1 SUMMARY OF THE WORK PEST CONTROL SERVICES, PROJECT# P2015(16) - 14 Definitions: i. A BID is a complete and properly signed proposal to do the Work or designated portion thereof for the sums stipulated therein, submitted in accordance with the Bidding Documents. ii. ADDENDA are written or graphic instruments issued by the Architect / Owner prior to the execution of the Contract which modify or interpret the Bidding Documents by additions, deletions, clarifications or corrections. iii. ALTERNATE Bid is an amount stated in the Bid to be added to or deducted from the amount of the Base Bid if the corresponding change in the Work, as described in the Bidding Documents, is accepted. iv. BASE BID is the sum stated in the Base Bid for which the Bidder offers to perform the Work described in the Bidding Documents as the base, to which work may be added or from which work may be deleted for sums stated in Alternate Bids and/or Unit Costs. No exclusions will be accepted and at the discretion of the owner, may result in disqualification of the Bid proposal and determined non-responsive. v. BIDDER is a person or entity who submits a Bid Proposal. vi. BIDDING DOCUMENTS include the Invitation to Bid, Contractors Check List, General Instructions to Bidders, Bidder s Forms (Bidders Proposal & Signature Sheet, Bid Bond Form, Non-Collusion Affidavit Form, Substitution Request Form, Subcontractor Listing Form, Contractor Qualification Form), Summary of The Work, General Conditions, applicable Washington State Prevailing Wages, the Drawings, the Specifications and all Addenda issued prior to and all Modifications, issued after release of the Base Bid Documents. vii. CASE BY CASE will generally be work that is requested by Metro Parks Tacoma on a case by case basis, dependent upon the needs of the district. This may or may not include regular hours, after hours, holidays, or emergencies. viii. LIQUIDATED DAMAGES is the amount of $ per working day to be assessed for failure to physically complete the contracted work within the physical completion time specified. Such damages will accrue separately for each phase or stage of work as specified. In the event damages occur on a concurrent date, the larger of the two damages will apply for such days. ix. PAYMENT APPLICATION Process by which monthly pay requests are submitted to Metro Parks Tacoma, Parks & Natural Resources, Attn: Doreen Klaaskate x. PROJECT WORK to include a fixed start and completion date, detailed scope, and a total base contract amount, including alternates. This work will note specific project location(s) at time of award. 2 xi. SCHEDULE - work associated with each of the service dates and locations. Generally, this will include but not be limited to routine maintenance, preventative maintenance, or seasonal work. xii. SUB-BIDDER is a person or entity who submits a Bid to a Bidder for materials or labor for a portion of the Work. xiii. SUBMITTAL Specified number of copies data / cut sheets, drawings & samples of proposed products or alterations to be installed for the completion of said work, provided to Metro Parks Tacoma, Parks & Natural Resources for review in assessment of proposed changes. xiv.unit PRICE/COSTS identifies a per each sum (as specified in the Bidder s Proposal ), used by the contractor to establish the Base and/or Alternate Bid, and to develop additions or deletions of services or material as described for future reference. At the discretion of MPT, the unit prices will be used in a manner that attains the best and lowest responsive and responsible award(s) for the successful Bid. 3 SCOPE OF WORK The scope of this project shall be for materials and labor to perform inspection, emergency and professional services at District-wide locations, which may or may not, include Enterprise Funds locations, and as otherwise identified and referenced in these documents. Metro Parks Tacoma is seeking bids to provide Integrated Pest Management Service (IPMS) on a regular monthly scheduled basis and in response to some special periodic needs, such as bees, wasps, and other pests for its major facilities listed below. Additional facilities may be requested on an as-needed basis. Also, requested on an as needed basis is riddance and disposal of common nuisance animals i.e. squirrels, raccoons, possums, and bats as well as dead animal removal. Refer to the list of Locations listed under Description of Unit Prices. The Park District is seeking bids for the biennium, expiring December 31, For budget tracking purposes, the District is requesting a separate bid for each of the facilities below. One contract will be awarded for all facilities to the extent that the budget will allow, with additional facilities as funding becomes available on an as needed basis. The IPMS should at a minimum address the following pests: roaches, silverfish, ants (excluding Carpenter and Pharaoh), rats, and mice. Wasps, bees, and hornets shall be included at time of service, below 10 feet. The District requires preventative treatment against carpenter ants at the following locations during peak activity season (March-June) Metro Parks Tacoma Headquarters, Fort Nisqually, the Pt. Defiance Rose Garden Gazebo, and the Wapato Maintenance Shop back building. The service provider will conduct routine visual inspections of pest-prone areas at all facilities listed above, for signs of active pest infestations, and for conditions, which are conducive to attracting or harboring pests. Each facility shall have multicatch traps installed inside the facility and tamper resistant (not soft plastic) bait stations outside secured to the ground. The proper identification of pests by the IPMS provider will ensure a proper approach to eradicating the pest population. In the event an IPMS specialist cannot identify a pest, technical support from a staff entomologist can perform identifications of unusual or unique pests. Strategically placed insect monitors that trap a portion of the pest population should be used to help identify areas needing attention. Once IPMS has identified the pests and developed a treatment plan, a map of the facility with all equipment to be used in the eradication process shall be developed. All equipment shall be numbered in accordance to the plan and should be dated by the service staff on every visit to insure all equipment was physical inspected at the time of each visit. A copy of the treatment plan / map shall be provided to Metropolitan Park District staff at each facility for their understanding to avoid possible interference by District staff with the equipment. In addition, an on-site logbook should be present that documents each visit and identifies findings of each physical inspection. A regular debrief on the findings of every service visit to insure the designated contact at the facility is aware of current findings and pest activity. Based on inspections, and trending developed through the inspections, pest thresholds should be established to identify problem areas. Once thresholds are reached, action plans should be developed and agreed upon by the IPMS professional and the Park District representative. The IPMS should have well-trained, experienced technicians that are locally based to ensure a knowledge of the area local pest pressures. This will also insure availability in case the Park District has an immediate service need. In addition, the IPMS provider should have technical resources available to the local professional, including a board certified Entomologist and a registered & licensed sanitarian. 