ADDENDUM NUMBER ONE REQUEST FOR QUALIFICATION (RFQ) Page 1 of 2 CMc CONSTRUCTION MANAGER AS CONSTRUCTOR PUBLIC WORKS & PARKS SERVICE FACILITY PROJECT TOWN OF WINDSOR, COLORADO April 5, 2016 Issued by: Town of Windsor, Colorado The following shall become as fully a part of the above-described RFQ dated March 28th, 2016 as if included and shall take full and complete precedence over anything stated or shown to the contrary. Each and every submitting qualifications shall be responsible for each and every item in this ADDENDUM NUMBER ONE to ascertain the extent and manner it affects the submittal. ACKNOWLEDGEMENT: Each qualification package shall include a signed copy of this ADDENDUM NUMBER ONE as acknowledgement. Please make the following changes and/or modifications to the documents: REVISIONS AND CLARIFICATIONS: 1. Item 3.A paragraph 1: the Town of Windsor is receiving responses to the RFQ for Manager as Constructor ONLY at this time. The successful CMc will solicit proposals for construction-phase labor, material, equipment, and subcontracts at a future date. Similarly, the Owner will solicit proposals for furniture, fixtures, equipment, and operating supplies at a future date. 2. Item 3.A paragraph 2: paragraph reads Submit three (3) printed copies (one loosely bound) of the submittal.. The intent is that at least one copy of the submittal be easy to insert and remove pages, by way of example, in a three-ring binder. The other printed copies may be solidly bound, by way of example, by a binding spine or spiral binding. All submittals may be loosely bound if preferred by firm. 3. Item 3.A.b.: correct item to read: Provide project information sheets on 3-5 projects in the last five (5) years that are most relevant to this RFQ (no more than two (2) pages per project).

At the option of the Town of Windsor, selected firms may be asked to attend interviews. The rating scale range shall be as shown, with 0 being a poor rating. RATING SCALE QUALIFICATION STANDARD 0-10 Scope of Submittal Does the submittal conform to the RFP? Is it responsive, project specific, and informative? How motivated is the firm for this project? 0-20 Project Experience Are the projects submitted relevant to this RFQ? Do they illustrate depth of competency? 0-25 Assigned Personnel 0-15 Qualification Statement 0-5 Do assigned personnel for the preconstruction phase have the necessary skills and experience for this project?
