1 CLAYTON STATE UNIVERSITY STUDENT HEALTH INSURANCE PLAN
2 The University System of Georgia is pleased to be able to offer their students affordable, benefit rich Student Health Insurance Plans (SHIP). While some students are required to be enrolled in a health insurance plan and are required to have insurance that meets the minimum standards, optional enrollment in SHIP is also available to all students, their spouse and their dependents. VOLUNTARY SHIP ENROLLMENT Go to Click the "Enroll Now" link in the Enroll Online Now! section in the upper right corner of this page. Online enrollment takes only minutes, pay via credit card or echeck.

4 THESE STUDENTS MUST HAVE HEALTH INSURANCE
5 All undergraduate, and ESL international students holding F or J visa status. All undergraduate students enrolled in programs that require proof of health insurance. All graduate students receiving a Full Tuition Waiver as part of their graduate assistantship award. All graduate international students holding F or J visas status. All graduate students enrolled in programs that require proof of health insurance. All graduate students receiving fellowships that fully fund their tuition. International Scholars holding J visa status. These students are required to have student health insurance that meets the minimum standards set by the University System of Georgia. Students in these categories who are not covered by a policy held by a parent, spouse, employer, or if their policy does not meet the minimum standards, will be charged for a USG Student Health Insurance Plan (SHIP) policy.
6 STUDENTS MAY REQUEST A WAIVER Students who fall into one of these categories and are already covered by an insurance policy (i.e. through a parent, family, or employer-sponsored plan) can easily opt out of the SHIP plan through a secure on-line waiver request process through UHC. Once the policy information has been verified and is determined to meet the minimum standards, all SHIP charges on their student account will be waived. Students (both undergraduate and graduate) who fail to submit creditable health insurance information will automatically be enrolled in and billed for the system-wide student health insurance plan. The waiver periods at all USG institutions are strictly enforced.
7 WAIVER PERIOD Students that have the SHIP charge, but have their own insurance, should submit their waiver requests as soon as that option becomes available. Fall period is August 1- Sept 15 Spring/Summer is Dec 1 Feb 15
8 Submit WAIVER Request If they are currently covered by a comparable plan, students have the option to complete a Waiver Form to request a review of their existing coverage. If it is determined that the coverage meets United Healthcare minimum coverage standards, the student will not be required to enroll in the Clayton State University s student insurance plan. To request a review of their existing coverage: Go to Select Students Applying for Insurance Waivers Select Waiver Form Located in the middle of the page Submit requested information. The review process could take up to 10 business days. Students must submit a waiver request every semester For Fall covering August 1 December 31 For Spring/Summer covering January 1 July 31
9 If UHC approves the waiver, a SHIP charge exemption will appear on their CSU student account in approximately 3 business days.
10 ENROLLMENT Students that have the SHIP charge and need the health insurance coverage, do not have to wait for the default coverage that begins after the waiver period ends. Students can confirm enrollment and receive their insurance card early. After enrolling, they will receive an from UHC with instructions on how to print their health insurance card. Please follow these steps to enroll immediately: Go to Select Students Confirming Your Enrollment Select Enrollment Form Located in the middle of the page below Waiver Form Submit requested information. Students must submit enrollment every semester they receive the SHIP charge.
