Important Note/Nota Penting (i) The acceptance of this form does not impose any obligation on the Bank to issue UOB ClubPlus Club Card(s) to me/us. 5 Product Disclosure Sheet ClubPlus Club Card PRODUCT DISCLOSURE SHEET (Read this Product Disclosure Sheet before you decide to take up the UOB ClubPlus Club Card. Be sure to also read the general terms and conditions.) United Overseas Bank (Malaysia) Bhd. ( Bank ) ClubPlus Club Card (PLC) As at 15 August What is this product about? This is a special Card programme that facilitates the participating club in managing the issuance of the club membership cards, the billings to the members of the club and the receivables of club members. The Bank, on behalf of the participating club, will make ClubPlus Club Card (PLC) available to all club members. PLC serves as a membership card as well as a Club Charge Card, which allows club members to charge his/her spending/billing in the club on his/her card account. 2. What do I get from this product? 1. Credit Limit Upon the confirmation of the participating club of your eligibility to hold the PLC, a line of credit will be granted to you by the bank. You will enjoy 20 days interest free period from the statement date ( the Payment Due Date ). 2. Finance Charges PLC Cardmembers who do not settle the Total Balance Due (Full Payment), a finance charge of 18% per annum applies. Finance Charge will be applied to Total Balance Due on Retail Transactions, calculated on a daily rest basis from the day the transaction is posted to the card account (if the Total Balance Due in the Statement of Account of the previous month is not fully paid by the then Payment Due Date) or the date next following the Payment Due Date (if all outstanding balances in the Statement of Account of the previous month are fully paid by the then Payment Due Date) until repayment is made in full. 3. What are my obligations? Simply make repayment on transactions charged/billed by or before the Payment Due Date. PLC Cardmembers has to make full payment of the Total Balance Due, Finance Charges for retail transactions will be imposed and calculated from the day the transactions are posted to the card account. 4. What are the fees and charges I have to pay? 1. There is no Annual Fee imposed for PLC Cardmembers. 2. Card Replacement Fee per PLC replacement. 3. Additional Statement Request Fee 5.30** for walk-in or facsimile request and 6.36** for mail request. 4. Other fees and charges: Refund of credit balance 2.12** for Commission and 0.15** for Stamp Duty. **Inclusive of GST 5. What if I fail to fulfill my obligations? Late payment penalty charges 1% of the total outstanding balance as at statement date or a minimum of 5, whichever is higher. Right to set-off The Bank has the right to set-off any credit balance in any one of your accounts maintained with the Bank against any outstanding balance in the card account. 6. What are the major risks? The PLC operates as a Club Charge Card. You are therefore required to settle the full amount by the due date. If you do not settle the full amount by the payment due date, you will incur 18% p.a. finance charge on the outstanding balance. 7. What do I need to do if there are changes to my contact details? Ini adalah satu program kad yang istimewa yang memudahkan kelab yang mengambil bahagian dalam menguruskan pengeluaran kad keahlian kelab, bil kepada ahli-ahli kelab dan bayaran yang belum diterima daripada ahli-ahli kelab. Bank, bagi pihak kelab yang mengambil bahagian, akan menyediakan Kad Kelab ClubPlus (PLC) kepada semua ahli kad kelab. PLC berfungsi sebagai kad keahlian serta kad caj, yang membenarkan ahli kelab untuk membuat bayaran bagi segala perbelanjaan di kelab dengan menggunakan akaun kadnya. 2. Apakah yang boleh saya perolehi daripada produk ini? 1. Had Kredit Sebaik mendapat pengesahan daripada kelab yang mengambil bahagian berkaitan kelayakan anda sebagai pemegang PLC, had kredit akan diperuntukkan oleh pihak bank. Anda akan menikmati 20 hari tempoh tanpa faedah dari tarikh penyata ( Tarikh Akhir Bayaran ). 2. Caj Kewangan Ahli Kad PLC yang tidak menjelaskan Jumlah Baki Yang Perlu Dibayar (Bayaran Penuh), caj kewangan 18% setahun akan dikenakan. Caj Kewangan akan dikenakan ke atas Jumlah Bayaran Perlu Dibayar untuk Transaksi Runcit, dikira berlandaskan dasar kiraan atas baki harian bermula dari tarikh transaksi diposkan ke akaun kad (jika semua Jumlah Bayaran Perlu Dibayar dalam Penyata Akaun bulan lepas tidak dijelaskan sepenuhnya pada Tarikh Akhir Pembayaran berkenaan) atau tarikh seterusnya selepas Tarikh Akhir Pembayaran (jika semua baki tertunggak dalam Penyata Akaun bulan lepas dijelaskan sepenuhnya pada Tarikh Akhir Pembayaran berkenaan) sehingga pembayaran balik sepenuhnya dibuat. 3. Apakah tanggungjawab saya? Membuat pembayaran balik untuk transaksi yang dicaj/dibil pada atau sebelum tarikh pembayaran. Ahli Kad PLC mestilah membuat bayaran penuh untuk Jumlah Bayaran Perlu Dibayar, Caj Kewangan untuk Transaksi Runcit akan dikenakan bermula dari tarikh transaksi diposkan ke akaun Kad Kredit. 4. Apakah fi dan caj yang perlu saya bayar? 1. Tiada Fi Tahunan dikenakan untuk Ahli Kad PLC. 2. Fi Penggantian Kad untuk setiap penggantian PLC. 3. Fi permintaan penyata tambahan 5.30** untuk permintaan jumpa terus atau faksimili dan 6.36** untuk permintaan melalui surat. 4. Fi dan caj lain: Pulangan Baki Kredit 2.12** untuk komisen dan 0.15** untuk Cukai Setem. **Termasuk GST 5. Jika saya gagal untuk menunaikan tanggungjawab saya Caj penalti pembayaran lewat 1% daripada jumlah keseluruhan baki tertunggak pada tarikh penyata atau minimum 5, mana yang lebih tinggi.
