MEDICAL REIMBURSEMENT ACCOUNT The Medical Reimbursement FSA allows you to pay for certain IRS approved medical, dental, vision and hearing expenses not covered by your benefit plan on a pre tax basis. The maximum you may contribute to this account is $2,500 per year. DEPENDENT CARE REIMBURSEMENT ACCOUNT The Dependent Care FSA allows you to set aside pre tax money to be u lized for qualified dependent care expenses so that you and/or your spouse can work or a end school full me. This FSA is limited by the IRS to $5,000 per year ($2,500 if married and filing separate tax returns). LIFE AND AD&D BASIC LIFE AND AD&D Plains All American provides Basic Life Insurance equal to two mes your annual salary, at no cost to you. This benefit is subject to a maximum of $140,000 for truck drivers and $500,000 for all other employees. The company also provides AD&D coverage equal to five mes your annual salary to a maximum of $350,000 for truck drivers and $500,000 for all other employees. OPTIONAL LIFE Plains All American offers Op onal Life Insurance. You can elect up to $500,000 or five mes your annual salary (whichever is less) for employee coverage. Spouse coverage can be elected up to $250,000. Child coverage can be elected up to $10,000. VOLUNTARY AD&D Plains All American offers Voluntary Accidental Death & Dismemberment Insurance. You can elect up to $500,000 for employee coverage. Spouse and child coverage is based on a percentage of the employee s amount (10% 60%) dependent upon who is covered. This is a brief summary of your benefits. Refer to the Master Contract/Summary Plan Descrip on for plan details. 2
4 DISABILITY SHORT TERM DISABILITY (STD) The Company provides Short Term Disability Insurance at no cost to you. This benefit provides 100% of your base weekly earnings for six weeks, then 60% of base earnings. This benefit dura on is 180 days for truck drivers and 90 days for all other employees. LONG TERM DISABILITY The Company provides Long Term Disability Insurance at no cost to you. This benefit provides 60% of your monthly earnings to a maximum of $3,500 per month a er a wai ng period of 180 days for truck drivers and $10,000 per month a er a wai ng period of 90 days for all other employees. EMPLOYEE ASSISTANCE PROGRAM (EAP) Optum provides resources to assist you with the resolu on of challenges involving health, personal, family, financial, legal and work related issues. You may speak with registered nurses and master level counselors who can help you 24 hours a day. 401(K) On your first day of employment you will be eligible to par cipate in the Plains All American 401(k) Plan. You may make pre tax contribu ons of 1% 50% of gross pay not to exceed the IRS maximum ($18,000 for 2016). The Company may match your contribu ons with cash up to an aggregate of 6%. Your before tax account and the company match account are both 100% vested at all mes. Rollover contribu ons may be accepted from other qualified plans upon employment. Catch up contribu ons up to $6,000 as provided by the IRS are allowed for employees age 50 and over or who will be age 50 by year end and who are contribu ng the maximum before tax contribu on. HOLIDAYS, VACATION, SICK LEAVE HOLIDAYS New Year s Day Good Friday M e m or i a l Da y Independence Day Labor Day Thanksgiving Day and Friday a er Christmas Eve Christmas Day Floa ng Holiday (1) VACATION Full me employees accrue vaca on hours each pay period based on years of service. During the first year, a new employee s accrual is prorated from their hire date. Employees may carry over up to 2 weeks of vaca on into the following year. VACATION Years of Service Vacation Weeks A er 5 3 A er 10 4 A er 25 5 SICK LEAVE Full me employees receive two weeks of sick leave per year. 5 CREDIT UNION As a Plains All American employee, you are eligible to par cipate in the Peoples Trust Federal Credit Union. EDUCATIONAL REIMBURSEMENT With management approval, employees who are con nuing their educa on may receive up to 100% reimbursement for tui on and books a er sa sfactorily comple ng courses leading to a degree (up to $5,250). TRAVEL ASSISTANCE Cigna Secure Travel provides emergency medical and travel services, as well as helpful pre trip planning assistance, when traveling 100 miles or more away from home on company business or on vaca on. This protec on is available regardless if you are traveling domes cally or interna onally. The customer service center is available 24 hours a day, 365 days a year. Below is a brief summary of a few services offered through Cigna Secure Travel. Emergency medical evacua on 24 hour mul lingual assistance Pr e trip planning services, including foreign travel IDENTITY THEFT Medical referrals Pr e sc r i p on refill services Assistance with lost or stolen items Emergency travel services Cigna s Iden ty The Program provides resolu on services to help you work through cri cal iden ty the issues you may encounter. The program can assist you with a review of credit informa on to determine if an iden ty the has occurred, help repor ng iden ty the to credit repor ng agencies, and cancella on and replacement of lost or stolen credit cards. One on one assistance is available 24 hours a day, 365 days a year. ONLINE WILL PREPARATION PROGRAM The Online Will Prepara on Program, through Cigna, allows you to create and personalize essen al life and health legal documents online at no cost. These documents include a last will and testament, a living will, a healthcare power of a orney and a financial power of a orney. The online will center is secure, easy to use, and available to you and your covered spouse.
