3 Celebrate our Successes Self-Funded Health Plan - Calendar Year 2013 Medical claims dropped by 12.6% Pharmacy spend was approx. 50% less $700,000 savings High cost claimants are down (20% decrease) 43 claimants (.62% of population) over $50,000 account for approximately 31% of the total medical claim cost Inpatient admissions down nearly 13% Readmission rate decreased by 2.2% 120 spouses moved to their own plan $400k savings Expenses as compared to Budget 18% savings (actual paid expenses compared to budgeted) or $5.4M Calendar Year 2014 $3.8M savings expected No Health Plan Premium Increases in 2015!
4 Health Insurance What s New in 2015 Separate Medical/RX Out of Pocket Maximum s for Select/Base PPO Plans 2014 OOP Maximums Select PPO Base PPO Employee $4k $6k Family $8k $10k Integrated Medical and Rx OOP s 2015 OOP Maximums Employee Family Select PPO IN / UHC Network $4k / $5k (med) $1,500 (Rx) $8k / $10k (med) $3,000 (Rx) Separate Medical and Rx OOP s Base PPO IN / UHC Network $5k / $5,100 (med) $1,500 (Rx) $10k / $10,200 (med) $3,000 (Rx) Currently the Select and Base PPO plans have no Rx OOP limits (an unlimited exposure for the plan member). The 2015 modification will cap prescription out of pocket costs in the future.) IRS regulations already requires High Deductible Health Plans (Choice HDHP) to have prescription costs accumulate towards annual out of pocket maximums.
5 What s New in 2015 Health Savings Accounts Annual Contribution Limits Individual Coverage $3,300 $3,350 - Family Coverage $6,550 $6,650 If you are age 55 or older, you may make annual catch-up contributions of up to $1,000. After the first payroll of 2015, GRMC will contribute $500 in to Individual HSA s and $875 in to Family HSA s. GRMC will also contribute up to an additional $500 to health savings accounts based on completion of wellness incentives.
6 What s New in 2015 Flexible Spending Accounts New Carryover Provision Up to $500 of remaining Health Care FSA dollars can be carried over in to There are special rules where this applies. For example, to carry over:» The participant must be an active member on the last day of the plan year and will be electing for the new plan year» If the participant doesn t elect a Health Care FSA in the upcoming plan year, a manual election will be set up and balances will only be available via manual claim submission.
7 Wellness GRU Healthy You Wellness Incentives for Choice HDHP Participants HSA Seed $500 (Individual) $875 (Family) HSA Incentives Employer will provide up to an extra $500 (Individual/Family) for completion of Incentives. Both the Clinical Health Risk Assessment and Biometric screening will continue to be required and both must be completed to earn wellness incentive dollars Timeline for completion: November 1, 2014 May 31, 2015 Health Risk Assessment Biometrics Two HSA payouts in 2015 July and October $100
8 Wellness Incentives Education Complete Weight Watchers weight loss program (limit 1 time per year) 50 Complete approved Diabetes Education Series (limit 1 time per year) 50 Complete GRU Healthy You Education Class (limit of 5 per year) 10 Complete GRU Cancer Center Tobacco Cessation Program (limit 1 time per year) 100 Complete GRU Stress Management Class (limit 1 time per year) 25 Healthy Lifestyles Exercise Log (30 minutes per day, 3 days/week) for 3 consecutive months 50 Participate in Local Y Team Lean event (1 time per year) 50 Participate in community-based runs, walks, etc. (up to 4 per year) 25 Complete results-based wellness coaching (up to 3 sessions) 50 Preventive Care Obtain Annual Preventive Exam Routine Physical 50 Mammogram for women PAP screening for women PSA/Prostate Screening (for men age 40+) 50 Colorectal cancer screening over age Coronary Artery Calcium Screening (age 55 to 75 for men, age for women) 50 Preventive Dental Screening 25 Vision Screening 25 Hearing Screening 25 Other Donate Blood (minimum 2x/calendar year) 100

11 What s New in 2015 Long-Term Disability Special Enrollment Option for 2015 All employees who previously declined the 60% LTD insurance option may elect this option during open enrollment without requiring Evidence of Insurability Pre-ex condition Pre-ex condition for increases in amounts of coverage means during the 3 months prior to the effective date of any increase in an Employee's amount of insurance the Employee received medical treatment, consultation, care or services, including diagnostic measures, or took prescribed drugs or medicines for the disabling condition. If the pre-ex provision applies to an approved claim, benefits will be paid at the 50% LTD option level.
12 What s New in 2015 Long-Term Disability Special Enrollment Option for 2015 Pre-ex condition The pre-ex exclusion will not apply if Total or Partial Disability begins later than 12 months (1/1/16 or thereafter) after the effective date of any increase in the employee's insurance. The pre-ex exclusion will not apply if you did not receive medical treatment, consultation, care or services, including diagnostic measures, or took prescribed drugs or medicines for the disabling condition during the 3 months (Oct/Nov/Dec) prior to the 60% LTD option effective date (1/1/2015).
