2 Terms & Conditions Participants must not be suffering from any medical condition, illness, disease, mental illness or physical / psychological impairment. Players are not advised to participate if they are feeling unwell. In such cases, please inform the event organizer and match officials immediately. NTUC Club, U Sports and its agent reserve the right to change the content of the event with or without prior notice. Due to limited places available, only registrations with full payment made are deemed as confirmed on a first-come-first-served basis. Payment received is NON-REFUNDABLE. All registered participants for this event will be contacted via or telephone for further confirmation of their particulars and other reservation details. Each player can only represent ONE team. Managers / Captains shall undertake the responsibility of ensuring the health and well-being of the players competing in this Tournament. All participants MUST bring along their NTUC Membership Card / Union Card or nebo Card along with NRIC Card upon Registration for verification by Tournament Officials. Participants who are below 18 years old are required to submit a separate parental/ guardian consent form each (which can be found below) together with the team s registration form. Failure to submit the signed parental/ guardian consent form will result in disqualification. Only players listed in the registration form are eligible to participate in the competition. Changes to the line-up will not be accepted once it has been submitted to the presiding Tournament Official on the actual event day / date. Players shall comply with the instructions and decisions of the event organizer and match officials. Any protest must be made on an Official Protest Form on the actual event day. A non-refundable protest fee of S$50 must be paid along with the protest form by the Captain. All prizes are not exchangeable for cash and are not transferrable. Any changes / amendments / decisions would be made at the sole discretion of the Organising Committee. Each category requires a minimum of 10 participating teams to proceed. Upon closing date, if the minimum no. of participating teams is not achieved in the respective categories, the sole discretion of the Organising Committee to scrap the category will be undertaken. Submission of the official registration form after the captain briefing would not be included for the Tournament Draw. Any withdrawals of the participating team must be made before the Captain s Briefing unless otherwise stated. All participants are covered under Accidental Medical Expenses Insurance capped at $250 for injuries sustained at the U Games 14/15 event On the event day only Upon closing date, if a minimum no. of participating teams is not achieved in the respective categories, the sole discretion of the Organizing committee to scrap the category will be untaken T&C of nebo membership: 1-year renewal is only applicable to existing nebo members with less than 1-yr membership validity from date of event. 2-year new membership is applicable to new members only. By signing below, you consent to the collection, use and/or disclosure of your personal particulars by NTUC Club for the purposes of organising this event, and/ or uses by NTUC Club for analysis and research purposes pertaining only to matters in relation to such events/ programmes. All registered participants will be included into the contact list of NTUC Club, to receive regular updates on future engagement events and programmes organised by NTUC Club, as well as opportunities to contribute, share opinions and/or feedback through any of the contacts provided through any means. Any photographs, motion pictures, recordings, or any other media records of this event may be used by NTUC Club for any legitimate purpose, including any commercial and marketing uses, and/ or to be uploaded onto NTUC Club s public sites and channels. In the event that registered participants do not wish for his/her photographs and/ or any other media records of him/ her at this event to be used by NTUC, please inform and provide clear instructions to U Sports in writing at info@usports.com.sg In the event that registered participants do not wish to receive regular updates on future engagement events and programmes organised by NTUC Club, as well as opportunities to contribute, share opinions and/ or feedback, please inform and provide clear instructions to U Sports in writing at info@usports.com.sg, within 14 days of the event. U Sports will consequently be unable to inform you of subsequent engagement events.
