8 AFRO Essential Medicines Price Indicator INTRODUCTION Millions of people in Sub-Saharan Africa continue to experience a heavy burden of infectious diseases and increasingly afflicted with chronic non-communicable diseases and these in turn strain the already over-stretched national health budgets. People continue to die although effective medicines exist. Over 50% of the population in some parts of Africa has no regular access to essential medicines. Even though inefficiencies in the allocation and use of meager resources still exist, many governments are undertaking health reforms. One of the key objectives of these reforms is to improve efficiency without reducing the quality and quantity of services. Improving the management of health resources can help achieve this objective. Medicines are also fundamental to the good performance of the health care delivery system. The availability of medicines in health facilities, as well as the public s access to them, contributes to maintaining the population s confidence and trust in the health care delivery system. Better management of the available medicines contributes to a better health care delivery system. Efficient procurement will greatly contribute to achieving this goal. In 1999, the forty-ninth Session of the Regional Committee for Africa endorsed the Intensified Essential Drugs Program for the African Region. One of the components of this program was information exchange to improve policy implementation and containing cost. Containing cost through efficient procurement of medicines requires good knowledge of the prevailing prices of medicine at national and international pharmaceutical markets. This knowledge - market intelligence - is critical when comparing prices before and during public tendering through international bidding or from pre-selected suppliers. We hope that the present fourth edition of the AFRO Essential Medicines Price Indicator will be a useful tool for Member States in their attempts to improve procurement efficiency in order to achieve better prices and improve affordability of essential medicines. In order to prepare the current edition of the price indicator, electronic data collection form was sent to selected international generic medicines supply agencies and to all the 46 Member States of the African Region. The data collection form was based on the Model list of Essential Medicines revised March The information requested included package size, package price, unit price; the type of price quoted 1 (CFR, FOB etc) the vendor, the currency used for the tenders, the exchange rate in American dollars. It is important to bear in mind that the final landed price of a pharmaceutical product depends on a number of factors such as the tendering and payment procedures used, whether the products are directly supplied by the manufacturer or by an agent, the currency exchange rates, insurance costs as well as shipping and handling charges. The AFRO Essential Medicines Price Indicator therefore only provides an indication of the price of medicines at a given time. It cannot be used to place orders and it does not guarantee the product quality. Member States interested in making orders are therefore advised to contact the supply agencies for the latest list of products and price offers or the national procurement agencies to get the names of other supplies and their reliability. 10 AFRO Essential Medicines Price Indicator ACRONYMS AND DEFINITIONS Ex Works (EXW) Means that the seller delivers when he places the goods at the disposal of the buyer at the seller s premises or another named place (i.e. works, factory, warehouse etc.) not cleared for export and not loaded on any collecting vehicle. Free on Board (FOB) Means that the seller delivers when the goods pass the ship s rail at the named port of shipment. This means that the buyer has to bear all cost and risks of loss or damage to the goods from that point. The FOB term requires the seller to clear the goods for export. This term can be used only for sea or inland waterway transport. Cost and Freight (CFR) Means that the seller must pay the cost and freight necessary to bring the goods to the named port of destination. The risk of loss or damage to the goods after goods have passed the ship s rail in the port of shipment is transferred from the seller to the buyer. Consequently, the buyer will have to arrange the insurance. The CFR term requires the seller to clear the goods for export. This term can be used only for sea and inland waterway transport. Cost, Insurance and Freight (CIF). This term has exactly the same conditions and obligations as CFR. However, in CIF the seller also has to procure marine insurance against the buyer s risk of loss or damage to the goods during carriage (transport) to the named port of destination. The CIF term requires the seller to clear the goods for export. This term can be used only for sea and inland waterway transport. Cost and Insurance Paid (CIP). Means that the seller pays the cost of carriage necessary to bring the goods to the named destination. In addition the seller also has to procure insurance against the buyer s risk of loss or damage during carriage to the named place of destination. Consequently, the seller contracts the insurance and pays the insurance premium. The CIP term requires the seller to clear the goods for export. This term may be used irrespective of the mode of transport, but is generally used for transport by air, rail and road. Delivery, Duty Paid (DDP) Means that the seller delivers the goods to the buyer, cleared for import, and not unloaded at the named place of destination. The seller has to bear all costs and risks involved in bringing the goods thereto including duty for import in the country of destination. This term may be used irrespective of the mode of transport. Whilst the EXW term represents the minimum obligation for the seller, DPP represents the maximum obligation. viii