4 When a problem occurs the Park District should be able to contact the IPMS provider and receive a minimum eight-hour response time, seven days a week. The IPMS provider should also be prepared to commit extra time and resources to address a concern until it is resolved. The IPMS provider should have an internal Quality Assurance program staffed by dedicated QA Inspector(s) that regularly assess the quality of the program in place. The IPMS provider is also requested to provide the following information: A. An example of written action plans B. A comprehensive written Integrated Pest Management Program C. A summary of Technician retention D. Three years of experience and available technical resources of the service provider E. The degree of initial and ongoing training provided to the field technicians. Including: new techniques focusing on prevention and reduction of pesticide risk. F. Ability to demonstrate mastered competency within the industry, and provide referrals from existing clients that will be willing to provide the Park District with feedback. G. Ability of the IPMS provider to provide a comprehensive service offering of pest and sanitation issues, including but not limited to: 1. Anobiids / Deathwatch Beetle 2. Ants 3. Beetles, other 4. FRIDAY, DECEMBER 5, 2014 To: Metropolitan Park District of Tacoma "Owner" 4702 S 19th St Tacoma, WA The undersigned Bidder declares that he has read and fully understands the Notice Inviting Bids and each and every other Contract Document referred to therein and agrees to all of the terms, conditions and provisions contained therein; that he has examined the site of the work and has made the investigations and formed the estimates as to all conditions and contingencies referred to in and required by the Contract Documents, and he proposes and agrees that if his bid as submitted in the Proposal be accepted, he will contract in the form provided to perform all of the work and in the manner required by the Contract Documents and to complete the same within the time stipulated; that he will accept in full payment therefore the prices named herein. Said prices are to include and cover the furnishing of all materials, the performing of all labor requisite or proper, supervision, overhead, profit, taxes (excluding State sales tax), and the providing of all necessary machinery, tools, appurtenances, equipment and other means required to fully complete this contract, except as otherwise specifically provided in the Specifications. 2. Bidder further agrees that he will sign the contract in accordance with the Proposal as accepted and furnish the required bonds within ten (10) days from date of mailing of said notice of acceptance to him at his address as given below or within such additional time as may be allowed by the Architect / Owner, but in any event within twenty (20) days after said mailing. 3. Bidder agrees that his Proposal shall remain open and not withdrawn for a period of not less than sixty (60) days from the date of opening bids. State the amount to complete all base work per the contract specifications and drawings; including all equipment, demolition, labor, materials, contractor s overhead and profit, and all other incidentals required (not including WSST) to complete the work, no exclusions. Unit price sheet must be in package for bid to be responsive. The Contract shall be awarded to the lowest and most responsible bidder and/or the best combination of. Metro Parks Tacoma reserves the right to waive informalities or technical defects as of the interest of the Owner may require. Metro Parks Tacoma, Parks & Natural Resources will consider the quality of the Bid Proposal, compliance to desired specifications, bidding regulations, and advertisement for quotes, and other factors to determine the best price offer. Consideration may also be given to: a. The ability, capacity and skill of the Contractor to perform the project or provide the goods and services; b. Whether the Contractor can perform the project or provide the goods or services promptly, or within the time specified, without delay or interference; Bidder s Proposal, Metropolitan Park District of Tacoma, Page 1 of 4
6 c. The reputation, experience and efficiency of the Contractor; d. The quality of performance on previous contracts or purchases; e. The previous and existing compliance by the Contractor with laws and regulations relating to contracts and services; f. The sufficiency of the financial resources and ability of the Contractor to perform the work or provide the goods and services being purchased; g. The ability of the Contractor to provide future maintenance and service for the goods or services purchased; h. The number and scope of conditions attached to the quotation; and i. The payment terms/billing procedures, quality, delivery times, taxes, freight charges, shipping points and reliability of Contractor and its vendor(s). Additions or deletions may be negotiated by the Metro Parks Tacoma representative and a change order to the contract issued if an agreement is reached. Added work not cited in the contract may, at the option of Metro Parks Tacoma, have added work performed at the labor rate per hour and cost per trip as bid below. Owner reserves the right, but without obligation, to weigh the irregularities and informalities. The Owner further reserves the right to reject any portion of any bid, and/or to reject all bids. This Bidder s Proposal will potentially result in a contract for a period of one (1) to two (2) years. Construction shall commence immediately upon receipt of the Notice To Proceed by Contractor, or as determined in the contract and shall be completed and invoiced no later than December 1 st of the respective year. All costs associated with this contract is incorporated in the bid price of this project. This includes, but is not limited to all misc. labor, installation, benefits, freight, and materials. Liquidated Damages shall be as a result of the Contractor s failure to meet deadlines in accordance with the Bid Documents and Project Schedule. 6.
