1 CLARIFICATIONS ON RFP TENDER DOCUMENT FOR THE SELECTION OF INSURANCE COMPANIES FOR IMPLIMENTATION OF RSBY IN 33 DISTRICTS OF GUJARAT Please consider the following clarifications with regard to the RFP document as discussed during the pre-bid meeting held on 15 th February, Page-14 Under clause: 5.1, sub clause a: It is further added that The Insurer will provide RSBY benefits to senior citizens aged 60 years and above with a topup of INR 30,000/- amount as an additional amount per senior citizen for secondary and tertiary treatment. The scheme would be cash less and run on the RSBY platform as an add-on cover. Therefore, available family floater cover under RSBY gets raised by Rs. 30,000/- per senior citizen in case a family comprises of at least one senior Citizen. This add-on cover of Rs. 30,000/- per senior citizen will be for treatment of senior citizen only. Senior Citizens are also eligible for cover under the primary floater of Rs. 30,000/. The premium will be paid based upon the RSBY Beneficiary Families enrolled which has at least one senior citizen. The premium for such families shall be on per family basis, irrespective of the number of senior citizens existing in the family. Page 36: Under clause 16: It is further added that Insurer shall establish a separate RSBY Project Office at Gandhinagar for coordination with the State Government/Nodal agency at the State Capital on a regular basis. Page-42 Under clause: 22.1, sub clause a: Average Family Size of enrolled family should not be less than 4.5 shall be applicable to fresh enrollments only. 1 P a g e

2 Page 52 Under PART II: clause 1; sub clause 1.1- Qualification Criteria and sub clause 1.2 amendments are as follows: 1.1 Qualification Criteria Only those insurance companies which are registered with IRDA for at least three continuous years as on the Bid Due Date and meeting the criteria as defined below shall be eligible to submit a Bid for award of the Contract. The conditions mentioned below shall be the Qualification Criteria. If any Bidder fails to meet the Qualification Criteria, its Bid shall be rejected. The qualification criteria are as follows: I. Parameters for All Companies a. Nature of Entities i. The Bidder should be a registered private or public owned insurance company incorporated under The Companies Act, 1956 and/or 2013, in India. ii. Insurance companies shall not be entitled to form a consortium. If an insurance company does not meet the Qualification Criteria on its own merits and forms a consortium with other insurance company(ies), then the Qualification submitted by such consortium shall be rejected. b. Technical Parameters of Qualifications for all companies: i. The company should be registered with Insurance Regulatory Development Authority (IRDA) to carry out health insurance business. ii. The company shall be registered with IRDA for at least three years. iii. The company shall have a group health cover policy of at least 40,000 lives in each of the last three years. 2 P a g e

3 II. Documents Required: The bidder shall submit the following documents as part of the Qualification bid: i. General Information about Company. ii. Registration and Latest Renewal Certificate of the Company with IRDA. iii. A copy of document which provides proof that the Insurance Company has a group health insurance policy covering at least 40,000 Lives for each of the last three financial years. iv. Undertaking from the bidder for unconditional acceptance of terms and conditions of Tender. III. Financial Documents: i. Last 3 Year s Audited Balance Sheet with Auditors Report. ii. Last 3 Year s Profit & Loss Account. iii. Memorandum and Article of Association of the Company. Note: For evaluation of the Technical Bids, the State Nodal Agency will apply theses evaluation criteria as Appendix 16. Page 58 Under clause 6: sub clause 6.1: It has been added that the Technical Bid (including all of the documents listed above) shall be submitted electronically on as well as submission of the same in one hardcopy in person to this office (address for communication) before 16:00 hrs of 25 th February, Page 61 Under Clause: 8.4, sub clause c: Point ii to vii has been amended as follows: ii. The bidder with the lowest premium rate for RSBY Inpatient care (LP1), RSBY Senior citizen (LS1) and RSBY Outpatient benefits (i.e. sum of premium rate for Inpatient care, Senior citizen care and Outpatient 3 P a g e

4 benefits) will be awarded the contract provided the bidder also have the lowest premium rate for RSBY Senior citizen (LS1) and RSBY Outpatient care (LO1). iii. If LP1, LS1 and LO1 are different bidders then the LP1 bidder will be awarded the bid provided LP1 is ready to match rate of LS1 and LO1. iv. If LP1 is not ready to match LS1 and LO1 rate then the LS1 bidder will be awarded the contract provided LS1 bidder is ready to match LP1 and LO1 rate. If LS1 is not ready to match LP1 and LO1 then the LO1 bidder will be awarded the contract provided Lo1 bidder is ready to match LP1 and LS1 rate. v. If LP1, LS1 and LO1 are not ready to match the bid price then LP2 bidder will be awarded the contract if they are ready to match LP1, LS1 and LO1 and so on. vi. If LP2 is not ready to match LP1, LS1 and LO1 then LS2 bidder will be awarded the contract if they are ready to match LP1, LS1 and LO1 and so on. vii. In case wherein no bidder agrees to match the lowest bids of LP1, LS1 and LO1, then in such circumstance the SNA shall have the authority to call for re-submission of only financial bids from all the bidders again. Page 75 Under Annexure-F: sub clause 14: has been amended as This Bid shall be governed by and construed in all respects according to the laws for the time being in force in India. The competent courts at Gandhinagar will have exclusive jurisdiction in the matter. * The financial quote should be without Service tax. 4 P a g e
