4 MEDICAL FORMS Students must have completed medical forms SIGNED BY a PHYSICIAN turned in by March 8, 2019 for BOTH self-medication and staff administered medication. -PLEASE get those forms done ASAP. -Doctors can fax the forms to BMS # FORM A Permission for all medication that faculty will be administering FORM B For any OTC medication or ANY medication WITHOUT a label FORM C For self-administering (ex: asthma, diabetes, etc )

5 Itinerary DAY 1 6:00-7:00 am EST Depart Beaumont Middle School for Charleston Lunch and Dinner en route 7:00 pm Ghost Tour in Historic Charleston 9:30 pm Check into Hotel with security on duty all night Holiday Inn Express and Suites Charleston Airport and Convention Center 3025 W Montague Avenue North Charleston, SC

6 Morning Day 2 Barrier Island Eco Tour Explore the tidal creeks of the salt marsh to experience and learn about marine life, sediments, barrier island formation, sand dune function, loggerhead sea turtles, erosion and deposition. Beach Cookout for Lunch (included) Tour Patirots Point Home of the USS YORKTOWN is South Carolina's most unique and memorable attraction. The maritime museum includes four ships, including the USS Yorktown aircraft carrier, which played an important role in World War II. Lunch Navy Pier ($10 given to each person)

7 Day 2 Continued Historic City Market This area is often called the Slave Market because it's where house slaves once shopped for produce and fish. Today stalls are lined with restaurants and shops selling children's toys, leather goods, and regional souvenirs. Local "basket ladies" weave and sell sweetgrass, pine-straw, and palmetto-leaf baskets a craft passed down through generations from their West African ancestors. Dinner Cruise for a night of dinner/dancing Cruise along on a cruise ship designed for fun and comfort. So come aboard for a few hours of food, dancing with a live DJ and coastal views.

8 Day 3 Morning Carriage Ride Experience blocks of Charleston's historic downtown district where you ll see houses, gardens, mansions, churches, and parks, and learn about buildings, history, architecture, flora and the people that make up the "Holy City." Plantation Tour Charleston is known for it historical attractions and one of the best is the plantations that have been beautifully preserved over the years. Depart for Lexington Arrive in Lexington ~ 11:30 pm

9 Behavior policy *Students are representing Beaumont Middle School and excellent behavior is expected. *In order to attend the Charleston trip, students must have: -NO suspensions from school from October 11 forward -NO more than 1 assignment to the SAFE once a deposit is made. *All students attending the trip must have an employability score of at least 70% on eos by 4:00 p.m., Friday, February 1 st and must remain at or above 70% until the trip in order to attend. *Any students who doesn t meet the above requirements will be removed from the trip. NO refund will be given if this occurs after Dec. 19 th. Before Dec. 19 th, total refund minus $120 will be returned.

10 Money/Payments & Refunds Payment schedule *Regular payments are required in order to hold your spot on the trip! **MAKE CHECKS payable to BMS and PLEASE put your child s name on the memo line. Cash is also accepted. Refund Policy *Total refund minus $120 of the deposit if WRITTEN NOTICE of your need to cancel is given on or before December 19 th. *0% will be refunded if after December 19 th. If your student wants to purchase souvenirs he/she may need spending money. Deposit due by $ October 30 th Parent meeting on October 11 6:00 in the cafeteria 2 nd payment by January 11th Final payment by February 5 th! Deposits/Future or early Payments accepted at this meeting $ $200.00

11 Insurance All students must have health insurance in order to go on the trip. Please send in proof of insurance with your deposit form. If you have health insurance, PLEASE be prepared to send in up-to-date copies before the trip (many of us get new cards in January). If a student needs insurance, they can purchase student insurance through

12 Roommates Students will stay 4 to a room. Students will pick their roommates later in this semester. There is not to be any students of the opposite gender in your room at any time. Doors will be taped each night and security will be on each floor that we have students on each night.
