5 Meeting the needs of you and your family For the 2018/2019 plan year, most of your benefits details remain the same as last year. Even though not much is changing, annual enrollment is always the best time to take a look at your coverage and make sure it still fits the needs of you and your family. Look through this guide booklet to learn more about the coverage available to you and double check that you re making the most of your BP benefits. BP annual enrollment guide 2018/19 5

6 Changes for 2018 This is when you usually find out what s changing for the upcoming year. For 2018 the answer is not much. And that s a whole lot because of you. Since we introduced the BP wellness program in 2010, the average number of health risks throughout BP has been reduced by 38%. You ve done a great job of managing your risks and becoming smarter healthcare consumers. Great work; keep it up! So what changes can you expect for the upcoming plan year? For all plans, premiums will increase slightly for the year ahead. The good news is, BP s rates continue to be below national average! For employees in Washington State, enrolled in the Cherry Point HealthPlus medical option, you ll see additional changes to your coinsurance, as well as a few new enhancements. The BP wellness program is getting a few tweaks, with new live classes, a few new programs and a StayWell app that makes improved wellbeing more convenient than ever! This year, for the BP Million Step Challenge, in addition to the Fitbit, you can also track your steps with several other devices, thanks to StayWell s new platform. And don t forget, if you haven t already claimed your one-time credit, you have until the end of 2018 to use it towards a new Fitbit device. So there you have it.minimal increases, some new enhancements, but mostly steady as you go! 8 Medical plan changes Listed below are the updates and changes for the 2018/2019 plan year. Medical Plan Premiums Medical premiums are increasing for the plan year across all plans. While rate increases vary depending on which plan you select, BP s overall increases remain low and continue to trend below the national average. Employees in the Cherry Point HealthPlus PPO Option Co-insurance changes To continue to standardize our benefits offer, this year will mark the second set of changes for the Cherry Point HealthPlus PPO option. Beginning in April, co-insurance will now align with BP s Aetna medical options generally at 80% coverage for in-network services. This includes services such as, labs, x-rays, outpatient surgery, maternity, hospital semi-private room, chiropractic, home health care and skilled nursing. New! Access to telemedicine and concierge member services Beginning in April, participants enrolled in this plan will have access to these two new services: MDLIVE TeleHealth: This service allows you to phone a board-certified doctor 24/7 for support with a variety of health issues. Then when needed, the doctor can send a prescription directly to your pharmacy of choice. More details coming soon! Benefits Value Advisor (BVA): Get the medical support you need to help to help navigate your health benefits. 9 Dental Rates for dental will increase by one dollar for each coverage level and plan. Review full dental plan details in section three. Updates Regarding benefit limits Per the IRS, the following limits will apply for the 2018/2019 benefits year: Health Savings Account limits have increased to $3,450 for employee-only coverage and $6,900 if you cover your family. Health Care Flexible Spending Account limits have increased to $2,650. Commuter benefits are increasing to $260 for transit transportation and $260 for parking BP annual enrollment guide 2018/19 9

Fortunately, BP has taken out some of the guesswork by offering three PPO options that provide both choice and flexibility: Health+Savings, HealthPlus and Standard. All three options are in the same network, cover the same services and provide the same quality of care. The difference is when and how you pay for care. No one medical plan is right for everyone. 22 Vision plan Vision benefits are provided by VSP. When you elect vision coverage, you are eligible to receive a 100% covered annual preventive care eye exam. You also get plan benefits for eye exams, lenses, frames, and contacts. In addition, VSP provides discounts for PRK and LASIK procedures. For plan details, access the benefits handbook available at LifeBenefits. Vision plan benefit rates You pay 100% of the cost of vision coverage under the BP vision plan. 23 Dental Your dental benefits are provided by Cigna, for both the PPO and HMO options. When you elect dental coverage, you receive yearly, in-network preventive services covered at 100%. 27 Health+Savings is BP s high-deductible medical plan option. It provides comprehensive medical coverage at significantly lower premium costs. It also comes with a Health Savings Account (HSA) that both you and BP contribute to, helping you save for future health expenses. BP annual enrollment guide 2018/19 27

