For details please log on to our website and click "Tender" for details and forms. The Forms are also available in E&OS Deptt., Divisional office, Hazaribag. The cost of application fee will be Rs500/- (non refundable) as DD/Bankers cheque in the name of LIC of India, payable at Hazaribag. Last date for submission of application is upto Hrs. Life Insurance Corporation of India, Hazaribag Divisional Office, E&OS Department, Hazaribag invites application for empanelment from reputed firms/agencies/vendors for supply, installation & refilling of Fire Extinguishers to our Divisional Office at Hazaribag and Branch offices under Hazaribag Division. Indian Firms/Agencies interested for empanelment should apply in a prescribed Application Format (Annexure-A). Last date for submission of application is upto Hrs. Applications duly completed with all documents should be submitted in a sealed cover superscribing as "Empanelment for supply & installation of SMF Battery Note Counting Machine & Fake Note Detector Machine and should reach to the following address on or before up to Hrs. Forms will also be available on all working day from to at office of the Manager (E&OS), E&OS deptt. LIC of India, Hazaribag Divisional Office. Empanelment shall be made subject to fulfilling essential criteria /conditions as per (Annexure-1) given below: Essential Criteria /Conditions for Empanelment (Annexure- 1) (1) The Firm/Vendor should have valid license/certificate issued by Govt./Company. The License/Certificate should clearly mention type of product dealing with and ISI No. (2) Firms/vendors are capable of supplying only make approved by LIC of India. (3) The Firm/ Vendor should be in profession for at least 3 years (Copy of registration certificate must be enclosed). (4) The Firm / Vendor should have registration with state & local authorities for undertaking the profession (Copies of proof to be enclosed).

3 (5) The Firm / Vendor should keep sufficient stock in hand so as to comply with the urgent need without delay. (6) The firm /Vendor should furnish the specific brand or make, in case of authorized dealer. (Copy of valid authorized dealership certificate must be enclosed.) (7) Minimum turnover required for empanelment is Rs.15 lac per year for consecutive 3 years for the category. (8) The empanelment would be done only on the favorable recommendation of the duly constituted committee that would visit & inspect the premises, workshop etc of the applicants.
