3 before you start The BUSS(Q) online insurance application makes it easier to apply for, or top-up, your insurance inside super. Use the BUSS(Q) online insurance application for: Death Cover Total and Permanent Disablement (TPD) Cover Salary Continuance Cover (if you are a member of a Personal Plan). The application is designed to capture the most relevant information from you, as quickly as possible. It automatically adjusts to your responses to minimise the number of questions you need to answer. To help you get started, it will also pre-populate some of your information - including details of your existing BUSS(Q) insurance cover. Most people will complete the application within minutes with the outcome of your application provided at the end. This booklet walks you through the online application process. If you have any questions about using the application, please refer to the help section online. What information will you need? Before starting, please have the following information at hand: Your personal details and existing insurance information Your height and weight measurements Your personal medical details Your occupation and income details Your doctor s name and suburb. You will also need to know how much insurance cover you are applying for. Internet browser settings Please ensure you have enabled Javascripts and cookies in your internet browser options. Please ensure you have the latest version of Adobe Acrobat installed to view and save your completed application. 6. Online insurance application complete 1. 4 Starting a new application You can access the application via Simply log-in to MemberAccess and click the insurance tab to start a new application. Log into your BUSS(Q) MemberAccess account and click the insurance tab to access the online application. Click Edit to update or change your details. You will need to ensure these are correct before commencing your application. The green more/less buttons allow you to expand and collapse information. It is important you read the Duty of Disclosure before you start. Click the start new button to commence your application. 4

5 Entering your details Member details Your application will be pre-populated with the details BUSS(Q) has recorded for you. Do you need to make any changes? If yes, you will need to ensure these details are correct before completing the application. Click save and close to exit the application and return to your account to update your details. Your details will be pre-populated with information currently with BUSS(Q) If you need to update these, click save and close to exit the application. If your details are correct click Continue. 6 Personal and occupational details Here you ll be asked to tell us your height and weight, and answer some questions about your employment status. To search for your occupation click Enter details. Enter your height, weight, smoking and drinking habits. Search for your occupation by clicking Enter details. You will be taken to the Search for occupation screen. 6

7 PERSONAL AND OCCUPATIONAL DETAILS Search for your occupation This screen allows you to search and select your occupation. A list of occupations matching your description will appear. Enter your occupation and click Search. Chose the occupation that best matches yours. If your occupation cannot be found please provide details here. Click Continue. 8 Entering insurance cover details Applying for additional or new insurance cover This screen shows any existing cover you have with BUSS(Q). Here you can enter how much additional cover you want to apply for. To find out more about your cover please refer to the BUSS(Q) Product Disclosure Statement available at Don t forget to take into account any life insurance cover you have outside of BUSS(Q). Only existing insurance cover with BUSS(Q) will be prepopulated, otherwise this section will be blank. Select the requested cover and enter the amount. This summary shows a total of your existing cover, and the additional cover you re applying for. Click Continue. Only existing insurance cover within BUSS(Q) will be displayed. Any additional information outside of BUSS(Q) will need to be disclosed in the application. This will be asked for in the following sections. If you want to make changes to your TPD Cover only please call 1800 MY BUSSQ ( ). 8

9 Your Personal Statement Your Personal Statement is built by answering a number of questions about yourself. This includes medical history, family history and pastime information necessary for the assessment of your application. If you answer yes to some questions, you may be asked to provide additional information. Click your response for each question. You will be directed to additional screens if more information is required. This collects further information that may be required to assess your application. Click Continue. 10 YOUR PERSONAL STATEMENT Search for medical condition If you ve been asked to provide more details about a medical condition, you ll need to search to find the best match for your specific condition. You ll then be asked some more questions about your condition, with your answers summarised at the bottom of the page. Enter your medical conditions and click Search. Additional questions will be asked until all required information has been collected. All questions and responses will be summarised. 10

11 Entering your doctor/ medical centre details Here you provide details of your current doctor(s) or medical centre. If you have been seeing a doctor for less than 12 months, you ll be asked to provide details of your previous doctors as well. If you do not have a current doctor or medical centre, please provide details of the last doctor / medical centre you attended. Enter your doctor/ medical centre name, address and phone details. Click Continue. If you have attended this doctor/medical centre for less than 12 months, click Add to provide details of other doctor/ medical centres you have attended. 12 Application summary This is a summary of all the questions and answers you ve completed. At this point you must check all the information you ve provided is correct. You can also see what you ve done, and come back if you need to track down more information. Applications will only be saved for 30 days. All questions and your answers will be summarised. Ensure that your details are correct. To amend incorrect answers click Edit your details. Once details have been checked or reviewed click Confirm. 12

13 Application outcomes This screen shows the decision that s been made about your application. It will be one of three outcomes. BUSS(Q) will write to you to confirm your application. If your application is accepted, the letter will include the date of acceptance. Accepted Standard Terms Accepted Standard Terms means your application has been accepted without any loadings (i.e. additional premiums) or exclusions (i.e. special conditions you won t be covered for). 14 Application outcomes Accepted Amended Terms Accepted Amended Terms means your application has been accepted with loadings (i.e. additional premiums) or exclusions (i.e. special conditions you won t be covered for). These are detailed under Policy Conditions in the print version of the application and will be provided in a letter by BUSS(Q). Further Assessment Required Further Assessment Required means your application will need to be reviewed further before a decision is made. 14

15 Submitting an application Here you can see the details of the insurance you have requested. After you have read the declaration, check the box and click Submit application. Read the declaration carefully. If you agree to the terms, check the box and click Submit application. By checking the box and clicking on Submit application, you ve agreed to the declaration. 16 Printing a submitted application After you have submitted your application, you can print a hard copy for your records, or save it as a PDF onto your computer. Click Print customer copy to print or save the application. A copy of the application will open as a PDF. Ensure you have the latest version of Adobe Acrobat installed to view and save applications. 16

17 Save and close an application You can save and close an unsubmitted or incomplete application at any time, as long as you come back to it within 30 days. Click Save and close at any time to save the application. Your application will be saved for 30 days. Note: The application has not been submitted, therefore interim cover does not apply. 18 Resume a saved application You can return to a saved application via Simply log-in to MemberAccess and click the insurance tab to access your saved application. Click Resume incomplete application to continue a saved application. 18