28 Consider Health+Savings With over 50% of BP employees enrolling in the Health+Savings option, this plan has quickly become the most popular BP medical plan option. Not sure what it s all about? With so few changes happening this year, now may be a good time to investigate new ways to make the most of your benefits and your money. One way might be to look into the Health+Savings medical plan option. Now you may be thinking that high deductible health plans can only benefit young, healthy singles. But, with low premiums and a BP contribution of $1,000 per year and an extra $1,000 if you cover your spouse or domestic partner, the Health+Savings option can potentially help you save on health care costs no matter your age, health or family size. Health+Savings qualifies you for a special feature that can help you save even more a Health Savings Account, also known as an HSA. It works a lot like a retirement savings account. You make contributions, and money invested grows over time. Your HSA money can be used for eligible health care expenses and you can take it with you when you leave BP or retire! 29 Is Health+Savings right for you? You ve heard of BP s Health+Savings option, but maybe you weren't sure it would be right for your situation. Let's find out how it's working for people in every phase of their lives and careers. Dave Health+Savings well for me, it s a no brainer really. It s BP s lowest priced medical plan and besides my annual physical, I rarely go to the doctor. Sure, the deductible is higher, but since I earn a $1,000 BP contribution in my HSA each year, I can always use that towards additional medical costs that may come up. For me it s win, win! Marla With two young children and third child on the way, I needed a healthcare plan that would work for all of us. We have the same access to the doctors and providers as we do in the other BP plans. And right off the bat we save over $3,000 a year on premiums compared to the HealthPlus option. During our physicals we also take a metabolic syndrome screening to earn an extra $2,000 contribution from BP. No matter how you run the numbers, for us, this is the best financial choice. Matt Karen and I are both over 55. Don t get me wrong we re still in great shape. But with retirement just around the corner, that might change. I hear some couples can spend $240,000 on healthcare after they ve retired. I never thought Health+Savings would work for us, so imagine my surprise when the decision support tool said it was the best way forward. With this plan, we have much lower premiums, a Health and Savings Account, and because I m over 55, I can invest $1,000 more in catch-up contributions. 31 HSA contributions You contribute to your HSA through tax-free payroll deductions. You can start, stop, or change your contribution amount at any time. BP makes a contribution of up to $2,000 ($1,000 for you and $1,000 for your spouse/domestic partner, if applicable) into your HSA when you and your spouse/domestic partner complete the physician certification form process by 15 December 2018! More details are available at LifeBenefits. You can also contribute to your HSA during the plan year on a pre-tax basis up to $3,450 if you have employee-only coverage or $6,900 for family coverage. Please note that any contributions made to your HSA by BP for completing and returning the physician certification form reduce the amount you can contribute to your HSA during the year. If you have already met the maximum contribution by the time you qualify for the incentive, the contribution payment will be made via payroll, including any and all applicable tax withholdings. Your HSA, your money You can use the money in your HSA to pay eligible healthcare expenses for you and your eligible family members, including things like deductibles and coinsurance. Your HSA earns interest, and the earnings on your balance are also tax-free. You control your HSA, giving you the flexibility to decide how, when, and where to spend your healthcare dollars. If you don t spend the money in your HSA, it can continue to grow, year after year, for use in the future. Unlike with the health care flexible spending account, there s no use it or lose it rule. When your balance reaches $1,000, you can begin investing to help your account grow even more over time. And your HSA is portable it belongs to you. You don t pay taxes on the money when you spend it (as long as it s for eligible healthcare expenses). 37 Login problems? Are you having problems logging in to your online enrollment? This section will help you out. Forgot your user ID? You ll need your BP HR & Benefits Center user ID and password to access the site. Click "I Forgot My User ID" on the LifeBenefits main page. You will be prompted to provide the last four digits of your SSN, your birth date, and, if needed, your ZIP code. You will then be directed to the password page to enter your password. Forgot your password? Click I Forgot My Password on the LifeBenefits main page. If you created a hint when you set up your account, it will display to remind you of your password. If you have no hint on file or still do not remember your password after reviewing your hint, you will be prompted to answer some security questions and then reset your password. 