The Solid Waste Authority reserves the right to accept or reject any and all proposals or any one item in a proposal. 2 HORRY COUNTY SOLID WASTE AUTHORITY INSTRUCTIONS TO PROPOSERS 1. This request for proposal includes the following: A. Instructions To Proposers B. Specifications 2. Proposal (s) will be examined promptly after opening; immediately thereafter, all proposals will be tabulated with said tabulation being made available to all participating. It is not a practice to award any proposal until the Authority has had ample time to review each Proposal. Award will be made however, at the earliest possible date. If the mail is delayed beyond the date and hour set for the proposal opening, Proposal (s) thus delayed, will NOT be considered. Any Proposal not present at appointed time will not be opened. Facsimiles will not be accepted. 3. TAXES - The Authority pays South Carolina State Sales Tax. The Authority is exempt from Federal Excise Tax and will issue exemption certificates as requested. 4. AWARD OF PROPOSAL - Award of Proposal shall be made to the lowest responsible proposer meeting the Specifications, taking into consideration the following: A. Superior Quality B. Adequate Maintenance and Service C. Past Experience with Company's Equipment D. Company's Reputation E. Known as Technically Good and Innovative Company F. Delivery Date G. Guarantees/Warranties H. Performance of Proposer s equipment in hands of other agencies, plants, and firms. 5. Each proposer must submit a proposal (s) on the blank form attached. The Proposer shall sign his proposal form correctly and proposal (s) may be rejected if there are any omissions, alterations of form, additions not called for, conditional proposal or any irregularities of any kind. 6. DEVIATIONS - Any deviations from these Specifications MUST be noted in detail and submitted in writing with this PROPOSAL. The absence of this Specification deviation will hold the Proposer strictly accountable to the Specifications as written herein. Failure to submit this document of Specification deviation, if applicable, shall be grounds for rejection of the item (s) when offered for delivery. 7. CHANGES - Any changes in Specifications after the Purchase Order/Contract has been awarded, must be with the written consent of the Authority; otherwise, the responsibility for such changes shall be with the Vendor.

INSURANCE: Contractor shall at all times carry Workers Compensation Insurance in the amount of $500,000.00; Commercial General Liability Insurance including bodily injury and property damage in the amount of $1,000,000.00; Automobile coverage in the amount of $1,000,000.00; and Professional Liability Coverage in the amount of $1,000, Insurance certificates will be provided to the Authority at the time of the acceptance of the contract, listing the Authority as additionally insured. 15. The Authority reserves the right to reject any or all Bid Proposals. It further reserves the right to waive technicalities and formalities in proposals as well as to accept in whole or in part such proposals or proposals where it deems it advisable in protection of the best interests of the Authority. 16. Any material included with the Proposal becomes the property of the Authority and are not returnable. 17. Any Proposer aggrieved in connection with the solicitation or award of a contract may protest to the Executive Director in writing within 7 days after such person knows or should have known of the facts giving rise to the grievance.

5 HOUSEHOLD HAZARDOUS WASTE SERVICES BID SPECIFICATIONS I. INTRODUCTION: The Horry County Solid Waste Authority, Inc., is requesting proposals from qualified firms to provide a turnkey service for the disposal of household hazardous waste. The Authority currently operates a permanent household hazardous waste collection facility at the Authority s landfill located at 1886 Highway 90, Conway, South Carolina. The facility is open Tuesday Friday from 8:00AM 1:00PM and on Saturday from 7:00AM 3:00PM. II. SCOPE OF WORK: The proposal shall provide for a complete turnkey service for the mobilization/demobilization, sorting, packaging, loading, documentation for shipment, transport and disposal of household hazardous waste. This includes providing any additional necessary packaging materials. 1. The following are considered household hazardous waste under this scope: Paint and paint thinner (oil based and latex) Oven cleaners, drain cleaners Solvents and degreasers Furniture polishes and waxes, furniture strippers and finishing products Swimming pool chemicals Cleaners: Including rug and upholstery, bathroom cleaners, all purpose cleaners Used motor oil Antifreeze, gasoline, hydraulic and brake fluid Pesticides, herbicides, fungicides and insecticides Batteries Fluorescent light tubes Propane Cylinders Aerosols

6 Flammables Combustibles Acid and Base Waste Oxidizers Unknowns 2. The following items are not included as household hazardous waste and shall not be collected by the successful proposer: Radioactive materials Infectious or biological waste Explosives/shock sensitive materials, ammunition PCB s 3. The proposal shall provide a cost for handling unknowns. This listing must include in detail the cost for identification, sorting, packaging and loading these types of materials. Also included in this listing will be the cost for any and or all Chemist, Technicians, per diem, PPE, mobilization and demobilization. The successful proposer shall provide to the Horry County Solid Waste Authority, Inc. a listing of all waste collected and packaged for disposal prior to leaving the site. The successful proposer shall complete all manifest and shipping papers and provide such completed records within ten (10) working days to the Authority once receipt at a disposal facility is complete. The successful proposer must meet all Federal and State Regulatory requirements. This contract will be awarded to the successful proposer for one year beginning November 1, 2018 and ending October 31, The Solid Waste Authority will have the option of extending the contract for one (1) year by notifying the successful proposer 60 days prior to October 31, This contract will not be extended beyond October 31, 2020.

7 III. HOUSEHOLD HAZARDOUS WASTE PROPOSAL REQUIREMENTS 1. Briefly state the proposer s understanding of the services being sought by the Authority. 2. Identify by name and title the individuals who would be in charge of providing these services. Because of the scope and complexity of the project, the Authority expects that the Project Manager will be a principal experienced in household hazardous waste collection. Identify specific contracts on which that Project Manager is working and give assurances that he/she will be able to provide the necessary oversight and attention to this project. 3. Explain the Proposer s knowledge and understanding of Federal and State of South Carolina environmental regulatory requirements and experience working with the relevant regulatory agencies. IV.
