Tele : , , os.jodhpur@licindia.com INSTRUCTIONS TO BIDDERS The sealed tender is invited for rate contract under two bid system for the purchase of Media Stationery as per enclosed Annexure - A, B, C & D. The Bidders are advised to follow the following instructions: As two bid system is to be followed, three envelopes duly sealed should be used for submission of tender as detailed below:- (i) Envelope No. 1: Technical Bid (Annexure - A, B & C ) duly completed and signed should be put in this envelope. The envelope should be sealed properly with sealing wax and super scribed as "Tender for Media Stationery" (2017)- Technical Bid. Firm s name and address should be written on the envelope. The DD / Bankers Cheque for tender fee and EMD should be enclosed separately with the technical bid only. Annexure - A should be supported with relevant documents. Page 3 to 6 duly signed with seal of this tender must also be put in this envelop no.1 (ii) Envelope No. 2: Financial Bid (Annexure - D) duly completed and signed should be put in this envelope. The envelope should be sealed properly with sealing wax and super scribed as "Tender for Media Stationery" (2017)- Financial Bid. Firm s name and address should be written on the envelope. Annexure-D should be duly filled with net rates. (iii) Envelope No. 3: Both the Envelope No. 1 and 2, are to be put into this envelope. The firm /supplier should be in the profession for at least 3 years (copy of proof must be enclosed) 2. The firm /supplier should be on the approved panel of at least 3 reputed firms The firm/supplier should have registration with state and local authorities for undertaking the profession (copies of proof to be enclosed) 4. Minimum annual turnover of the company should be Rs. 20 lacs and above during any of the last three years and experience of having executed an order of one PSU/Govt. client for any one of the last three years. 5. The Company / Firm should have a valid PAN Card No. issued by Income Tax Department. 6. Certificate of satisfactory completion of work/supply issued by concerned department/authority/reputed firm must be attached as proof. 7. The firm/suppliers who have been black listed/removed earlier by any office of LIC of India should not apply. Declaration: I/we have read the instructions appended to the form and I/we understand that if any false information is revealed at a later date, any contract made between ourselves and the Corporation, on the basis of the information given by me/us shall be treated as invalid at the sole discretion of the Corporation and I/we will be solely responsible for the consequences and shall make good all loses caused to LIC of India in the process. I/we agree that the decision of the Corporation in selection of tender will be final and binding on me/us. All the information furnished by me hereunder is correct to the best of my/our knowledge and belief. I/we have no objection if inspection of my/our premises/workshop, shop etc. is done by the official of the Corporation including inspection of the quality of any or all items of the tender. Tele : , , os.jodhpur@licindia.com Terms and conditions of the tender The tender should be sent so as to reach this office ON OR BEFORE THE DATE & TIME SPECIFIED. Tender received after that will be rejected. Both the Envelope No.-1(Technical Bid) and No.-2(Financial Bid) are to be put into the Envelope No-3. The envelope should be sealed properly with sealing wax and super scribed as "Tender- for Media Stationery" (2017). The name of firm should be mentioned on the envelope 2. The rates to be quoted in Annexure - D shall be on F.O.R. basis, i.e. inclusive all taxes, GST and transportation expenses, packaging charges,unloading expenses etc and it has to be for brand/ quality prescribed in annexure D. The rates shall be valid up to the next tender or 12 months whichever is earlier. However Tender can be extended for further six month. 3. The Corporation reserves the right to accept any quotation in full or part. This does not necessarily mean that the lowest quotation will be accepted. 4. Successful tenderers will have to provide Security 10% of the contracted value (where the quantum of order placed exceeds amount of Rs. 10 Lacs) in the form of Demand Draft/ Bankers Cheque/Bank Guarantees through Scheduled Banks. The deposit shall not carry any interest. Earnest Money Deposit (E.M.D.) will be forfeited in case the tenderer refuses to execute the Order, either full or part, if placed at the quoted rates. In that event LIC may decide/debar/blacklist the concerned Supplier and the decisions will be final and binding on all concerned 5-Quotation must be given, as per prescribed format as mentioned in Annexure duly sealed and signed by proprietor/director/manager of the firm and should be either TYPE WRITTEN or must be IN INK and should NOT Be with any hedging conditions or overwritten. If the rates are overwritten or not legible,the quotation will be subject to rejection. 6-Corporation reserves the right to accept any quotation in full or part. This does not necessarily mean that the lowest quotation will be accepted. The Corporation may within in its right award tendered job in part to one of the tenderer and remaining job to another tenderer. 7-All deliveries must be made on the basis of F.O.R Divisional Office, LIC of India, Divisional Office Jodhpur IT Department or as per our instructions within the jurisdiction of Jodhpur Division free of charge. If the delivery is not made to place mentioned above, the amount paid for shifting the goods from the place of delivery to IT department will be deducted from the bill amount. 8-If after the supply of materials, it is discovered that materials supplied are not according to the specification accepted, or not running smoothly, Penalty may imposed as decided by our competent authority or SUPPLY WILL BE REJECTED AT THE SUPPLIER S COST and they will have to supply materials exactly according to the specification and in the event of non- compliance with the condition, Corporation will be at liberty to take such action as it deems fit. -: 4 :-

