1 SOLICITATION ADDENDUM Date: Subject: Provide Maintenance Services at SCI-Pittsburgh Solicitation Number: Due Date/Time: /2:00PM Addendum Number: 2 To All Suppliers: The Commonwealth of Pennsylvania defines a solicitation Addendum as an addition to or amendment of the original terms, conditions, specifications, or instructions of a procurement solicitation (e.g., Invitation for Bids or Request for Proposals). List any and all changes: Vendor Sign-in Sheet, Questions and Answers (1 through 20) Type of Solicitation: Electronic Bid (SRM) - Review the Questions section of your solicitation response to ensure you have responded, as required, to any questions relevant to solicitation addenda issued subsequent to the initial advertisement of the solicitation opportunity. Except as clarified and amended by this Addendum, the terms, conditions, specifications, and instructions of the solicitation and any previous solicitation addenda, remain as originally written. 3 RFQ # Questions and Answers (1 through 20 ) 1. At the site visit it was discussed about the continuously high conductivity (between 7000 and 8000) in the boilers. Dave Thomas stated that this has been a problem for a long time. The boilers we are currently using are 300 hp. From August 6, 2017 to date we have purchased over 6, worth of boiler chemicals. The winter boiler is over 12,000 hp and will require massive amounts of chemicals. Would the state be willing to purchase the boiler chemicals or should it be priced into the bid? The awarded contractor is responsible for purchasing boiler chemicals. 2. Can the DOC share a copy of the Site Tour Sign In sheet? The vendor sign in sheet is attached to Addendum What is the dollar threshold for temporary repairs vs. permanent repairs? There is not a dollar amount for temporary or permanent repairs. 4. How will emergency repairs be addressed after hours when DOC approval may not be available? After hours, a temporary repair at the expense of the contractor should be performed. The DOC on-site manager should be informed of emergency repairs on the next business day. 5. REF. Item VII. SOW, Payment Terms. Q. Will the Maintenance Services Contractor be reimbursed full costs for actual services including any mark up for specialty repairs or emergency services outside of the base cost? Temporary repairs are performed at the expense of the contractor. For failure of non-consumable equipment or fixtures that requires repair or replacement, the contractor shall obtain approval from the DOC on-site manager before proceeding. If the work is authorized, contractor will be reimbursed based on a time and material basis. Payment for repair and replacement work shall be contingent upon contractor providing the Department updated as-built drawings.

Page 3 SOW, DOC will remove the majority of fire extinguishers on site, but will leave a limited number for use by the contractor(s) in the boiler plant, guardhouse, and the primary entrance point in other buildings. The maintenance contractor will be responsible for inspection and maintenance of these fire extinguishers. Q. Can the DOC provide a count of the fire extinguishers that will be left under the responsibility of the Maintenance Services Contractor? Yes, the count of fire extinguishers will be provided to the contractor awarded the contract. 7. For security purposes, will the DOC provide the Maintenance Services Contractor with radio communication with Security Personnel? No. Each contractor is responsible to connect, activate, and pay for telephone and/or data services in their own name. 8. It is our understanding that we would make only temporary repairs, then notify David of the DOC. My question is does a bucket catching steam condensate drips constitute a temporary repair. Yes, if there is no other way to make a temporary repair. 9. Would the DOC or the winning contractor be responsible for the pressure vessel inspections? The contractor is responsible for all work and costs associated with maintenance and inspections of the boilers. 10. Can the monthly maintenance guard tour reports, inspection reports and PM reports along with the Boiler and chemical reports be submitted electronically? Yes, it is preferred by .

Utility Plant Operations, paragraph 1, states, Maintain 24/7 on-site coverage by a NIULPE licensed boiler operator responsible to monitor and maintain operation of the boiler plant systems However, at the walkthrough, it was stated that the licensed operator only needed to be oncall. Does the licensed operator have to be on-site 24/7 or on-call 24/7? Please refer to addendum number 1 posted on emarketplace at Utility Plant Operation, Paragraph 3, states, Maintain at least one NIULPE licensed boiler operator, who possesses a current license from the City, to be on-site for one shift a minimum of seven days/week. How many hours is a shift required to be to comply with the Bid, expectations of the DOC, and the City of Pittsburgh? A shift for a boiler operator is defined as one (8) hour period minimum. Please refer to addendum number 1 posted on emarketplace at The City and NIULPE have six different classes of boiler operators, as defined by NIULPE Schedule C. What class of operator is acceptable to meet the IFB requirement, Section A. Utility Plant Operation, Paragraph 3, that states a NIULPE licensed boiler operator who possesses a city of Pittsburgh license must be on site for one shift seven days a week? Please refer to addendum number 1 posted on emarketplace at The Bar Screen Building/Sewer Pump Building is required to be checked for proper operation daily. Will the DOC repair the pumps and pipes in the Bar Screen Building/Sewer Pump Building, so the equipment is operating properly before the contractor start date? No, DOC will not ensure that the pumps and pipes in the bar screen/sewer pump building will be repaired prior to award of contract. 15. RFP says the contractor is responsible for their own internet, phone and computer equipment. Does this include the computer for the BAS system? No, DOC will supply a computer for the BAS system.

6 16. Does the BAS require an internet connection to maintain control? Unknown. 17. RFP specifically states that filters are to be changed quarterly. Will the DOC also require that each filter is dated and initialed to ensure compliance? DOC does not require that each filter is dated and initialed, but that is a good practice. 18. The RFP states that the DOC will place moisture absorbing material in deenergized panels and contractor is required to check quarterly and replace as needed. Will the DOC provide a list of locations where the absorbent is placed? Contractor winning award will receive information from on-site DOC manager. 19. There are three storage buildings that already have rooftops with structural failure and are flooding. Is the DOC aware of the current condition of the failing roofs? Yes. 20. Does the DOC have a plan to repair the failing roof structures on three storage buildings? Yes, DOC has projects to repair roofs.