38 Steps to enrollment Online Online enrollment is the preferred way to enroll and the easiest! Follow the steps below to get started! Go to LifeBenefits and log on to the BP HR & Benefits Center online. Go to action needed and select the option to complete annual enrollment. From there, follow the easy steps to enroll and double check the coverage you select meets your needs for the year ahead. When you re done, click Complete Enrollment and then OK. For your elections to take effect, you must complete this step and see the Completed Successfully page. Double check your elections! After you enroll you will receive a confirmation and then at the end of annual enrollment you ll receive a confirmation statement mailed to your home. Review these confirmations carefully to ensure your elections were accurately captured. Over the phone To enroll by phone, call the BP HR & Benefits Center toll-free on to enroll in your plan year benefits. Representatives will be available to assist you from 7 a.m. to 7 p.m. Central Time, Monday through Friday. When you call, be sure to have your BP HR & Benefits Center password ready. 38 Your life. 39 If you don t enroll Online enrollment closes at 11:59pm Central Time on Friday, February 23, If you miss the enrollment deadline, the following automatic defaults will apply. If you re currently enrolled in the BP Medical Plan If you don t take action during annual enrollment to enroll for benefits for the plan year, you will be assigned coverage based on: the coverage you have as of March 31, 2018 plan availability your completion of the wellness eligibility requirements for the HealthPlus or Health+Savings options, if applicable. If you re not currently enrolled in the BP Medical Plan If you don t take action during annual enrollment to choose your benefits for the plan year, you will not have coverage under the plan for Your next opportunity to enroll will be in 2019, for coverage effective April 1, HSAs and other health accounts To contribute to the HSA, health care flexible spending account, or dependent care spending account, you must enroll each year. Your current elections will not carry over. If you elect the Health+Savings option, you will not be eligible for the health care flexible spending account. Also, if you were enrolled in Health+Savings for the plan year, but do not enroll for the plan year, contributions you made to your HSA in the prior year may not be tax deductible, and BP will no longer pay applicable administrative fees. If you think you might be unable to meet a standard for a reward under this program, you might qualify for an opportunity to earn the same reward by different means. Contact StayWell on and we will work to find a program with the same reward that is right for you in light of your health status. Medicaid and the Children s Health Insurance Program (CHIP): If you are eligible for health coverage from BP or another employer but are unable to afford the monthly premiums, you may qualify for a premium assistance program that some states offer to help pay for your coverage. These states use funds from their Medicaid or CHIP programs to help people who are eligible for employersponsored health coverage but need assistance with paying their health premiums. This typically applies to households with an annual income under $44,000. If you or your dependents are not currently enrolled in Medicaid or CHIP, but you think someone might be eligible for either of these programs, you can contact your state Medicaid or CHIP office. Dial KIDS NOW or go to insurekidsnow.gov to find out how to apply. If you qualify, you can ask the state if it has a program that might help you pay the premiums for an employersponsored plan. Special enrollment opportunity: If it s determined that you or your dependents are eligible for premium assistance under Medicaid or CHIP, BP is required to allow you and your dependents to enroll in a company-offered plan. To qualify for this special enrollment opportunity, you must be eligible for BP coverage but not already enrolled. In addition, you must contact the BP HR & Benefits Center and request BP health coverage within 60 days of being declared eligible for Medicaid or CHIP premium assistance. If you enroll within the 60 days, Medicaid or CHIP will pay for a portion of the BP health plan premium cost. For a list of states that participate in premium assistance, go to the plan s summary description at bp.com/lifebenefits, which contains state-by-state information and contact numbers. Reconstructive surgery benefits: The Women s Health and Cancer Rights Act requires the company to notify plan participants of the reconstructive surgery benefits following a mastectomy. Benefits will include coverage for prosthesis; treatment of physical complications of the mastectomy, including lymphedema; all stages of reconstruction for the breast on which the mastectomy was performed; and reconstructive surgery on the non-diseased breast as necessary to produce a symmetrical appearance. Coverage will be provided in a manner determined in consultation with the attending physician and the patient. All benefits will be subject to the applicable deductibles, coinsurance, and copayments. HIPAA special enrollment rights: Under HIPAA, if you lose your group health plan coverage, you may be able to get into another group health plan for which you are eligible (such as a spouse s plan), even if the plan generally does not accept late enrollees, as long as you request enrollment within 30 days. (Additional special enrollment rights are triggered by