5 9. Work order may be placed in parts depending upon the requirements during the year. The material will be delivered at the IT Department of LIC of India, Divisional Office, Jeevan Prakash,1-West Patel Nagar, Circuit House Road, Jodhpur (Raj.) or any other place that may be specified by the Corporation if so needed within the stipulated Period of 15 days from the date of purchase order, failing which penalty clause will be imposed as below: Period within which supply is received Upto 15 Days % of Penalty NIL 16 to 20 Days 1% 21 to 30 Days 2% 31 to 40 Days 3% 41 Days onwards Order Cancel 10- REFILLING work is to be done at our Jodhpur Division office headquarter, for this work tenderer has to be deputed skilled person as and. 11-NO ALTERATIONS IN QUANTITY OR QUALITY of the items indented will be accepted and the Period of execution and no enhancement in the rate of article shall be accepted unless previously ratified by the Corporation in writing. 12-Any tender not in compliance with given terms and conditions will be liable for rejection. No extra terms and conditions of the vendor will be accepted. If any vendor gives his own terms and conditions, his tender will be rejected without informing him. 13-Tender shall be valid for the period of One Year but Corporation reserves the right for any alteration in period of the tender which will be strictly binding on the suppliers. 14- For Media stationery this shall be a running contract with an option to accept supply within the Range of +/- 25%. The supplier is legally bound to meet our requirements from 75% to 125% of the ordered quantity at our discretion during the contract Period. We will place the order depending upon our requirement. 15- Payments will be made through NEFT, only after the complete and successful supply of order and on duly verification of quality of goods supplied with the quality/brand/company mentioned in tender. No advance or ad hoc payments will be made. T.D.S. shall be deducted as per IT rules. 16- Quantity of items during the period mentioned is approximate however Corporation reserves the right to decrease the quantity according to future need or any changes if and this will be acceptable to all. 17- The Corporation reserves the right to cancel the contract/annual rate contract without assigning any reason at any time by giving 30 days notice in advance in case of simple termination contract/annual rate contract but in case of breach of the terms of the contract/annual rate contract may be terminated forthwith. -: 5 :-

6 18 -If it is found that the vendor is incompetent to complete the job or is unable to follow the instructions given, the order may be withdrawn from such a vendor and he will be intimated of cancellation of order without any damage or compensation. 19. If at any point of time the Vendor is blacklisted from any of Offices of Corporation then this Contract will be terminated immediately. 20- After engaging the services of the firms, if it is found that the services are not rendered to the satisfaction of the Corporation, the services will be terminated immediately and Corporation shall have the right to engage the services of any of other firms as it may deem fit. In this course if any financial loss/damage to reputation of Corporation is caused, the firms shall be legally bound to make good the same in monetary terms to be decided by LIC of India. 21 -Any dispute arising out of this acceptance shall be referred to for Arbitration to The Sr. Divisional Manager, L.I.C. of India, Divisional Office, Jodhpur and his decision shall be binding on the Firm/Vendor/Manufacturer. The Firm/Vendor/Manufacturer shall not raise any question of the competence of the Sr. Divisional Manager to act as sole arbitrator. The competent authority reserves all the rights to accept and/or reject any tender without assigning reasons thereof. Any dispute arising out of or relating to this tender shall be deemed to have arisen in JODHPUR and shall be under adjudication a court in JODHPUR. 22- The tender notice is available at our official website: THE INSURANCE LAWS (AMENDMENT) ACT, In terms of provisions of Section 33 (3) of The Insurance Laws (Amendment) Act, 2015, Insurance Regulatory and Development Authority of India (IRDAI), is authorized to verify II such books of account, register, other documents and the data base in the custody of the contractor in respect of service outsourced by the LIC of India. It shall be the duty of the contractor to provide such documents/statements/information as may be by IRDAI within such time as may be specified by IRDAI. 2. In terms of provisions of Section 33 (4) of The Insurance Laws (Amendment) Act, 2015, Insurance Regulatory and Development Authority of India (IRDAI), if it considers expedient to do so, may direct any person hereinafter referred to as Investigating Officer, to make an investigation as specified under Sec.33 (1) or carry out an inspection as specified under Section 33 (2) of the Insurance Laws (Amendment) Act, 2015, who may examine on oath any Manager, managing Director or Other Officer of the service provider or contractor where the services are outsourced by LIC of India.
