1 REQUEST FOR QUOTATION (RFQ) Sidewalk Trip Hazard Remediation Concrete Sidewalk Shaving A. INTRODUCTION The is accepting quotations for the remediation of concrete sidewalks in Kindersley, Saskatchewan. Quotations for completing this service for the are currently being accepted until: Closing Date: June 28, 2016 B. SCOPE OF WORK / SERVICE REQUIRED Services must include the remediation of concrete sidewalk trip hazards from 1/2 and up to 2 in pre-designated work areas within the community of Kindersley. Remediation services must remove trip hazards completely, from one end of the raised sidewalk joint to the other, if applicable, leaving zero point of differential between concrete slabs. Any panels inspected and deemed not to be at a zero point of differential will be repaired to the required specifications. Remediation service may not cause any damage to landscaping, retaining walls, curbs, sprinkler heads, utility covers or any other objects adjacent to sidewalks. The service provided must repair each sidewalk trip hazard without damage or visible markings to adjacent slab(s) or curb(s). Equipment / Process Required Contractor must use a horizontal saw cutting technique that guarantee s a planar surface after cutting. Contractor must guarantee specified repair slope (1:8 or 1:12 based upon requirements outlined by the Town is achieved. If defined slope is not achieved, contractor must repair to specification at no additional charge within 24 hours of discovery. No water-cooling is allowed which creates slurry and contaminates storm drains and causes excessive environmental impact. Contractor must use a dust abatement system that limits the dust emitted from the cutting. Contractor must guarantee that the removed trip hazard will have a uniform appearance and texture. The finished surface shall have a coefficient of friction of at least 0.6 and show via previous testing that the technique used yields the specified coefficient of friction. The hardware must be flush to the ground and capable of working at any angle and perform trip hazard removal in hard-to-reach areas, around obstacles, on narrow walkways, next to fences and retaining walls or buildings. Grinding or pulverization of the concrete is NOT acceptable or allowed, causing micro-cracks or weakening the concrete panel. Contractor s trip hazard repairs may not leave ridges or grooves that could hold water and prevent drainage of rain water or irrigation. Sidewalk repair equipment and all other items incidental to the work shall not be left or stored on the sidewalk or on private property while not in use.

2 A dust abatement system must be used to limit the dust being emitted. All clean-up services must be included in the quotation price and may not be charged at an additional rate. All clean-up will consist of full clean-up of all debris after each concrete shaving is performed. All costs incurred for disposal of waste material shall be included in unit cost and not paid for separately. Data Collection A data collection device that records the details of the work and transmit the data electronically. The recording device must be able to record sizes of trip hazards, gps coordinates, hazard locations plotted to a map, address, and also provide this data electronically in order to integrate into a GIS system. Contractor shall submit a detailed invoice setting forth the services performed in accordance with the formula for saw cutting calculations. All invoices must show the cut depth, size, length, width and address location. Further Specifications Total quantity of trip mediation for sidewalks for 2016 is approximately 3000 to 3500 linear foot. Please ensure pricing is set forth for this quantity of work. The will randomly inspect repairs to ensure the repair specifications are being met. If less than 95% of the inspected repairs meet the repair specifications, further inspection will be completed to determine the actual quantity of failed repairs. The contractor at no cost to the Town is responsible to fix any failed repairs. The will not pay for services that do not meet the repair specifications set out in this RFQ. If contract is awarded, the successful proponent must complete remediation trip hazard services prior to September 15, C. SUBMISSION REQUIREMENTS All Submissions should be submitted following the below format: STEP 1 Provide Business Information Provide detailed background information and business description. Be sure to include the following: 1. Name of company 2. Information on owner(s) and key manager 3. Business Location(s) Include head office location and any satellite office location(s) 4. Business History A brief history of the organization including years of operation. This must include examples of past work practices identifying past and/or current projects of similar scope. Experience for trip hazard remediation as asset.

3 STEP 2 PREPARE AND SUBMIT QUOTATION 1. Trip Mediation Process Describe your company s process as outlined in the scope of work listed in this RFQ 2. Cost for Services Provide your cost to complete this service 3. References Provide a minimum of three (3) references detailing past concrete remediation projects. Disclose any actual or potential conflicts of interest and existing business relationships you may have with the, its elected or appointed officials or employees. D. EVALUATION OF RFQ The RFQ will be selected on the following criteria: 1. Experience Years of Service Work in mediation Services 2. Process/techniques/equipment 3. Availability/timing 4. Cost of service 5. Warranty/Guarantee to achieve repairs following Town requirements as specified in the RFQ 6. Proponents finding discrepancies or omissions in the RFQ, or having doubts as to the meaning or intent of any provision,

4 should immediately notify the representative. If the Town of Kindersley determines that an amendment is required to this RFQ, the Town of Kindersley representative will issue an addendum. No oral conversation will affect or modify the terms of this RFQ or may be relied upon by any proponent. An agreement will be drafted in consultation with the preferred proponent(s) which will contain the relevant provisions of this RFQ, the accepted submission, as well as such other terms as may be mutually agreed upon, whether arising from the accepted submission or as a result of any negotiations prior or subsequent thereof. If at any time the reasonably forms the opinion that a mutually acceptable agreement is not likely to be reached within a reasonable time, give the preferred proponent(s) written notice to terminate discussions, in which event the Town of Kindersley may then either open discussions with another proponent or terminate this RFQ and retain or obtain the services to develop the project in some or any other manner. 7. Prior to commencing work, the successful proponent shall obtain all authorizations required by the law to enable them to carry on business and to complete work required by the agreement, such as Saskatchewan GST requirements and a business license. 8. Proponents are solely responsible for their own expenses in preparing, and submitting quotations, and for any meetings, negotiations or discussions with the or its representatives and consultants, relating to or arising from this RFQ.