marriage, birth, adoption, and placement for adoption.) So if you are eligible for coverage in another plan (such as a spouse s plan), you should request enrollment as soon as possible after your current coverage ends. HIPAA privacy notice: When you enrolled in the BP Medical Plan, you should have received the plan s Notice of Privacy Practices. BP regularly updates this to include new regulatory changes. To view the notice, you may request a copy at any time by contacting the BP HR & Benefits Center on or by accessing bp.com/lifebenefits. Click Benefits handbook and HIPAA Notice of Privacy Practices under Related links. Imputed income: If a dependent or domestic partner you enroll in a BP plan does not also qualify as your dependent for federal income tax purposes, there are tax ramifications. Tax law does not exclude BP s contribution toward the cost of your dependent s coverage from your income, meaning that you could incur imputed income on the value of the BP-provided coverage. The law also prohibits you from paying for such coverage on a beforetax basis. You re responsible for notifying the BP HR & Benefits Center if your covered dependent does not qualify as your tax dependent, and domestic partners will be presumed to be nonqualified unless you provide evidence to the contrary. Truthfulness of statements: All coverage under the BP benefit plans is based on the truthfulness of statements made by you and your dependents. By enrolling, you certify that the information you provide about your and your dependents eligibility to participate in BP benefit plans is true and correct. Any fraudulent statement or withholding of information about your and/or your dependents eligibility may retroactively invalidate coverage and be a violation of the BP code of conduct, subjecting you to legal action including termination of coverage and recoupment of benefits paid. Notice regarding BPs wellness program: BP s wellness program is a voluntary wellness program available to all participants in the BP Medical Plan. The program is administered according to federal rules governing wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act, as applicable, among others. If you choose to participate in the wellness program, you will have the opportunity to complete the Health Questionnaire. The HQ is a voluntary health risk assessment that asks a series of questions about your healthrelated activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). If you participate in the Health+Savings Plan, you will also have the opportunity to complete the physician certification form where you will complete a metabolic screening and measure your blood pressure, HDL cholesterol, triglycerides, fasting blood glucose and the size of your waist. Both of these opportunities are voluntary to you, as is participation in the wellness program overall. Also, if you are in the Health+Savings Plan, although you are not required to complete the physician certification form, only employees who do so will receive the BP contribution to their Health Savings Account. Additional points-earning opportunities are available throughout the BP wellness program, some of which are health-related. Please refer to the Wellness Guide for a complete description of each opportunity. If, due to a medical condition, you are unable to participate in any of the health-related activities or achieve any of the health outcomes required to earn an incentive, a reasonable accommodation or an alternative standard is available. You may request a reasonable accommodation or an alternative standard by contacting the BP HR & Benefits Center at The information from the Health Questionnaire and from your physician certification form will be used to provide you with information to help you understand your current health and potential risks, and may also be used in partnership with a StayWell Health Advisor to design wellness activities that suit you specifically. Protections from Disclosure of Medical Information: As further described in the Plan s summary plan description, the BP Medical Plan is required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program co-ordinator and BP s Benefits team may use aggregate information it collects to design a program based on identified health risks, the BP Medical Plan will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment. Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements. The only individual(s) who will receive your personally identifiable health information from the BP wellness program are Alight, StayWell, approved members of the BP Benefits department and Aetna, and all done in order to provide you with services under the wellness program. In addition, all personally identifiable medical information obtained by BP Benefits through the wellness program will be maintained wholly separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of the wellness program will be used in making any employment decision. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with the wellness program, we will notify you in accordance with applicable law. You may not be discriminated against in employment because of the medical information you provide as part of participating in the wellness program, nor may you be subjected to retaliation if you choose not to participate.
