Name of work : Operation and general maintenance of electromechanical services for THSTI at 496, 470 & 450 Udyog Vihar, Phase-III, Gurgaon 02. Estimated cost : Rs. 50,17, Earnest money : Rs. 1,00, Last date of sale of tender : Last date of receipt of tender: up to 3.00 PM (Both technical and price bid) 06. Date of opening of tender : at 3.30 PM (Technical bid only) 07. Time of completion of job : 12 Months 08. Cost of tender : Rs (Non-refundable) 09. Pre-qualification criteria:- a) The contractor should have yearly turnover of not less than 100 lakhs (every year) for the last three years. The contractor should attach audited Account/ITCC Certificate for the last three years duly certified by Chartered Accountant. b) The contractor is required to produce and attach the solvency certificate of Rs lakh from the Scheduled Bank in original duly attested by chartered accountant or senior branch manager of the scheduled bank, addressed to the undersigned. c) The contractor shall have minimum of 05 years experience in operation/ maintenance/ repair/ overhauling/ servicing of electromechanical services i.e. Diesel Generator Sets, Air-conditioning system and equipment with screw compressors, internal electrical works, H.T. panel, L.T panel etc. in support of which a list shall be submitted along with the tender failing which the tender will not be accepted.

5 P a g e 5 d) The contractor should have a valid electrical license for the entire duration of contract. e) The contractor should be registered in Class I/ II in CPWD/ PWD/ MES/ Railways/Autonomous Body(S) or in any other Govt. Department. f) The contractor should be authorized/carry authorization letter from OEM for maintaining air conditioning package unit & DG Sets. g) The contractor should have completed three similar nature of works in CPWD, PWD, MES, Railways, Autonomous Body or in any other Govt. Department of minimum value of 30 lakhs in each financial year for last three years [Similar nature of work means operation and maintenance of electro-mechanical services, such as Operation & maintenance of air-conditioning system of more than 25 TR capacities, D.G. sets 500 KVA/ 320 KVA or more capacities each or any Other electrical maintenance work.]. Attested copies of the Completion Certificates issued by the Executive Engineer or above are required to be enclosed with the technical bid. The contractor should also give complete details of the concerned engineer such as name with designation, valid address, and telephone/mobile number with STD Code, etc. Contractors must submit definite proof from the appropriate authority, of having satisfactorily completed any three similar works, during the last three years which shall be to the satisfaction of the Institute. The completed works will be open to inspection (inspection will be done at the risk and cost of the contractor) and in case feedback is not up to the standards, the tender will be summarily rejected & no queries will be entertained in this regard. h) No subletting or sub-contracting of the work will be permitted without the express consent of THSTI authorities. i) The tenderers should have competent personnel stationed in the office/service center in Delhi/ NCR. The service center in Delhi/NCR must be also stocking spares for providing prompt services during the contract period. A proof to this effect must be furnished. j) Conditions of Pre-qualification without which bid will NOT be accepted under any circumstances are:- 1- EMD & tender cost as mentioned. 2- PAN/TAN certificate copy duly certified by chartered accountants. 3- Current Solvency certificate as mentioned. 4- Certified copies of proof of Registration with PF, ESI authorities. 5- Valid Electrical license for the electricians proposed to deploy by you at our site with their names. 6- Proof of being in similar business for last 5 years.

6 P a g e 6 7- Completion certificates of at least 3 similar orders executed for organizations as mentioned in clause 9 G in last 3 years. 8- Proof of office / service center in Delhi/NCR as mentioned. 9- Signed copy of entire NIT documents along with blank price schedule as a token of acceptance of all out terms & conditions as mentioned in NIT. 10- List of T & P proposed to be mobilized at site. Copies of all documents of pre-qualification criteria and as asked for in the tender may please be attached with the (Part-I) Technical Bid ONLY. On short fall of any documents/demand draft (EMD)/ Tender Cost, etc., the tender will be summarily rejected and no correspondence will be entertained in this regard. Decision of the Competent Authority shall be final in this regards. All documents pertaining to the requirement of the prequalification criterion will be furnished in the (part-i) technical bid envelope ONLY. Engineer IEE

