, we have worked with Healthbreak to migrate the last two years of lab data for all participants into the new SimplyWell portal. Below is an overview of the program components and resources available at no cost to full-time and half-time employees on the health plan: Online sign-up and registration system Confidential health record to track your medical history Online health questionnaire with comprehensive report Onsite health screening Lab work: Chem 13, CBC and lipid panel Height, weight, body mass index, and waist girth Blood pressure Personalized My Home For Health dashboard that prioritizes your health risks Health screening history with personal Health Score feature Daily health news offered by verified sources Robust healthy living resources, health trackers, and other tools Online health and disease management guides Extensive library of educational health modules Monthly electronic newsletter Online tracking of points and the opportunity to earn rewards Note: The SimplyWell program is in full compliance with all federal and state privacy laws, including HIPAA. Data is stored on one of the nation s secure data storage sites. All personal health information (PHI) is stored in individual tables to ensure records are secure and the data is only synthesized when username and password are input. No data is stored on the internet. 6

7 7 Flexible Spending Accounts Health Care FSA The health care FSA allows you to set aside money from your paycheck before income taxes are withheld to pay for eligible expenses such as deductibles, copayments, and other health-related expenses that are not reimbursed by insurance. Regional Care Inc., the FSA administrator, will issue you a debit card, which you can use at the point of service. Additionally, you can submit claims for reimbursement by completing a claim form and submitting it along with an explanation of benefits (EOB) or an itemized receipt from the provider. Due to health reform, the health care FSA maximum contribution is limited to $2,500 for the 2013 plan year. Health Care FSA Grace Period AVH has elected the IRS grace period, which allows members an additional 2½ months (until March 15 of the following year) to incur claims. You will have until March 31 to submit your claims for reimbursement. Dependent Care FSA The dependent care FSA allows you to set aside money from your paycheck on a pre-tax basis for day care expenses to allow you and your spouse to work or attend school full time. Eligible dependents are children under 13, a spouse, elderly parent, or a child over 13 residing in your home who is physically or mentally unable to care for himself or herself. Below are examples of eligible expenses: Day care facility fees Before- and after-school care You may contribute up to $5,000 to the dependent care FSA for 2013 if you are married and file a joint return or if you file a single or head of household return. If you are married and file separate returns, you can each elect $2,500 for the 2013 plan year. You and your spouse must be employed or your spouse must be a full-time student to be eligible to participate in the dependent care FSA. 8 8 About Your Retirement Plan Aspen Valley Hospital offers a defined contribution plan and a cash balance pension plan. Defined Contribution Retirement Plan 401(a) Employer Matching Contributions Account Number (b) Employee Contributions Account Number Upon hire, employees may start contributing their own monies to the 457b plan. After one year of service and 1,000 hours worked in the plan year, Aspen Valley Hospital will match 50% of the employee contribution up to 2.5% of earnings on a per pay-period basis. During 2013, employees under age 50 may contribute up to $17,500 via pre-tax deductions; employees age 50+ may elect to have an additional $5,500 withheld due to catch-up provisions in the law. Funds in both accounts are 100% vested from the day of the deposit. Cash Balance Plan (your plan in lieu of social security) Account Number Aspen Valley Hospital contributes 7.5% of salary to full-time and half-time employees who have worked 1,000 hours during the plan year (January 1 through December 31). The vesting schedule is 100% after three years of vesting service. Social Security Deduction: FICA is not deducted from full-time and half-time employees who work 1,000 or more hours per year. Aspen Valley Hospital contributes to the cash balance retirement plan in lieu of social security. 9 9 Basic Life and Accidental Death and Dismemberment (AD&D) Cigna After six months of employment, all eligible full-time and half-time employees will receive life and AD&D insurance coverage in the amount of two times their annual compensation at no additional cost. Supplemental Life Cigna Employee: You may elect coverage in increments of $10,000 up to a maximum of $500,000 for yourself. Spouse: You may elect coverage in increments of $5,000 up to a maximum of $500,000 for your spouse. Dependent: You may elect coverage in increments of $2,000 up to a maximum of $10,000 for your child(ren). Children 14 days to 6 months old are eligible for $500. Evidence of insurability (EOI) is not required for child(ren) coverage. If you did not elect coverage for you or your spouse when first eligible and you wish to elect coverage now, evidence of insurability (EOI) will be required. Coverage will not take effect until approved by Cigna. Guarantee Issue Amounts Coverage elected for yourself up to $100,000, or your spouse up to $30,000, will be available without any evidence of insurability if you enroll within 31 days of becoming eligible. Important: In order to elect coverage for your spouse or dependent child(ren), you must purchase