1 June 1st, 2016 issue (No.1) Current State Notification for Child Allowance Please never fail to submit Current State Notification for Child Allowance. Those who have received Child Allowance are required to submit a current state notification every June. If you fail to submit it, you will not be entitled to receive the allowance for the month of June and later months. As the notification form will be sent to subject people in early June, please fill out the necessary items in the form and send it back by the returnmail envelope enclosed with necessary stamps put. The notification form can be accepted at Childrearing Support Section. Necessary Items Current State Notification Copy of recipient s health insurance card (Unnecessary for a member of the National Pension System.) *In case of movingin after January 2 nd, 2016, the income certificate for Child Allowance in 2016 issued by the local government you resided in as of January 1 st, 2016 is necessary. *In case the obtained income certificate does not include spousal deduction, your spouse s income certificate is also necessary. *The recipient who lives apart from the subject child/children is required to submit other necessary documents. Deadline Must be received by Thu, June 30 th, 2016 Issuance of Certificate of Municipal and Prefectural Tax for 2016 The certificate of Municipal and Prefectural Tax for 2016 calculated based on the income and the deduction in 2015 will be issued from Mon, June 6 th. Necessary Items Document to identify yourself such as a valid passport, a valid driver s license card. In case of not having above items, more than one item below are necessary. Health insurance card, Pension booklet, Student ID card, Company staff ID card, Cash card with your name *Applicants are required to present the above ID documents at Municipal Taxation Section. *If any person other than you apply, the letter of proxy is necessary. Notice of National Health Insurance Tax Payment Notice of National Health Insurance Tax Payment for 2016 will be sent in the middle of June. To the person who made a payment by deduction from your pension in 2015, it will be sent in the middle of July. Even if the householder is not a member of National Health Insurance, this notice will be sent to the householder when the household has at least one family member who is a member of this insurance. In that case, the tax of the householder is not included. *The person aged 75 and above who is a member of Older Senior Citizens Medical System is required to pay the premium of the system. The payment notice of the premium will be sent in July. For further details, please check the city magazine Onojo or call us. Please receive the vaccination of Measles and Rubella (MR) When your baby becomes one year old, please make him/her receive the vaccination of Measles and Rubella(MR)as soon as possible. Also, please make sure to have him/her receive the second vaccination before elementary school enrollment. Please contact your family doctor regarding the vaccination. Appointment in advance is necessary. In case of failing to receive the vaccination within the subject age, the payment is not covered by the government. Please pay attention to the routine vaccination age and period.