7 P a g e 7 The envelops should be super subscribed clearly with Name of work and TECHNICAL BID / PRICE BID, providing name(s) and postal address of the Contractor with Phone/ Mobile/ Fax numbers and address if any. The offer shall remain open for 60 days without any change in the rates etc. Earnest Money Deposit (EMD) amounting to Rest. 1,00,352 /- shall be accepted only in the form of Demand Draft drawn in favor of the Translational Health Science and Technology Institute payable at Gurgaon and submitted along with Part-I, Technical Bid without which the tender will summarily be rejected. In case the tender documents are down loaded from the web site the above mentioned cost of the tender will have to be paid through a separate Demand-Draft along with the tender (In the envelop of Part-I, i.e. Technical Bid), failing which the tender will be summarily rejected. 8 P a g e 8 GENERAL TERMS AND CONDITIONS Name of Work: Operation and General Maintenance of Electro-Mechanical Services at THSTI at 496, 470 & 450, Udyog Vihar, Phase-III, Gurgaon. 1. The contractors may inspect the D.G. sets, air-conditioning system and equipment s at THSTI, 496,470 & 450 Udyog Vihar, Phase-III, and Gurgaon before submitting their tender, but after prior appointment with Engineer-in-charge. 2. Tenderers are required to submitted their bids in two envelops separately ( as applicable in 2 bid system) :- I. Technical Bid II. Price Bid 3. (a) Last date of sale of tender : (b) Last date of receipt of tender: up to 3.00 PM (Both technical and price bid) 4. Date of opening of the Price Bid, will be intimated separately to the prequalified tenderers after evaluating the technical bids. No interim queries will be entertained whatsoever in this regard. 5. Earnest Money Deposit (EMD) amounting to Rs. 1,00, in the form of Demand Draft drawn in favor of Translational Health Science and Technology Institute payable at Gurgaon shall be submitted along with the Technical Bid only, without which the tender will be summarily rejected. 6. Duly sealed tenders shall be dropped in the Tender Box placed at the Reception Counter of the Translational Health Science and Technology Institute with its office at 496, Udyog Vihar Phase-III, Gurgaon. Technical Bid must contain one Xerox copy of the unfilled bid document duly signed and stamped on each page as a token of acceptance of all terms and conditions/ clauses of the tender enquiry. 7. The tenderers should have competent personnel stationed in office/ service center in Delhi/ NCR. The service center in Delhi/NCR must also be stocking spares for airconditioning system, D.G sets and other electrical accessories etc., as has been installed at the said premises of THSTI i.e. 496, 470 & 450 Udyog Vihar, Phase-III, Gurgaon, for providing prompt services during the contract period. 8. The Competent Authority, THSTI reserves the right to cancel any or all the tenders without assigning any reason. 9. All bids shall be the property of THSTI, and bidders will have no right to claim whatsoever on the same.

9 P a g e The Contractor shall be responsible for any damage to the equipment(s) caused due to faulty operation/ poor servicing/ mishandling etc. by their staff and shall rectify the defects free of cost. Decision of the THSTI authority shall be final in this matter. 11. The Contractor shall submit a list of staff that will be employed by them in event of awards of works showing their full name, local and permanent address, age, Father s name and Police station and shall be fully responsible for the safety and security of their staff. No compensation shall be payable by THSTI in case of any accident/ death of any of them while operating/ servicing the plants and equipment s and on account of any other reasons happening in or outside the THSTI. 12. The THSTI authority shall be fully empowered to depute any staff at any work within THSTI premises as per the site requirement or in-case of any emergency/ urgent work related to electro-mechanical services and additional charges will not be paid by THSTI in such cases. 13. No escalation in any form either of material (spares) consumable or labor shall be payable by THSTI during the tenure of this contract. In event of changes in minimum wages notified by ministry of labor, your contract prize will deemed to cover such a hike and it will be covered under your contract value. In this regard the Institute will not be liable for any additional payment. 14. The Air-conditioning unit, DG Set and electrical equipment covered under Annexure- I, II, III for THSTI are to be operated on 24 hrs. basis throughout the year and are meant for maintaining the temperature and humidity conditions as( 24 ᴼC ± 1 ᴼC) and 50% ± 5 % R.H. 15. (a) The Contractor shall ensure that the breakdown call of normal nature is attended within 24 hrs. Breakdown due to reason beyond control shall however be attended/ rectified within reasonable time to be decided in consultation with Engineer-in-charge of the THSTI. (b) Periodical and day-to-day maintenance checks shall be carried out and documented in prescribed check list and submitted to the Engineer-in-charge periodically. (c) For absence of any minimal staff as mentioned in clause 2 of Annexure-V penal deductions will be made from your running bills. The details of which are as mentioned below:- i. Supervisor / Foreman : Rs, 600/- per day ii. A.C & R Operator cum Mechanic : Rs. 500/- per day iii. Electrician cum D.G. set Operator : Rs. 500/- per day iv. Helper : Rs. 300/- per day