11 AFRO Essential Medicines Price Indicator METHODS FOR ANALYSIS AND COMPARISON OF MEDICINE PRICES NOMENCLATURE The World Health Organization s Model list of Essential Medicines revised March 2005 was the basis for the list of medicines in the present price indicator. Dosages are generally those included in the publication. Some products that are widely used and are supplied by a number of suppliers but are not in the model list are also included. The international nonproprietary name or the most common generic name has been used for these items. UNITS FOR COMPARISON The price unit in this document is the American dollar. The unit of measurement for products used to compare prices varies with the type of pharmaceutical preparation as follows: Pharmaceutical preparations Tablet Injectables Topical solid form Topical liquid form Suspension or oral liquid Eye, Ear Nose and throat Preparations Suppository Pessary Inhaler Comparison Unit TAB AMP or VIAL GM or TUBE ML or TUBE ML or BOTTLE ML SUPP PESS DOSE PRICES All prices have been converted to the US dollar and the average inter-bank exchange rates for the data collection period were used. The changes in the inter-bank rates can cause prices to vary widely. The most current rate at the time of the analysis should always be used when calculating medicine prices. It should be noted that direct comparison of prices between suppliers for a given product may be unrealistic because of the varying units of presentation and terms and services offered by different suppliers. Similarly units of presentation for some of the products in countries are not specified and vary; therefore direct comparison of prices may not be possible and realistic. Taking this into consideration, all attempts will be made to address this gap in the future edition. When calculating and projecting prices, it is important to consider factors such as delivery times, insurance costs, modes of transport (air/sea/land), handling charges. An analysis of these factors is beyond the scope and intent of the present price indicator. However, it is reasonable for estimation purposes to add 20-30% for shipping costs to FOB prices. Tender prices in CIF or DDP prices do not require adjustment for shipping charges. Defined Daily Dose (DDD) for some of the medicines listed in this edition of the price indicator was taken from the database of Collaborating Center for Drug Statistics Methodology. A DDD is defined as the statistical average daily dose of a medicine used for its main indication in adults. The DDD can be used to estimate consumption of medicines, estimate cost of treatment and when combined with price information, it can be used for budgeting purposes. However, it should be noted that DDDs should not be used for defining dosing schedules for specific indications and ix

For information on prescribing and dosage schedules, doctors and pharmacists should refer to other sources of information such as the Model Formulary. AVERAGE PRICES When using the price indicator for comparison with other prices, or for estimating the total value of a proposed procurement list, it is useful to use an average price. The average price is used primarily for budget estimates. The mean price and the median price are the two types of average prices that can be used. The mean price, the most commonly used average, is the simple average of all prices for a medicine: values are added up and divided by the number of values. The mean price is useful when medicine prices are relatively close and the same type prices are used (FOB, EXW etc.) The median price splits a set of values in half when a series is put in ascending order if there is an odd number of a value in the series. When there is an even number of values in the series, the median is calculated by averaging the two middle values. The median value is likely to be more useful in estimating prices (or comparing with local prices) when there is skewed distribution because, unlike the mean value, the median is not affected by outliers i.e. very high or very low values. COMPARISON PROCEDURE 2 Comparison of prices that a country has paid with prices paid by other countries is a useful exercise and can give an indication of the efficiency with which procurement is carried out. Other indicators of procurement efficiency are the accuracy of the needs estimation, timeliness of deliveries and quality of products procured. With the present document, it is possible to compare prices. The method below presents one approach to making such a comparison. Please follow these steps: 1. Make a list of products that you routinely procure in large volumes or which represent a high percentage of the budget; 2. For each product, list the vendor pack used and the most recent price for this vendor pack. Be sure to note whether the price is DDP, CIF, CFR, FOB, EXW or any other type of price. As discussed earlier, prices may need adjustment so that they are compared accurately; 3. Select the corresponding / equivalent product in this guide. If the prices you are using locally are DDP or CIF prices, then it is suggested that the FOB and EXW be adjusted upwards to reflect the difference between FOB and CIF or DDP. For FOB or EXW, it is reasonable to add 20% to 30% to the list price for freight and insurance, depending on the distance and mode of transport. Any standard percentage charged for handling should be added to the price. 4. Once the pack list has been adjusted to reflect freight, insurance and other additional costs, divide the pack price by the number of comparison units contained in the vendor pack. (For example, divide the pack price by 1000 if the pack is a tin of 1000 tablets, or by 100 if the pack is a tin of 100 tablets). Make sure that the comparison unit is the same as the comparison unit on the price list. 5. Because the packaging can add considerably to the cost of a product, caution is needed when comparing unit prices between different packages. 13 AFRO Essential Medicines Price Indicator An analysis of this type includes many assumptions that affect its outcome. An extremely high ratio between locally available prices and the prices in this price indicator may indicate that a less expensive source is available. If the locally available prices are much lower than the prices in this document, it might reflect effective procurement, but could also be a result of a compromise of quality control, differences in packaging or other factors that might reduce product quality. These comparisons are only indicators and should be viewed as pointers for further investigation. QUALITY ASSURANCE does not guarantee the quality of the products listed in this document. National procurement agencies should always request quality certificates of all products from suppliers. Medicine quality, like price and other technical information, is an important part of the tender evaluation process. In order to ensure good quality and conformity to tender specifications, the medicine should comply with standards of the national medicine regulatory authority of importing country. In addition, a -type Certificate issued by a competent national regulatory authority in the producing country and knowledge of the Good Manufacturing Practice (GMP) status of the producers help to ensure the quality of the tendered products. National regulatory agencies should always ensure, through regular inspection and laboratory testing, that medicines consumed in the public and private sector are effective, safe and of good quality. This price guide provides information on estimated shelf life and stability indicators for some of the medicines listed. However, specific information on shelf life (expiry date) and recommended storage temperatures provided by each manufacturer have to be taken into consideration and followed to maintain the quality of the product. xi
