Eligible emplyees are defined as permanent full time r part time emplyees wrking 20 r mre hurs per week, and eligible retirees are defined as retirees under age 65 with at least 10 plus years f creditable service and participatin in the City s healthcare plan during the last five years prir t retirement. Yu may have heard news stries abut the rising cst f healthcare natinwide. The City f Richmnd has experienced substantial increases in ur healthcare csts. Hwever, we are very prud t infrm yu that yur cntributins fr healthcare benefits will nt increase this year as the City f Richmnd will absrb the 6.7 millin dllar premium increase that we experienced this year, while cntinuing t ffer healthcare benefits that are cmpetitive when cmpared t ther lcal municipalities. It is estimated that this actin will result in an average savings t all participating emplyees and retirees f apprximately $500 fr the upcming plan year. The City f Richmnd carefully weighed all alternatives and had t make difficult decisins. A determinatin was made t cntinue t ffer health insurance cverage thrugh CIGNA Healthcare. In an effrt t ffer quality healthcare t all emplyees and retirees, the City f Richmnd streamlined yur benefits ptins int tw new plans. A brief descriptin f the new healthcare plans available are summarized n the next page.
3 Plan A -PREMIER PLAN- Open Access Plus 20/40 HEALTHCARE BENEFIT PLANS The pen access plus 20/40 has in-netwrk and ut-f-netwrk benefits. The in-netwrk plan allws yu t use netwrk physicians; hspitals, pharmacies and ther netwrk prviders at n charge after the c-pay and deductible are met. The in-netwrk primary care physician c-pay is $20.00, and the specialist c-pay is $ If yu need t see a specialist, n referral is required when yu select a dctr in the CIGNA Open Access Plus netwrk. The ut-f-netwrk benefits allw yu t chse a nn-participating physician. This plan prvides lwer c-pays at the time f service with higher bi-weekly premiums. Plan B- CLASSIC PLAN Open Access Plus 25/50 The pen access plus 25/50 has in-netwrk and ut-f-netwrk benefits. The in-netwrk plan allws yu t use netwrk physicians, hspitals, pharmacies and ther netwrk prviders at n-charge after the c-pay and deductibles are met. Brad Street, Rm 902 by 5:00pm n Friday, June 17, 2011: Enrll in a healthcare plan electin as a new participant. Terminate yur healthcare plan cverage under the city prgram. Make a change between yur rllver healthcare plan electin and the ther available healthcare plan ptin. Add r remve dependents frm yur cverage. Be sure t prvide dcumentatin that certifies the dependents yu adding are eligible. Required dcumentatin includes phtcpies f the marriage, birth certificate, adptin decree (r prf f placement fr adptin) r curt rder dcumenting legal guardianship r legal custdy. These dcuments must be submitted at ne f the Open Enrllment sessins with yur cmpleted applicatin, r submitted t the Human Resurces Department by 5:00 pm n Friday June 17, 2011, r yur dependents will nt be added t yur cverage. ENROLLMENT PROCEDURES & DEPENDENT ELIGIBILITY VERIFICATION Enrllment prcedures and frm cmpletin If yu want t make a change t yur rllver healthcare plan electin yu must cmplete a CIGNA enrllment frm as fllws: 1. Sectin A: check pen enrll bx and effective date 7/1/2011 (if making a change), check new enrll and effective date 7/1/2011 if enrlling fr the first time under the City s plan, CIGNA accunt n , Branch Cde- active, retired, cbra (A,R,C), type f change- Medical Ben. Optin- enter A fr Plan A (Premier Plan) r enter B fr Plan (Classic Plan), add dependent and date f 7/1/2011, cancel emplyee and last date f cverage 6/30/2011, and/r cancel dependent(s) and last date f cverage 6/30/ Sectin B: all f the required infrmatin (d nt fill in emplyee ID number) 3. Sectin C: check the bx marked Open Access Plus 4. Sectin D: Skip 5. Sectin E: Skip 6. Sectin F: cmplete if it applies 7. Sectin G: emplyee signature required, but spuse is nt required A sample frm has been enclsed with this letter that identifies the sectins that must be cmpleted. 4
6 DEPENDENT ELIGIBILITY VERIFICATION This year s healthcare pen enrllment prcess will als include a dependent eligibility verificatin prcess fr emplyees and retirees wh select cverage fr dependents (emplyee + ne and emplyee + family). The dependent eligibility verificatin prcess ensures that cverage is nly extended t thse wh are eligible fr plan benefits. The City requests yur cperatin in this imprtant effrt. If yu select cverage t include a new dependent nt currently cvered (i.e. emplyee +ne r emplyee + family) yu must prvide dcumentatin f each dependent s status by prviding the scial security number f each dependent, and prf f eligibility fr any dependent cvered under the healthcare plan as fllws: Spuse- legal spuse f the ppsite sex Scial security number, and Phtcpy f marriage certificate r phtcpy f the tp prtin f the first page f the emplyee s mst recent Federal Tax Return that shws the dependent listed as Spuse. Nte all financial infrmatin and Scial Security Numbers shuld be redacted r marked ut. Child- natural, adpted, stepchild, legal guardian (unmarried child up t age 26) Scial security number, and Phtcpy f birth certificate (r adptin agreement) shwing emplyee s name r shwing the name f the emplyee s spuse, and Phtcpy f marriage certificate shwing the emplyee and parent s name if stepchild, r Phtcpy f the tp prtin f the first page f the emplyee s mst recent Federal Tax Return that shws the dependents parent listed as Spuse. Nte all financial infrmatin and Scial Security Numbers shuld be redacted r marked ut. Phtcpy f the Final Curt Order granting permanent custdy with presiding judge s signature, if legal guardian The required dcumentatin fr verificatin f dependent eligibility must be submitted alng with the enrllment frm fr all newly added dependents, and must be a cpy. N riginal dcuments will be accepted. Yu d nt need t submit dependent verificatin dcumentatin fr dependents already cvered under yur CIGNA healthcare plan. ENROLLMENT FORM SUBMISSION REQUIREMENTS Yu are nly required t submit a CIGNA enrllment frm if yu wish t make changes t enrll as a new participant, terminate cverage, add a dependent, r change yur autmatic rllver electin. This CIGNA enrllment frm must be submitted by Friday, June 17, 2011 at 5:00pm. Enrllment frms may be submitted t the CIGNA representatives at ne f the pen enrllment sessins, submitted by mail, r hand-delivered t the Department f Human Resurces at 900 E. Brad Street (City Hall) 9 th flr Richmnd, VA DO NOT MAIL ENROLLMENT FORMS TO CIGNA. Any benefit electins, effective July 1, 2011, will be unchangeable during the next fiscal plan year unless yu experience a qualifying event during the plan year that allws yu t make a change. 5