10 P a g e The number of operation and maintenance staff shall always be as per clause 2 of the Annexure V. Every staff shall be allowed one rest day/ weekly off, after every six days of working as per Labor rules. For this purpose suitable relievers shall be arranged by the Contractor at no extra cost. 17. For undertaking works like cleaning of cooling coil, de-scaling of condensers, servicing of split air-conditioning units, servicing of precision/ package airconditioning units and for proper inspection/ maintenance, if any extra manpower is required, it shall be arranged by the Contractor without any extra charges. 18. For inspection of ACB s, relays, control wiring, checking of alignment of machine/ motor etc., specialized manpower, if required, shall be arranged by the contractor without any extra charge. 19. Since the contract is for operation on 24 hrs. Basis, physical custody of all the plants and equipment s covered under this contract, shall vest with the contractor. The equipment / plant will be handed over to the contractor on as is where is basis and will be taken back by THSTI in healthy condition after expiry of contract period. In event of plant and equipment being found in unhealthy condition, THSTI will make good the same at risk and cost to the contractor and suitable recovery from running bills / SD / any other means will be made by THSTI. Decision of the Engineer In charge in this regard will be binding on the contractor. 20. No T & P shall be supplied by the Institute. List of useful T &P owned by the contractor shall be submitted along with the technical bid. 21. Water and Electricity for cleaning, testing, operation and maintenance shall be supplied by the Institute free of cost. 22. All spare parts including Refrigerant, fuel, lubricant oil, filter, lights, switch & sockets, MCCB, MCB, wires, cables, PVC pipe, channel light fitting, transformer oil, compressor oil, de-scaling chemical etc., shall be supplied by the Institute free of cost which are not covered in service job. Dismantled material against each spare part supplied, shall be handed over to the Institute by the contractor. However, workers required for the installation of all material will be provided by the contractor at his cost. 23. All consumables going permanently into the installation will be supplied by THSTI and all other working consumables necessary for upkeep of equipment will be to your account. 24. The Institute shall have the option to extend the terms of this contract for an additional period as mutually agreed upon, on identical terms and conditions.

11 P a g e TERMS OF PAYMENT:- 1. No advance payment of any kind shall be made 2. The contractor shall ensure the payment to the staff deployed/ engaged for this contract by 7 th of every month and raise monthly bill along with the proof of payment. The institute shall release the payment within 10 days from the date of receipt of bill. 3. Security Deposit shall be from each bill of the contractor and shall be released after expiry, of the contract. 4. Income tax shall be deducted from the bill as per Govt. of India rules. PAN/TIN number(s) may be provided along with copies thereof. 5. The rates shall be inclusive of all WCT, Excise Duty, VAT, Service Tax, Duties, Octroi, Packing, Forwarding, Cartage, Insurance, Loading-unloading, road permit/ state entry permit and Delivery, Installation, Testing, Commissioning, etc. at site Risks, Overhead Charges, General Liabilities/ Obligations etc. and any other taxes as applicable. Any variation in the above said taxes till the completion of work will also be to contractor s accounts. 26. It may be noted that the entire staff and labor of the contractor will follow all the security instructions issued by the Institute from time to time and these instructions may include the provisions to make suitable barricades or temporary wall fence to ensure that the labor confines itself to the area of the work only. Serious actions will be taken in case any of the contractor s workmen are found to be tampering with the Laboratory equipment and the property of the Institute. Suitable damages will be recovered from the contractor s bills if anyone is found to do so. All decisions of the Engineer-in Charge in respect of the same will be final and binding on the contractor. 27. Clearance of Site on Work Completion: As a part of the work under this contract, the contractor shall restore the territory embraced within the site of his operations to at least as good order and condition as at the beginning of the work under this contract. 28. The Contractor shall ensure compliance of all statutory laws and bye-laws of the Central Govt./ State Govt. / Municipal authorities related to the employment of their staff and all such obligations under Wage Act. Workmen compensation Act, ESI Act, provident Fund, Miscellaneous provision Act, Bonus Act, contract Labor Act 1971, etc. THSTI will not be responsible for such matters in any way. Copy of all such documents including electrical license shall be submitted along with the tender failing which the tender will summarily be rejected.

