4 Carefully read and sign the liability waiver South Carolina s Official Training Grounds, LLC (DBA SCOTG ) Release of Liability Agreement and Waiver of certain legal rights. Please read carefully before signing. All applicants MUST sign In consideration for my SCOTG membership acceptances, and the services and amenities to be provided by SCOTG in connection with my membership, I confirm my understanding that: 1. I acknowledge that shooting activities have inherent risk, hazards and dangers that cannot be completely eliminated. I UNDERSTAND THAT THESE INCLUDE BUT ARE NOT LIMITED TO: a. The risk of handling firearms and being near others that have firearms in their possession. b. The risk of personal injury and / or damage to personal property from shooting events. c. The risk of personal injury from the physical requirements of shooting events. d. The risk of possible ear damage from noise. e. The risk of possible eye damage. EYE AND EAR PROTECTION IS REQUIRED AT ALL TIMES ON SCOTG GROUNDS. 2. While SCOTG does maintain shooting club and range liability coverage I understand that each range other than those owned by SCOTG in which an event is conducted is responsible for maintaining liability insurance for that event. SCOTG is not responsible for that coverage or verification of that coverage. Completion of a separate Waiver for each individual range is the responsibility of the range owner not SCOTG, unless owned by SCOTG. I further understand that I will be required to meet and abide by any rules or requirements of the host range as well as those set forth by SCOTG. 3. I understand the risks, hazards, and dangers as described above and have had the opportunity to discuss any questions with an officer of SCOTG. I understand that these activities may require good physical condition and a degree of skill and knowledge. I believe that I have the physical conditioning and degree of skill and knowledge needed for me to engage in these activities safely. I understand that I have responsibilities. My participation in this activity is purely voluntary. I am voluntarily using the services of SCOTG with full knowledge of the inherent risks, hazards and dangers involved and hereby assume and accept any and all risks of injury, paralysis or death. 4. I represent that I am under no type of restraining order which prevents my possession

5 and/or use of firearms. I am able to pass the SCBI and SLED background check and legally free to purchase and own firearms in the state of South Carolina. 5. I assume responsibility for the actions of any and all guests I bring to the SCOTG range. 6. I consent to receive medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during the activities at the shooting range. 7. Last, I, for myself, my heirs, successors and executors, and subrogates, hereby and knowingly and intentionally waive and release, indemnify and hold harmless SCOTG staff personnel, SCOTG and all officers, agents, employees and volunteers from and against claims, actions, cause of action, liabilities, suites and expenses (including reasonable attorney s fees) which are related to, arise out of, or are in any way connected with any participation in any activity including, but not limited to, negligence of any kind in nature, whether foreseen or unforeseen, arising directly or indirectly out of any damage, loss, injury, paralysis, or death to me or my property as services, animals or equipment, whether such damage, loss, injury, paralysis, or death is a result of negligence of SCOTG and course designs or from some other cause. I, for myself, my heirs, my successors, executors and subrogates, further agree not to sue SCOTG, any officers or directors, agents, employees or volunteers as a result of any injury, paralysis or death suffered in connections with my use and participation in any activity connected with SCOTG. I have read, understand and fully agree with the Release of Liability Agreement and Waiver of certain legal rights above and I understand that this Waiver and Release Agreement applies equally to any minor children in my home.
