1 SSAC APPLICATION FOR MEMBERSHIP Statement of Membership Policy The primary function of the Saguaro Scramblers Agility Club Inc. (SSAC) is to promote the sport of Dog Agility in Southern Arizona. Membership is suggested for those enthusiasts who wish to support the club s endeavors to bring opportunities for competition to our region. Membership is open to persons over 18 years of age, in good standing with one or all of the following: The United States Dog Agility Association (USDAA) American Kennel Club (AKC), the North American Dog Agility Council (NADAC). Members subscribe to the purposes of SSAC, agree to the By-Laws and Constitution of the SSAC, and the rules of our sanctioning bodies. (USDAA, AKC, NADAC) SSAC is dedicated to providing the best trial environment possible, including furnishing equipment meeting the standards of our sanctioning organizations (USDAA, AKC and NADAC) and ample volunteer labor to ensure an efficient, professional trial. Therefore ALL members are expected to actively participate in the successful operation of our trials by contributing a minimum of 12 volunteer hours per year. (See Dues, pg. #4) Application Requirements Read Carefully!! This completed application must be accompanied by (1) sponsorship of two people who have been members of SSAC for one year, (2) the signed membership agreement and waiver (below), (3) dues payment for current year. By signing this I demonstrate my willingness to volunteer, becoming a contributing member of the SSAC. Volunteering at two trials and attending either a SSAC Board or General meeting will be necessary before application will be approved. SSAC is dedicated to the concept of good sportsmanship and expects this of all members. The SSAC meets monthly. An application will be read at the first meeting of the Board following its receipt, after which the applicant is eligible to participate in club activities but without full member discounts. When the above requirements have been completed, at the next Board meeting, the application will be voted upon: affirmative vote requires 2/3rds of the Board members present. Member sponsorship page (pages 5 and 6) must be included with completed application. Any dues paid in the 4th quarter will apply to the remainder of the year and the next calendar year. Volunteer Position Choices The continued success of the SSAC sponsored events depends entirely on club volunteer support. Seminars, fun runs, practices and trials are absolutely dependent on the voluntary contributions of time and talent by each and every member. As a member you are required to volunteer a minimum of 12 hrs./year (see Dues, pg.4) in one or more of the positions listed below. Please indicate your choice(s). Day of Trial volunteer jobs include: leash runner, gatekeeper, pole setter, assistant course builder, score runner and scribe. # Equipment includes: cleaning after a trial, ongoing repairs and maintenance. ** Hospitality: Transporting trial officials to and from airport/hotel/trial site. Applicant's Signature Membership Agreement & Waiver of Liability Please read carefully, sign and date the Membership Agreement & Waiver of Liability in order for your application to be considered. o I, the undersigned, hereby agree to abide by the constitution and bylaws of the Saguaro Scramblers Agility Club Inc., as well as the rules of recognized agility organizations. o I recognize that failure to abide by these rules and regulations may result in termination of membership from the club o I recognize that all minors must be accompanied by a Parent/Guardian at all times in the training areas o I also agree to hold Saguaro Scramblers Agility Club Inc., its members, directors and officers harmless from any claim for damage, loss, or injury which may be alleged to have been caused directly or indirectly to any person, dog or thing by the act of my dog(s) while in or on any property or location at which Saguaro Scramblers holds classes or related activities. o I hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense by reason of the liability imposed by law upon any of therefore mentioned parties for damage because of bodily injuries, sustained by any person or persons, including myself, or on account of damage to property arising out of or as a consequence of my participation in training. o I hereby confirm that any dog I bring in or on any property(ies) is current on all shots and has no communicable diseases. 5 NOTE: Annual dues will be reduced to $20.00/year when the 12 hour volunteer commitment is fulfilled.