12 P a g e The Contract/agreement will be valid for a period of one year. The time allowed to commence the work is 10 days from the date of issue of letter of intent / award letter. In the event the contractor fails to commence the work, the award letter will be cancelled and EMD will stand forfeited. 30. The Contractor will provide Earnest Money of Rs. 1,00,352/- in the form of demand draft drawn in favor Translational Health Science and Technology Institute payable at Gurgaon , and payable Gurgaon. In case the contractor retiles from the offer within the validity of the tender, the Earnest Money will be forfeited. Similarly if the contractor fails to commence the work after issue of the letter of intent / award letter, the amount of earnest money will also be forfeited. After commencement of the work, the amount of earnest money will be refunded on receipt of Performance security in original. The offers of the contractor who do not deposit the earnest money in the above manner will summarily be rejected. Additionally on award of work, the contractor will submit performance security which will be valued for the contract duration. The value of performance security will be 5% of the deemed annual contract value and which will be refunded after successful completion of contract period. The performance security may be furnished in the form of an account payee Demand draft, fixed deposit receipt and bank guarantee from commercial bank. 31. Any claim dispute or difference arising out of or in connection with this agreement and which cannot be settled by mutual consultations, shall be referred to sole Arbitration or an Arbitrator to be appointed by mutual consultations. The award of the Arbitrator shall be final and binding between the parties to this agreement. The Arbitrator proceeding shall be governed by the Arbitration and conciliation ordinance dated 26 th March, 1996 and shall be conducted in Delhi. This agreement shall be subject to jurisdiction of the courts at Delhi. Engineer (IEE)

16 P a g e 16 ANNEXURE IV Terms & conditions for annual maintenance contract of 03 Nos. of DG set at 496 Udyog vihar Phase-III Gurgaon Haryana. Inclusive under AMC:- Scope of work for DG set 1. Total 12 monthly service visits from OEM authorized personnel shall be made during the contract period of 1 year. There will be no limit on emergency calls / complaints. 2. The contract shall cover all the routine service and labor charges for service and minor jobs like change of gauges / water pump / belts etc. Alternator brushes, relays of panel, AMF panel testing, rectify the fault (without parts). 3. All consumables going permanently into the installation will be supplied by THSTI and all other working consumables necessary for upkeep of equipment will be to your account. 4. Checking Lube Oil level before starting the engine. 5. Checking fan belt condition on weekly basis. 6. Minor leakages, which require only tightening of Nut / bolts and hose, pipe clips. 7. Checking and tightening of coupling bolts on regular basis. 8. Monitoring of Engine parameters like oil pressure and water temperature. 9. Immediate reporting of any major fault noticed during operation. 10. Maintaining the records of running hours in logbook provided by us. 11. Replacement of11 KV Pole fuses /HT fuses as and when required. Kindly note that when the DG Set taken up for servicing/breakdown maintenance, the contractor will provide a standby DG Set of minimum capacity of 50 KVA to provide power supply to emergency loads during that period. The charges of rental etc. for the same shall be covered under the scope of the contract and nothing extra will be payable on that account. In case non availability of the same, a per hour will be imposed on the contractor and, which shall be recoverable from the monthly running bill. Exclusion of this contract:- 1. Major jobs like overhauling of Engine shall be paid extra. Spares and consumables shall be paid extra. Any other repairs / machining / overhauling jobs, if required, to be carried out will be treated as extra. 2. All accessories like fuel injection equipment, starter motor, and battery charging alternator etc. shall be repaired / serviced by the service contractor of respective manufacturer, and charges shall be on actual basis. 17 P a g e 17 ANNEXURE-V SCOPE OF WORK OF CONTRACT:- Kindly note that the equipment located at 496 Udyog Vihar premises are operated outside of equipment warranty. The equipment located at 470/450 Udyog Vihar Phase-III are under warranty services till date. 01. All the equipment s covered under Annexure I, II &III shall be operated round the clock on 24 hrs. Basis or as per site requirement on all days of the month and year including Sundays & Holidays. 02. The manpower (electrician cum D.G. operator & A/C operator in each shift and supervisor & two helper in general shift) shall be provided and deputed by the contractor in each shift (each shift of 8 hrs.) in facility or as directed by the Engineer-incharge for each of the three premises, separately. Total minimum strength for all three premises. Supervisor - 03, Helper - 06, A/C Operator- 09, DG Operator- 09. Appropriate number of relievers Suitable no. of extra manpower as and when required. Technical qualification of staff deputed at site:- a) Supervisor should have Diploma/degree in Electrical Engineer/Mechanical Engineer or I.T.I in electrical/mechanical trade with 10 years experience. b) Electrician cum D.G set operator should have I.T.I in electrical/mechanical trade or should have 7 years experience. c) A.C operator cum mechanic should have I.T.I in Air-conditioning system or should have 7 years experience. 03. All repair activities on all equipment s located in the three premises will deemed to be recovered under the scope of this contract unless otherwise mentioned. 04. All the air-conditioning units, D.G set and equipment s covered under Annexure I, II & III shall be maintained neat and clean and in perfect working conditions. In event of failure of any of equipment, the contractor will mobilize the OEM Engineer for site inspection and repair methodology and submit a report for the same in writing to THSTI along with estimate. Subsequently maintenance action will be initiated by the contractor after

