2 COMMUNITY RATING SYSTEM ELEVATION CERTIFICATE CHECKLIST The goal of this checklist is to assist the community with EC review. We suggest stapling this form to the front of your ECs for future reference and annual recertifications. The document provides specific detail on completing the form and diagram details to meet the CRS standards for Activity 310. This EC checklist is for: Property Address: Date: Reviewed by: Accepted, no errors Errors, corrections required Notes: SECTION A- PROPERTY INFORMATION Section A can be corrected by the community using the EC Correction Form The EC form is valid at the time of review Upper right hand side of the form lists the OMB Number and Expiration Date. Confirm the form is not expired. A2and/or A3 are completed with a valid address or property description Confirm the address and/or property description is valid in your community. A7 building diagram is complete to the type of construction Reference the FEMA EC packet for diagram descriptions. Note from the Specialist: Manufactured homes that are on a traditional pier system with skirting should be reflected as a Diagram 5. 1 A8/A9 (a-d) shall be completed as applicable to the structure If the section does not apply, N/A or should be added to the form. No field should be left blank. If A8 (d) and/or A9 (d) Engineered flood opening is marked, attached the signed and sealed engineered documents or the product evaluation certification to the EC The number of permanent flood openings, that are no higher than 1.0 foot above the higher of the exterior or interior grade or floor immediately below the opening. Reference section A the FEMA EC packet for engineered flood opening details. SECTION B- FLOOD INSURANCE RATE MAP (FIRM) INFORMATON Section B can be corrected by the community using the EC Correction Form B1 includes both the complete community name and six-digit community number as shown on the FIRM panel for the jurisdiction the structure is located. I.E.: City of Evansville or Vanderburgh County A structure located within an incorporated jurisdiction should not use the County as the community name (or the opposite). A structure located on property annexed into another jurisdiction since the panel was last updated should reflect the information for the jurisdiction since the annexation not the number or name for the community it used to be in. This allows the policy to be written as a part of the correct community. B2 has the correct County name B3 has the correct State B4 has both the correct map/panel number as shown on the FIRM For maps in a county-wide format, the sixth character of the Map Number is the letter C followed by a four-digit map number. For maps not in a county-wide format, enter the Community Panel number. The example below shows first a single community panel map which would require the full as the map/panel number and the second example is for a set of county-wide maps which require the entire 99009C0038. B5 has the correct suffix as shown on the FIRM The example above shows the suffix X for each panel. The suffix is found at the end of the map and panel number

3 B7 has the correct FIRM panel effective or revision date This is not always the same as the FIRM index dates as panels can be updated without the whole set being changed. An easy check for this is to use the FEMA Map Service Center to check a specific panel which would show the most current date for the specific panel. B8 identifies the correct flood zone as shown on the FIRM panel This is the zone affecting the structure only and can be checked by address on the FEMA Map Service Center. B9 contains the base flood elevation(s) The BFE for a property in the AE Zone should come from the Flood Insurance Study and often reflects the tenths place. B10 identifies the correct source of the base flood elevation(s) Zone AE elevations should be sourced from the FIS. A community providing the BFE from the FIRM by virtue of reading the map does not constitute Community Determined. Adequate information should be provided to locate the source when Other/Source is indicated. If other source is marked, vertical datum is still required C2 (a-h) shall have elevations provided. If not applicable, insert N/A or - All new construction shall include C2e. SECTION D SURVEYOR, ENGINEER, OR ARCHITECT CERTIFICATION Section D shall be completed, signed, and sealed by a State licensed professional. The EC lists survey, engineer, and/or architect as a qualifying professional. Check with your State office if standards are different. Example: State of Kansas does not accept EC sealed by an architect. SECTION E BUILDING ELEVATION INFORMATION (SURVEY NOT REQUIRED) FOR ZONE AO AND ZONE A (WITHOUT BFE) B11 identifies the correct datum for the BFE in B9 The datum is found on the individual map panel in the legend. Map panels in the same set could be in different datum due to updates of individual panels. B12 is correctly answered with Yes or No Non-coastal communities will always answer this No and it should not be left blank. If the answer is No then the Designation Date, CBRS and OPA do not need information designated. SECTION C BUILDING ELEVATION INFORMATION (SURVEY REQUIRED) Section C can only be corrected by the professional who sealed the document C1 identifies the stage of construction For CRS purpose only, the community is to only provide ECs for reviews that are marked as Finished Construction. C2 Verify