1 Government of Jammu and Kashmir Civil Secretariat, Finance Department (nsurance Division) CRCULAR No: 05 -FD of2018 Dated: Subject: J&K Group Mediclaim nsurance Policy for Employees and Pensioners. A web based application has been launched to facilitate review / correction/ checking of the entries in the enrolment forms submitted by the employees / pensioners recently for the proposed J&K Group Mediclaim nsurance Policy ( 1. Android based mobile users can download this web based application "J&K Health nsurance" from Google play store. Employees can access the above application with their mobile numbers as mentioned in their enrolment forms. nformation provided by the employees/pensioners earlier in the enrolment forms shall pre populate on their screens. The same should be checked and corrected as per the requirements. 2. Employees can check/correct the information in respect of name, age, sex, relationship with dependents, name of employer, name of the organization/ department, DDO's name & his code and other contact details and fill the correct information. Employees are also advised to check other allied information and correct it, if required. 3. Similarly, pensioners who desire to opt for the policy can also check/correct the information in respect of name, age, sex, relationship with dependents, PPO's No, department last served, name of bank branch and/ or treasury from where pension is being drawn and fill the correct information, subject to if he has already deposited his/her enrolment form with the concerned agency. 4. 3 Annexure-A Circular No: O~FD of2018 dated J&K Group Health nsurance Policy for Employees and Pensioners. Mobile based application namely "J&K Health nsurance" to facilitate all the employees and pensioners of J&K Government for reviewing/ correcting/ checking of their enrolment form submitted earlier has been released. How to operate the APP (Android) ' Download the application 'J&K Health nsurance' from Google Play store. ' Open the same and click appropriate option - Employee/Pensioner. ' Enter your mobile No. given in enrolment form, An OTP SMS will be sent to your registered mobile no. ' Kindly feed the same into given field. ' Read & click Agree on disclaimer screen. ' Now click on +sign or on front page left side, option is also given to view the details. ' Your filled enrolment form will appear on your screen. ' Ensure all the information are filled in correctly ' Check the correctness of all mandatory information, Such as Employee Name, DOB, Gender, Department, and Dependent Details and correct the same if so require. ' Likewise other modification in dependents information can also be made. ' At last use proceed and upload buttons for save. n some cases, Error Message such as your mobile No. is not registered can popup due to the following reasons; 2 The Aircel No. users whose No. has been changed to other operator, they won't be able to operate or review the details and shall call on Toll Free number for updating the new Mobile Number. 3 Those who have entered landline no. in the enrolment form also won't be able to check/review the details. 4 Mobile No with 9 digit/not visible/wrong no entered can't get access to view the form in Mobile APP. 5 Those who have not provided Mobile No. in enrolment form can't get the access. 6 There might also be reason in few cases where the form has not reached or yet may be in transit. Note :The facility is provided for android users only and the same can be downloaded from Google play store, further los based application will be ~ released soon in due course of time. 4 Annexure-B Circular No:OS:FD of 2018 dated Website of J&K Group Health nsurance Policy Employees who have already given details in 2017, are existing as enrolled employees and the given mobile number at that time is to be used as Login 10 for this portal. The employees and pensioners are requested to follow the below mentioned steps on the official website of J&K Group Health nsurance Policy for employees and pensioners- to review correct their enrolment forms. ~ How to operate the website. 1. Go to 2. Enter your registered mobile number. 3. Enter the OTP generated to login into your account. 4. Review the details therein and make corrections wherever needed. S. The requisite 000 code may be obtained for your respective 000 and used to submit form with changes. For New Enrolment ~ The employees, who have not deposited their enrolment form till date can now fill the prescribed form (enclosed) and submit the same to Ms. Trinity Reinsurance Brokers Ltd Srinagar/ Jammu through their concerned 000 on most immediate basis within 30 days.