Labor and working consumables required for any repairs or minor works will be deemed to be covered under the quoted price. 05. Log book shall be maintained neat and clean as directed by Engineer-In-Charge and the reading shall be recorded on hourly basis. (Log books shall be provided by the Institute). 06. Servicing of condensers, filter, panels etc. shall be carried out as per the site requirement and also as and when required to maintain system health. Arrangement of pump, replacement of gaskets/packing etc., shall be the Contractor s responsibility at his own expanses. Only water for servicing shall be supplied by the Institute. 07. Re-wiring/fresh wiring work (internal & external), installation of lights, switch & sockets, MCCB, MCB and other electro-mechanical works require in the building will be done by contractor on as and when requirement basis as per the Instruction of Engineer-In- Charge. Extra payment will not be made for it under any circumstances. However, all materials will be supplied by Institute free of cost. 08. Any work not expressly mentioned in this document but is essential to operate and maintain the systems mentioned in this document will be defined to be a part of contract. ENGINEER IEE

19 P a g e 19 ANNEXURE VI 01. Following checks shall be carried out on daily basis and recorded in the log book of D.G. sets i. D.G set check list as per the original manufacturer or as directed by Engg-In- Charge. ii. iii. iv. Topping up of Engine oil, fuel, coolant and battery water in D.G sets. However lubricant oil, fuel, coolant and battery water will arranged & supplied by the Institute or contractor will supply on extra cost as approved by Engg-In- Charge. Loading and unloading of D.G sets spares consumables Tightness of V-belts of D.G set as well as air-conditioning units. v. Condition of fuses, starters, relays etc. in the electrical control panel. vi. Rectification of water leakage from the pump glands/valve glands. 02. The following checks shall be carried out in air-conditioning system once in every week and reported to the Engineer-in-charge in writing. Defect/Short coming if any shall be rectified immediately. i. Leak test for refrigerant. ii. iii. iv. Charge of refrigerant. Oil level in the Compressor. Working of all pressure gauges, refrigerant valves, solenoids high and low pressure switches, oil failure switch etc. v. Alignment of all compressor and pumps with respective driving motors at least once in six months. vi. vii. viii. Tightness of foundation bolts etc. Tightness of all terminals and leads in the electrical control panel to avoid overheating/failure including checking of all interlocks. The terminal box of every motor for tightness.

20 P a g e All the air-conditioning units covered under this contract shall be cleaned and checked once in every 15 days as follows. a) Cleaning of air filters. b) Cleaning of the entire air-conditioning units/d.g sets/ Panel/D.B body. c) Cleaning of the electrical control panel from outside as well as from inside and tightening the wires, cable, bus bar, ACB, MCCB and MCB terminals. Checking of fuses, heater and blower inter locks etc. d) Oiling/greasing the blower, exhaust fan and driving motor bearings. e) Checking the V-belts, pulley etc. 04. Cleaning of the cooling coil/condenser coil of split air-conditioning/precision airconditioning units using chemical and water, jet vacuum-cleaner shall be used in places where drain line is not available. 05. During the course of operation and maintenance, if any spare part is felt necessary for replacement., the contractor shall give the requisition for such spare part with full details and nomenclature in writing at least 15 days in advance failing which it will be responsibility of the contractor to replace the part immediately which will be paid by the Institute at market rate in due course. 06. The Contractor shall prepare a list of spare parts required for all the plants and equipment s and submits to the Engineer-in-charge within one month from the date of commencement of work under this contract.
