2 WAIVER Assumption of risks, release of liability, waiver of claims and indemnity agreement *By signing this document, you will waive certain legal rights- including the right to sue.* **PLEASE READ CAREFULLY!!** ASSUMPTION OF RISKS: I am aware that soccer involves many risks, dangers and hazards, including, but not limited to: collision with other players and officials, collisions with goal posts and playing surfaces resulting from imperfections within playing surfaces and willful violence inflicted by players and/or spectators. I acknowledge that participation in this event will expose me to risk of injury or loss to person and/or property. I freely and voluntarily agree to assume all physical and legal risks that may arise from my participation in this event. I further state that I am in proper physical condition to participate in this event. WAIVER, RELEASE AND INDEMNIFICATION: In consideration of the Vernon Coed Tournament Organizing committee allowing me to participate in this event, I, for my heirs, executors, administrators, and assigns, release the Organizing Committee, the Vernon soccer Association, the Vernon National Oldtimers Soccer Club, The North Okanagan Women s Soccer League, the Greater Vernon Parks and Recreation Districts, the tournament directors, organizers, sponsors, officials, and all other organizations, corporations or persons connected with the event or related activities, and their respective officers, directors, representatives, servants, agents, employees, and volunteers, (herein referred to as the Event Organization ), and do herby waive as against the Event Organization all recourse, claims, demands, damages, actions, or causes of action arising out of, or in consequence of my death or any loss, injury, or damage to my person or property which I may suffer arising out of, or in connection with my participation in this event, or any related activity, whether prior to, during or subsequent to the event however caused, and even if such death, loss, injury or damage may have been contributed to or occasioned by the negligence of the Event Organization, other athletes, officials, participants, spectators, or any other personals that I may encounter as a result of my participation in this event or related activities. I further undertake and agree to defend, indemnify and hold harmless the Event Organizations from and against any and all losses, liabilities, damages, injuries, claims demands, costs and expenses arising out of or in consequences of the attendance or participation by me in this even or any of the related activities. ALL PARTICIPANTS MUST SIGN THIS WAIVER PRIOR TO PARTICIPATING TO BE CONSIDERED ELIGIBLE! TEAMS WITH INELIGIBLE PLAYERS WILL FOREFEIT. 2

3 RULES & REGULATIONS players on the field: minimum 5 women, 5 men and goalie can be interchangeable 2. All games are 35- minute halves with a 5-minute half time break. All games tied at the end of regulation time will go straight to a shoot out. Teams must alternate genders during the shootouts with the first shooter always being female. Only players on the field at the end of regulation time will be eligible to participate. 3. Players may only play for one team in any one division. 4. NO slide tackles. Providing there is clearly no one in the area, players are allowed to slide to prevent the ball from leaving the field or intercepting a pass. Slide tackles with contact will result in a yellow card. 5. Players under the age of 19 must have a parent or guardian sign. 6. You may update your roster at the beer garden prior to the start of the game. 7. A) There will be a maximum of 10 points awarded for each round robin game. 3 points for a win in regulation time 1 point for a goal scored by a male player 2 points for a win in a shootout 2 points for a goal scored by a female player 1 point for a loss in a shootout B) To promote cleanliness and fair play, 1 point will be deducted for each yellow card, and 3 points for each red card. 8. IF YOU MUST FORFEIT A GAME, TOURNAMENT ORGANIZERS NEED TO KNOW PRIOR TO KICKOFF. THE TEAM FORFEITING THE GAME MAY BE SUBJECT TO A FINE! 9. The Regional District is enforcing its NO DOGS ALLOWED IN PARKS bylaw. People have brought their pets in the past, so please be aware that the enforcement officers will be patrolling the parks that we are playing at. 10. ZERO TOLERANCE for alcohol will be enforced outside of the beer garden. We have made every attempt to provide a safe, fun and inexpensive beer garden. Consuming alcohol outside of the beer garden is illegal. All referees and tournament officials will be monitoring all fields. If alcohol is openly displayed on the sidelines at any of the fields the game will be stopped until it is disposed of. We greatly appreciate and thank you in advance for your cooperation on this matter. 3
