3 Lion Hills Pool/Tennis Membership Enrollment Form & Contract All Pool Memberships are for ONE Pool Season beginning the Friday before Memorial Day and ending the day after Labor Day. Tennis Courts are available for one full year from Effective. Pool Membership fees are due and payable prior to the first date of use for the season. (A monthly payment option is NOT available for Pool /Tennis Only Memberships.) 4 PAYMENT OF DUES: Monthly membership dues are collected within the first five (5) days of each month by ACH agreement on file. If the ACH payment does not have sufficient funds to make the full payment, the payment process will next go to the credit card on file below. If the ACH Payment and the Credit Card payment are not able to process due to insufficient funds or declined, Lion Hills will attempt to notify the member. The member will then have three (3) business days after notification to make alternate arrangements for the payment. If the member has not arranged payment within three (3) business days of notification, the membership will be suspended and the member will pay the amount past due plus a $25.00 service fee to reactivate the membership. Memberships remain in suspension until all past due amounts are paid in full. 5 No swimming is allowed by anyone unless Lion Hills Certified Lifeguards are on duty! 1. Obey the lifeguard. 2. All swimmers must sign in and out. 3. Swimsuits only; no cut off jeans allowed. 4. No running in the pool area. 5. No diving. Ever. 6. No glass or ceramic items allowed in the pool area. All beverages must be in plastic or aluminum containers. Only alcoholic beverages, which are not sold at EMCC, may be brought in. No other beverages or food of any type may be brought in to the facility. 7. No fighting, horseplay or excessive noise permitted in the pool area. 8. Smoking is permitted in designated areas only. 9. Chewing gum is not permitted while swimming. 10. Only lifeguards are allowed on lifeguard stands. 11. No loitering or talking to lifeguards on duty. 12. Children under 12 years old are not allowed in the pool area unless accompanied and supervised by someone who is at least 16 years of age. 13. Diapers are not permitted in the pool. Please use approved swim diapers. 14. Children who cannot swim must be accompanied by an adult at all times. 15. Parents are responsible for the actions of their children under the age of Dogs and other animals are not allowed on the premises. 17. Anyone with open blisters, cuts, sores, etc. is advised not to use the pool. 18. Spitting, spouting water, blowing the nose or discharge of any bodily waste in the pool is strictly prohibited. 19. The first violation of pool rules or lifeguard instructions will result in a warning. A second violation will result in removal from the pool area. A third violation will result in suspension from the pool. 20. If questions arise, obey the lifeguard. Lion Hills Center Pool Rules The following are rules for Lion Hills Pool Members. These rules must be followed in order for pool membership to continue. Guests: A member may bring no more than two (2) guests to the pool at any time during regular pool hours. The cost for each guest is $10 per visit. The member is responsible for the cost of each guest if the guest does not pay. The member is responsible for the cost of any food purchased by their guest if the guest does not pay. And, the member is responsible for seeing that their guest follows all pool rules. EMCC has the right to ask any patron to leave the premises for any behavior EMCC deems unbecoming or detrimental to other patrons or to EMCC. (Please keep for your records.) 4
