12 Wellness Program Carewise Health has exited the wellness program industry The wellness track will be the only path to earn an incentive in 2017 Effective January 1, 2017, you can self report your wellness activities on the benefits enrollment site or by contacting the Benefits Service Center Requirements must be completed and reported on or before June 30, 2017 to be eligible for the incentive Must be actively employed in August to receive incentive Employees who do not complete their Wellness activities: CDHP enrollees will not receive the HSA wellness incentive contribution All other enrollees will have an increase to their medical payroll deduction effective August 1,
13 Tobacco User Surcharge $180 per year surcharge added to your medical plan premium (prorated per pay) Does not apply to dependents including spouses Self-identify as tobacco product user on enrollment website Employees may update tobacco user status at any time by contacting the Glatfelter Benefits Service Center. Tobacco Cessation is free! Medications Support is available through EAP 13
14 Spousal Surcharge If your spouse has healthcare coverage through his or her employer, but you choose to enroll him/her in the GLT medical plan a surcharge of $200 per month will be assessed. This does not apply to your non-working spouse or if your spouse does not have coverage through his or her employer. Does not apply to your dependent children. Does not apply to dental or vision coverage. 14