at least the minimum amount ($10,000) of supplemental coverage for yourself. Supplemental AD&D Cigna You may elect supplemental AD&D insurance for yourself and family members in increments of $25,000 to a maximum of $500,000 for yourself, $250,000 for your spouse, and $10,000 for your child(ren). EOI is not required for any election of AD&D insurance. Survivor Life Benefits Cigna After six months of employment, Aspen Valley Hospital provides survivor life insurance benefits. Upon death of an employee, survivor life pays 30% of the employee s monthly pay to the surviving spouse and to eligible child(ren) with an overall family limit of $6,667 per month. Disability Insurance Aspen Valley Hospital provides short-term and long-term disability coverage to eligible employees at no cost to you after six months of employment: Short-term disability Aspen Valley Hospital: This policy may pay up to 50% of your average earnings for disability, once the extended illness bank (EIB) has been exhausted, for up to 90 days of lost time. Long-term disability Cigna: This policy may pay up to 60% of your average earnings after 90 days of total disability up to age 65 (limitations apply if your disability begins after the age of 60). Extended Illness Bank (EIB) EIB is paid time for a personal disability or that of an immediate family member. Full-time, half-time, and part-time employees accrue EIB based on hours worked. An employee who works 40 hours per week will accrue six days per year. Hours may be used after six months of employment. Each illness will be subject to 24 hours of lost work time before accessing EIB. Unused EIB rolls over from year to year to a maximum of 520 hours. Cigna Additional Benefit Programs Cigna Identity Theft This program provides identity theft resolution services, which include access to personal case managers who work with you to resolve identity theft issues. This program is available to all employees enrolled in any of the Cigna plans offered. Support is available 24 hours a day, seven days a week, and includes all types of identity theft. Contact information for this benefit can be found on the back page of this guide. Cigna Will Prep This program is available to employees enrolled in the employer-paid life and AD&D coverage for no additional cost. It provides employees with proactive tools for planning and protecting their family s financial future. This program provides eligible employees with access to a self-service website to assist with creating a lasting will and testament, living will, and power of attorney. In addition, employees can receive information on estate planning, identity theft, and life insurance calculators. Access this program at 9

10 10 Cigna Additional Benefits (continued) Cigna Healthy Rewards This discount program is available to eligible employees enrolled in the employer-paid life and AD&D coverage for no additional cost. This program can offer up to 60% off of certain health products and services. Cigna Secure Travel Cigna offers a secure travel service benefit free of charge to employees enrolled in the basic AD&D benefit (paid for by AVH), and to employees and their family members if enrolled in the supplemental AD&D family coverage. This service is available 24 hours a day, 365 days a year when traveling domestically or internationally more than 100 miles away from home (on company business or vacation). Cigna Life Assistance This service is available to help you and your household members lead a healthier, happier life by offering support and information regarding issues faced in daily life. It is available to those eligible for long-term disability coverage (paid for by AVH) through Cigna at no additional charge. Master s-level staff can provide a confidential, comprehensive assessment of your concerns and can recommend related services. This program includes 24 hour phone and crisis intervention access. In addition, it includes referrals for up to three free, face-to-face counseling visits for behavioral issues. Aspen Valley Hospital Additional Benefits Mind and body Fitness Vitamins, health, and wellness products Paid Time Off (PTO) Full-time, half-time, and part-time employees accrue PTO hours based on hours worked. PTO hours may be used after three months of employment for holidays off, vacation, and the initial period of personal disability. 11 11 Aspen Valley Hospital Additional Benefits (continued) Alternative Transportation Incentives Discounted RFTA bus passes are available in two different forms from Human Resources: Bus passes $15 for a $40 pass. Seasonal RFTA passes can be purchased and paid for over ten pay periods with a 50% discount applied; amount depends on pass selected. Vanpool: Free vanpool available. Carpool Stipend: A stipend of $2 per person, each way, is paid to employees who carpool more than five miles each way with other AVH employees to and from the hospital. Grand Junction Federal Credit Union Conveniently located adjacent to the hospital: Employees have access to free checking accounts, savings accounts, personal loans, and mortgages. Tuition Assistance Program (TAP) The TAP provides financial aid to employees for engaging in formal education opportunities that result in a degree related to job improvement, job advancement, or a career change at AVH. An employee can apply for TAP benefits after one year of employment. In return for tuition assistance, the employee agrees to a minimum of one year of service with AVH after completion of the program. Pet Care Insurance Aspen Valley Hospital offers a voluntary pet insurance benefit. All employees receive a 10% discount off pet insurance plans through Pet Care Insurance.
