1 REQUEST FOR PROPOSAL Concrete Work at Grand Ravines County Park July 13, 2018 The Ottawa County Parks and Recreation Commission (OCPRC) is seeking proposals for construction of a concrete stairs and pavement along with other miscellaneous items of work at Grand Ravines County Park in Georgetown Township. The work involves excavation, installation of a 52 long concrete stairway, installation of 160 s.f. of concrete pavement over an existing drainage pipe, rip- rap, restoration, and other miscellaneous items of work. The owner realizes that contractors are heavily scheduled at this time. Therefore, the work may be scheduled at the convenience of the contractor, however, once begun, work shall continue in an expeditious manner until the project is completed. Proposals are due at the OCPRC office by 5:00 pm, Tuesday, July 24, Faxed or E- mailed proposals are acceptable. The County reserves the right to accept or reject the bids from any company and to negotiate with any bidder deemed to be in the best interest of the County. 2 BID PROPOSAL FORM Concrete Work at Grand Ravines County Park 1. Contractor agrees to construct items as detailed on the attached documents. It shall be the responsibility of the Contractor to employ and assign to the project adequate personnel and equipment required to undertake and complete the work in a diligent, timely and orderly manner. 2. Contractor shall be responsible for all the Contractor's expenses incurred while performing services under this agreement. This includes license fees, fuel and fleet maintenance, insurance premiums, telephone and all salary/payroll expenses, and other compensation paid to employees or contract personnel that the Contractor hires to complete the work under this Contract. 3. The Contractor shall at all times observe and comply with all federal, state, local and County facility laws, ordinances, rules and regulations that may in any manner affect the safety and the conduct of the work. The Contractor shall indemnify and hold the County harmless against any claim or liability arising from the violation of any such provisions. 4. TOTAL BASE BID: The undersigned bidder hereby states that he will provide all necessary materials and labor called for by the plans and specifications and will accept as full and complete payment therefore the Lump Sum Base Bid Amount which is the summation of the cost of the items of work and is equal to the summation of the extension of the unit prices in the amount of: TOTAL BASE BID: $ Dollars And Cents. 5. The following unit prices will be utilized to assess bids, revise contract amounts, and develop change orders. Quantities indicated are estimates only and not guaranteed. Each Page 2 of 3