the benchmark is valid and vertical datum is provided Most control networks will assign a unique identifier for each benchmark. For example, the National Geodetic Survey uses the Permanent Identifier (PID). For the benchmark utilized, provide the PID or other unique identifier assigned by the maintainer of the benchmark. For GPS survey, indicate the benchmark used for the base station, the Continuously Operating Reference Stations (CORS) sites used for an On-line Positioning User Service (OPUS) solution (also attach the OPUS report), or the name of the Real Time Network used. 8 Preparation of the Floodproofing Certificate for Non-Residential Buildings The Floodproofing Certificate is required for all non-residential buildings to be floodproofed and is to be completed by the design professional. The first part of the Certificate contains information concerning the location and ownership of the building. FEDERAL EMERGENCY MANAGEMENT AGENCY NATIONAL FLOOD INSURANCE PROGRAM FLOODPROOFING CERTIFICATE FOR NON-RESIDENTIAL STRUCTURES O.M.B. No The floodproofing of non-residential buildings maybe permitted as an alternative to elevating to or above the Base Flood Elevation; however, a floodproofing design certification is required. This form is to be used for that certification. Floodproofing of a residential building does not alter a cummunity s floodplain management elevation requirements or affect the insurance rating unless the community has been issued an exception by FEMA to allow floodproofed residential basements. The permitting of a floodproofed residential basement requires a separate certification specifying that the design complies with the local floodplain management ordinance. 9 Section II requests information regarding the floodproofing design. The first item is the elevation, referenced to the datum of the FIRM (generally the National Geodetic Vertical Datum of 1929), to which the building is floodproofed. This elevation must be equal to or greater than the BFE. It is important to note that for insurance rating purposes, the floodproofing design must provide protection to 1 foot above the BFE to receive rating credit. If the building is floodproofed only to the BFE, then the building s insurance rating will result in a higher premium. Before a decision is made to floodproof to less than 1 foot above the BFE, insurance implications should be carefully considered. The second item is the height of the floodproofing above the lowest adjacent grade. This information is intended to be used by community building officials, FEMA, and NFIP insurance underwriters to analyze the level of safety that the floodproofing design will provide. Since floodwaters exert greater pressure on the floodproofed building as the height of the flooding increases (see Figure 1), floodproofing that exceeds 3 feet in height represents a greater risk and may result in insurance rates that reflect this increased risk. SECTION II FLOODPROOFING INFORMATION (By a Registered Professional Engineer or Architect) Floodproofing Design Elevation Information: Building is floodproofed to an elevation of. feet NGVD. (Elevation datum used must be the same as that on the FIRM.) Height of floodproofing on the building above the lowest adjacent grade is feet. (NOTE: for insurance rating purposes, the building s floodproofed design elevation must be at least one foot above the Base Flood Elevation to receive rating credit. If the building is floodproofed only to the Base Flood Elevation, then the building s insurance rating will result in a higher premium.) Section II Section III is the actual certification of the floodproofing design as required in Section 60.3(c)(4) of the NFIP regulations. It is important to note that design professionals signing this form are certifying that they have developed and/or reviewed the design plans and specifications and find them in compliance with accepted standards of practice for dry floodproofing. This certification is based on the floodproofing design, not the as-built condition of the building. The person signing this form must be a registered professional engineer or architect within the state or territory where the building will be constructed or substantially improved. 11

All structural components are capable of resisting hydrostatic and hydrodynamic flood forces, including the effects of buoyancy, and anticipated debris impact forces. I certify that the information on this certificate represents my best effort to interpret the data available. FEMA Form Section Ill The NFIP The NFIP was created by Congress in 1968 to provide federally backed flood insurance coverage, because flood insurance was generally unavailable from private insurance companies. The NFIP is also intended to reduce future flood losses by identifying floodprone areas and ensuring that new development in these areas is adequately protected from flood damage. The NFIP is based on an agreement between the federal government and participating communities that have been identified as being floodprone. FEMA, through the Federal Insurance Administration (FIA), makes flood insurance available to the residents of a participating community provided that the community adopts and enforces adequate floodplain management regulations that meet the minimum NFIP requirements. The NFIP encourages communities to adopt floodplain management ordinances that exceed the minimum NFIP criteria.
