9 P rogram rogram Criteria QUALIFICATION CRITERIA FIVE-STAR PLUS membership is earned upon reaching 300,000 Five-Star Annuity Credits and a minimum of 15 paid policies. FIVE-STAR PREMIER membership is earned upon reaching 700,000 Five-Star Annuity Credits and a minimum of 25 paid policies. An agent must be active and in good standing, and have a rolling 12-month placement ratio of 90% or higher to qualify for and fulfill PLUS and PREMIER membership benefits. FIVE-STAR ANNUITY CREDITS FIVE-STAR PLUS and FIVE-STAR PREMIER qualification is based on combined flexible and single annuity premiums received January 1 - December 31, See details in the chart (right). Renewal and annualized premium are not included. The writing agent on the original application will be the agent of record for this program. No Five-Star Annuity Credits will be given on conversions, internal transfers or business written on a member of the agency force or their immediate families. Five-Star Annuity Credits are calculated as follows: Credits apply to all fixed annuity business written for products issued by Annuity Investors Life and Great American Life : 100% x First-Year Flexible Paid Premium + 15% x Single Paid Premium = Total Five-Star Annuity Credits For example, if you produce $300,000 of First-Year Flexible Paid Premium and $200,000 of Single Paid Premium, your total Five-Star Annuity Credits would be 330,000. ELIGIBILITY To exercise your Five-Star Rewards membership (which includes booking and fulfilling your travel arrangements), you must be an active writing agent with a Retirement Group-designated agency in good standing with a Great American Financial Resources, Inc. subsidiary. Agents must receive an official written invitation to participate in the FIVE-STAR PLUS and FIVE-STAR PREMIER membership levels. Eligibility for these levels will be determined after the end of each month. TRAVEL Travel benefits are valid for 12 months from your date of notification. No exceptions will be made with regard to travel expiration dates.the Company will not pay cash compensation in lieu of the travel award.
