To help you achieve these goals, we have detailed the following information in this guide: An overview of your benefits. Options for you and your family such as: - Hospital Plans - Accident Plans - Dental Plans - Life Insurance How to get discounts on your prescriptions. 3 Offer of Employer-Sponsored Health Coverage The Affordable Care Act created an Individual Mandate that requires all U.S. Citizens to have health insurance or pay a penalty. The employer-sponsored health coverage being offered to you and your dependents satisfies the Individual Mandate. You, as an eligible employee, and your dependents are being offered: (1) Minimum Essential Coverage, which covers Preventative and Wellness benefits; and (2) a bronze level plan which provides Minimum Value and for which you will be charged no more than 9.5% of your Box 1, W-2 wages for employee-only coverage (costs for dependents will vary). Availability of Summary Health Information As an employee, the health benefits available to you represent a significant component of your compensation package. They also provide important protection for you and your family in the case of illness or injury. Your plan offers a series of health coverage options. Choosing a health coverage option is an important decision. To help you make an informed choice, your plan makes available a Summary of Benefits and Coverage (SBC), which summarizes important information about any health coverage option in a standard format, to help you compare across options. The following documents are available to you in electronic form: (a) Enrollment Guide; (b) Summary of Benefits and Coverage; and (c) Summary Plan Description. The documents are available on the web at: Paper copies are also available, free of charge, by calling (a toll-free number). The webpage and documents can be accessed via any device that can access the internet, and no special software is required. This is your offer of Employer-Sponsored Health Coverage. 4 Telehealth Family Plan In addition to your Minimum Essential Coverage Plan, you will also receive a telehealth family plan. Your telehealth plan will give you access to doctors and licensed therapists anytime, from anywhere by video or phone. You will be able to avoid the hassle of trying to schedule appointments, the expense of rising healthcare costs and get help when you need it. Highlights of your telehealth family plan: 1. Speak or video chat with state licensed and U.S. board certified physicians and therapists at no cost to you. 2. Bilingual licensed physicians and therapists available on-demand 24/7/ Get a needed prescription and pick it up at the pharmacy of your choice. Common conditions treated by your telehealth family plan: Flu symptoms Ear infections Headaches Mild lacerations Dehydration Cough Constipation Rashes Joint Aches Insomnia Bronchitis Diarrhea Nausea Itchy eyes Skin infections Abrasions Sinus infections Urinary tract infections Allergies Sore throats Insect bites & stings Nasal congestion Lice Vomiting Acne Respiratory infections and many more! 5 Discount Prescription Card Prescription drugs are often very expensive and you may be struggling to pay high prices or insurance deductibles. Whether the prescription is for a chronic condition or for short-term illness, the cost can sometimes be a barrier for you to regain your best health. With your Discount Prescription Card included in your Benefits plan, you can receive discounts off of prescription medicines for you and your family. 6 Available Voluntary Benefits During your FreedomCare Open Enrollment period, you have the ability to enroll in numerous voluntary products to keep you as healthy as possible. When you call, an Enrollment Representative will discuss your individual needs to find the best fit for you and your family. For your convenience, the FreedomCare Enrollment Center is available Monday Friday from 9am-9pm (EST) at Sample plans with rates* Accidental Injury & Disability $ Emergency room, inpatient, ICU benefits and more. Critical Illness $ Lump sum benefits from $10,000 to $70,000 at the first diagnosis of cancer, heart attack, stroke or renal failure. Term Life Insurance $ No required physicals or blood testing, coverage available from $10,000 to $50,000 Dental Insurance $ Includes a wealth of preventative services including 2 cleanings a year. Vision Insurance $ Includes a vision exam every 12 months, low cost lens copays and frame allowance every 12 months. *These are sample monthly rates for illustrative purposes only. Your actual rates may be higher or lower. Spouse and Dependent coverage available, prices vary. Call to speak with your enrollment representative. 7 Member Dashboard After you receive your Member ID card, you will be able to access details regarding your benefits using your secure online Member Dashboard. Please note that you are able to access the Summary of Benefits and Coverage for the Minimum Essential Coverage plan by: Contacting your Human Resources person Calling the Enrollment Center to request a paper copy View them online at freedomcarebenefits.com To access your account and Member Dashboard, you will need to create an account using your Member ID card.
