1 Zurich International Life Medical providers in the Middle East March 2018 Brokers and Banks who have terms of business with Zurich are authorized to set-up appointments for medical examinations for their customers. Tests can only be arranged with medical clinics and hospitals within this Zurich Panel. To book an appointment for the customer: Complete the Medical request form and it to the preferred clinic. The clinic will confirm the appointment request. Inform your customer to prepare for the tests. (e.g. fasting for blood tests, walking/running shoes for effort ECG) Advise the customer to carry a photo identification document (valid government issued ID card, or driver s license) to the medical appointment. The results of medical tests can take between one or two working days to reach Zurich. Top tip: Mention the name and location of the clinic, the date and time the medical appointment is scheduled on the application health and lifestyle questionnaire, so that our underwriting team can follow up for the test reports.
